Exposure therapy for social anxiety disorder in people who stutter: An exploratory multiple baseline design.

BACKGROUND & OBJECTIVES: Social anxiety disorder (SAD) is a debilitating condition, and approximately half of adults who stutter have SAD. Cognitive-behavioral therapy (CBT) has shown promise in decreasing social anxiety symptoms among adults who stutter, but exposure, arguably the essential component for successful CBT for SAD, has been understudied and underemphasized. Aims of this study were to develop an exposure therapy protocol designed specifically for people who stutter and have SAD and evaluate its potential efficacy in reducing social anxiety and stuttering severity using a multiple …

The Coexistence of Disabling Conditions in Children Who Stutter: Evidence From the National Health Interview Survey.

PURPOSE: Stuttering is a disorder that has been associated with coexisting developmental disorders. To date, detailed descriptions of the coexistence of such conditions have not consistently emerged in the literature. Identifying and understanding these conditions can be important to the overall management of children who stutter (CWS). The objective of this study was to generate a profile of the existence of disabling developmental conditions among CWS using national data. METHOD: Six years of data from the National Health Interview Survey (2010-2015) were analyzed for this project. The samp…

Utility of virtual reality environments to examine physiological reactivity and subjective distress in adults who stutter.

PURPOSE: Virtual reality environments (VREs) allow for immersion in speaking environments that mimic real-life interactions while maintainin … from 10PWS during four-minute speeches to a virtual audience and to a virtual empty room. RESULTS: Stuttering frequency and physiological measures (skin conductance level and heart rate) did not differ across speaking conditions, but subjective ratings of distress were significantly higher in the virtual audience condition compared to the virtual empty room …

Phase II trial of a syllable-timed speech treatment for school-age children who stutter.

PURPOSE: A recent clinical trial (Andrews et al., 2012) showed Syllable Timed Speech (STS) to be a potentially useful treatment agent for the reduction of stuttering for school-age children. The present trial investigated a modified version of this program that incorporated parent verbal contingencies. METHODS: Participants were 22 stuttering children aged 6-11 years. Treatment involved training the children and their parents to use STS in conversation. Parents were also taught to use verbal contingencies in response to their child's stuttered and stutter-free speech and to praise their child…

Nonword repetition and phoneme elision skills in school-age children who do and do not stutter.

Nonword repetition and phoneme elision represent the combined influence of several speech and language processes. In the present study we investigated nonword repetition and phoneme elision performance in school-age children who stutter (CWS) and children who do not stutter (CNS). Participants were 14 CWS (mean = 11.7 years, SD = 2.1 years) and age- and sex-matched CNS (mean = 11.8 years, SD = 2.0 years). Each talker group was further subdivided into two age groups: younger (N = 7; 8-11.5 years) and older (N = 7; 11.6-15 years). Repeated-measures analyses were conducted on the accuracy and res

Syllable-timed speech treatment for school-age children who stutter: a phase I trial.

PURPOSE: This clinical trial determined the outcomes of a simple syllable-timed speech (STS) treatment for school-age children who stutter. METHOD: Participants were 10 children, ages 6-11 years, who stutter. Treatment involved training the children and their parents to use STS at near normal speech rates. The technique was practiced in the clinic and at home with the parents during everyday conversations. RESULTS: Nine months after commencing treatment, stuttering had decreased by >50% for half of the children, with 2 children attaining 81% and 87% reduction. Intention-to-treat analysis sho…

Using syllable-timed speech to treat preschool children who stutter: a multiple baseline experiment.

UNLABELLED: This report presents the results of an experimental investigation of the effects of a syllable-timed speech treatment on three stuttering preschool children. Syllable-timed speech involves speaking with minimal differentiation in linguistic stress across syllables. Three children were studied in a multiple baseline across participants design, with percent syllables stuttered (%SS) as the dependent variable. In the week following the initial clinic visit, each child decreased their beyond-clinic stuttering by 40%, 49% and 32%, respectively. These reductions are only evident in the t…

A randomized control trial to investigate the impact of the Lidcombe Program on early stuttering in German-speaking preschoolers.

UNLABELLED: In order to investigate whether the Lidcombe Program effects a short-term reduction of stuttered speech beyond natural recovery, 46 German preschool children were randomly assigned to a wait-contrast group or to an experimental group which received the Lidcombe Program for 16 weeks. The children were between 3;0 and 5;11 years old, their and both of their parents' native language was German, stuttering onset had been at least 6 months before, and their stuttering frequency was higher than 3% stuttered syllables. Spontaneous speech samples were recorded at home and in the clinic pri…

The effects of temporal modification of second speech signals on stuttering inhibition at two speech rates in adults.

The recovery of 'gestural' speech information via the engagement of mirror neurons has been suggested to be the key agent in stuttering inhibition during the presentation of exogenous second speech signals. Based on this hypothesis, we expect the amount of stuttering inhibition to depend on the ease of recovery of exogenous speech gestures. To examine this possibility, linguistically non-congruent second speech signals were temporally compressed and expanded in two experiments. In Experiment 1, 12 participants who stutter read passages aloud at normal and fast speech rates while listening to s…

How effective is therapy for childhood stuttering? Dissecting and reinterpreting the evidence in light of spontaneous recovery rates.

