Recent advances in the method of application and monitoring of Electroconvulsive therapy (ECT) has facilitated its use even in the presence of serious physical illness. This case report b of a patient in catatonic stupor who developed an acute respiratory crisis, and in whom the use of ECT led to quick recovery not only from the psychiatric state but also helped recovery from the medical illness. The report highlights the need to use ECT, if indicated, in the presence of serious medical illness without hesitation.

The prognosis of twenty consecutive patients with acute spontaneous putaminal haemorrhage was studied using clinical and CT scan findings. Patients with haemorrhage of less than 4.5 cms were mostly conscious and had independent activity of daily living (ADL) at the end of 3 months; between 4.6 cms and 6 cms., they were mostly drowsy and had partly independent ADL; above 6 cms., they were in stupor or in coma with a high incidence of mortality. Early surgical intervention is advised in the last group.

A 70-year-old hypertensive woman suffered sudden onset of bilateral blepharoptosis. Within a few hours she developed focal signs attributable to a lesion of the mesencephalon, and a stuporous state, from which she did not recover. Neuropathological examination showed a mesencephalic haematoma, but no arteriovenous malformation. A review of reported cases suggests that mesencephalic haematomas have a worse prognosis when they occur in aged hypertensive patients, as compared with younger normotensive patients, in whom they are probably related to arteriovenous malformations.

A 50-year-old driver was arrested in a drunken stupor while he was driving a track. He suddenly died after five days of the arrest in a jail. As time went by, various symptoms of alcohol withdrawal appeared. He was in the state of delirium treatments for about a day before he died. Gross and microscopical examination revealed fibrosis and fatty degeneration of liver and heart lesions representing chronic and acute ischemia. We discussed the cause and the mechanism of the death in the case and reviewed those in the previous observations.

The Authors propose a new approach to the treatment and prognostic evaluation of post-traumatic supratentorial acute epidural haematoma (PSAEH). As far as this lesion is concerned a group of patients with a favourable prognosis and without indications for a surgical treatment may be identified. To this purpose, the clinical picture, characterized by a slight symptomatology undergoing a regression phase (stupor, headache, etc.) and a skull CT-scan, characterized by a slight shift of the middle line structures and by the volume of haematoma less than 55 cc, are important aids.