BACKGROUND: Similar positive results (e.g. immediate decreases in stuttering frequency and a 60-80% recovery rate from stuttering) have been reported for numerous therapeutic protocols for treating childhood stuttering, many of which have been diametrically opposite in their orientations and implementations. For example, Johnson advocated indirect treatments that simply advocated refraining from drawing any negative attention to childhood disfluencies as persistent and chronic stuttering was thought to progress via negative parental reactions to normal disfluencies. In contrast, direct interve…

A novel use of choral speech significantly reduces stuttering in a simulated presentation setting: An exploratory study.

Choral speech, defined as speaking in unison with another voice, has long been recognised for improving speech fluency while maintaining spe … opping from 10 % at baseline to below 3 % in both choral speech conditions. One participant reduced stuttering from 49 % to 2 %, and shortened reading duration from approximately 19 to 4 min with the TTS decoder, while improving perceived speech naturalness. To the author's knowledge, these preliminary results are first to suggest that choral speech with a T …

Treatment of obstructive sleep apnea relieves stuttering priapism in patient with sickle cell disease: a case report.

BACKGROUND: Stuttering priapism is recurrent, self-limited episodes of sustained penile erection and is common in patients with sickle cell disease (SCD). Prevention of stuttering priapism is important to avoid progression to episodes of ischemic priapism which can cause erectile dysfunction. Priapism has been shown to be associated with increased nocturnal hypoxemia in patients with SCD. CASE DESCRIPTION: A 43-year-old male with nocturnal episodes of stuttering priapism that was refractory to treatment with multiple medications was found to have obstructive sleep apnea (OSA). Following treat…

Turkish adaptation of the self-stigma of stuttering scale (4S): Study of validity and reliability (4S-TR).

PURPOSE: This study aimed to adapt the Self-Stigma of Stuttering Scale (4S) into Turkish and evaluate its factor structure, reliability, and validity in Turkish culture. METHODS: The original 4S scale was translated into Turkish (4S-TR) using a forward-backward translation technique and was administered to 350 adults who stutter (AWS). To discover latent variables evaluated on the scale, two-factor analyses were performed. Internal consistency and temporal stability were calculated to ensure reliability. Test-retest reliability correlation scores were calculated with multiple applications of …

Episodic stuttering as the presenting manifestation of acute ischemic stroke: A case report and systematic literature review.

BACKGROUND: Acquired episodic stuttering in adulthood represents a rare condition, which has been infrequently described in the literature. CASE PRESENTATION: We describe the case of a 62-year-old male who presented to the emergency room with three episodes of new-onset brief isolated stuttering with no other speech impairment or associated focal neurologic deficits. His brain magnetic resonance imaging was notable for the presence of a small acute ischemic stroke involving the left precuneus cortex. SYSTEMATIC LITERATURE REVIEW: We performed a systematic literature review to evaluate the as…

Human GNPTAB stuttering mutations engineered into mice cause vocalization deficits and astrocyte pathology in the corpus callosum.

Stuttering is a common neurodevelopmental disorder that has been associated with mutations in genes involved in intracellular trafficking. However, the cellular mechanisms leading to stuttering remain unknown. Engineering a mutation in N-acetylglucosamine-1-phosphate transferase subunits α and β (GNPTAB) found in humans who stutter into the mouse Gnptab gene resulted in deficits in the flow of ultrasonic vocalizations similar to speech deficits of humans who stutter. Here we show that other human stuttering mutations introduced into this mouse gene, Gnptab Ser321Gly and Ala455Ser, produce the …

Personal Perceptions and Perceived Public Opinion About Stuttering in the United States: Implications for Anti-Stigma Campaigns.

PURPOSE: This exploratory study was the first to obtain quantitative and qualitative data on both personal perceptions and perceived public opinion about stuttering in order to identify topics to include in anti-stigma programs for stuttering. METHOD: Three-hundred ten adults in the United States completed a web survey that assessed knowledge about stuttering and attitudes toward people who stutter (PWS) with questions addressing personal perceptions (direct questions) and perceived public opinion (indirect questions). RESULTS: Many participants reported favorable personal perceptions of PWS…

Follow-up of 6-10-year-old stuttering children after Lidcombe program treatment: a phase I trial.

PURPOSE: This Phase I trial sought to establish (1) whether the Lidcombe Program is viable for school-age children, (2) whether there is any indication that it requires modification for school-age children, (3) whether treatment effects are durable, (4) how many treatment sessions appear to be required to significantly reduce stuttering frequency and (5) whether there is an association between follow-up period and relapse tendency. METHOD: Twelve children were treated, and one required an addition to the Lidcombe Program. The results for this child were excluded from group analysis, leaving a…

Changing perspectives for practice in stuttering: echoes from a Celtic past, when wordlessness was entitled to time.

Changing perspectives for practice in stuttering therapy are informed by the changes in knowledge, social values, and belief systems of a society. The International Classification of Functioning, Disability and Health (ICF; World Health Organization, 2001) has a sociological emphasis with a focus on the ability and functioning of the person, and it is currently fostering changes in perspectives for working with those who stutter. These perspectives are reflected in Irish social mores in the 7th and 8th centuries, when social and legal codes enshrined the rights of people with speech disabiliti…
