Urethral distortion after placement of synthetic mid urethral sling.

PURPOSE: We retrospectively compared preoperative characteristics in patients with new onset lower urinary tract symptoms after receiving a mid urethral sling who did (group 1) and did not (group 2) have radiographically confirmed urethral distortion during voiding. MATERIALS AND METHODS: After receiving institutional review board approval we reviewed the charts of patients who underwent mid urethral sling excision for new onset lower urinary tract symptoms between January 2003 and July 2009. We collected demographic and questionnaire data, including the Urogenital Distress Inventory, Inconti

Conservative management as an initial approach for post-operative voiding dysfunction.

OBJECTIVES: The management of voiding dysfunction after tension-free vaginal tape (TVT) remains controversial. Timing of surgical intervention is debatable and the role of conservative management is not well researched. This study aims to determine whether self catheterisation is an effective first line management option for these patients. STUDY DESIGN: 389 women underwent a TVT operation over a five year period. Twenty-two (5.6%) developed post-operative voiding dysfunction. Twenty women commenced CISC and their progress was monitored by recording voided volumes and residual urine volumes i

Dyspareunia associated with paraurethral banding in the transobturator sling.

OBJECTIVE: We sought to compare the development of paraurethral banding and subsequent dyspareunia in women undergoing either a transobturator (TO) or retropubic (RP) sling. STUDY DESIGN: We conducted a retrospective cohort study comparing women treated with either a TO or RP sling during a 10-month period. RESULTS: A total of 25 TO sling patients and 28 RP sling patients were compared in the study. Paraurethral banding was observed in 13/25 (52%) of the TO group compared with none in the RP group (P<.001). Although no difference was noted in overall female sexual function index scores, de n

Atypical graft infection presenting as a remote draining sinus.

BACKGROUND: Synthetic materials are being used increasingly in reconstructive pelvic surgery. Multifilament polypropylene mesh in particular has been associated with healing abnormalities and other postoperative complications. This article describes an atypical infection presenting as a draining sinus tract to the lower extremity after intravaginal slingplasty. CASE: An otherwise healthy 75-year-old-woman presented with recurrent leg cellulitis 18 months after posterior intravaginal slingplasty for vaginal vault prolapse. A 35-cm fistulous tract draining from the pelvis to the lower thigh was

Haemorrhage and nerve damage as complications of TVT-O procedure: case report and literature review.

The transobturatorial vaginal tape procedure is associated with little complication rate. Bladder perforation, urethral invasion, vaginal erosion, postoperative bladder retention, de novo incontinence and retropubic pain and haematoma are the most reported complications. The aim of this article is to present an uncommon complication in a patient operated by transobturator vaginal tape with an inside-outside route.

The value of cystoscopy and bladder biopsy taken at the time of tension-free vaginal tape insertion.

The primary aim of this study was to determine if a cystoscopy performed at the time of a tension-free vaginal tape (TVT) insertion is useful in finding unexpected pathology. A secondary outcome measure was to assess whether a bladder biopsy and cystoscopy performed during the TVT operation provide useful information about the outcome of surgery. The findings in 100 consecutive women were reviewed. A total of 42 women demonstrated mixed detrusor overactivity (DO) and urodynamic stress incontinence (USI); 32 women had abnormal biopsies. Women with DO were no more likely to have abnormal bladder

Changing Trends in the Surgical Management of Stress Urinary Incontinence in Australia.

BACKGROUND: In the late 2000s, mid-urethral slings (MUS) emerged globally as the preferred surgical treatment for stress urinary incontinence (SUI). Despite their high safety and efficacy, through the late 2000s and 2010s, MUS became embroiled in widely publicised controversy surrounding mesh products used for pelvic organ prolapse. AIMS: To review recent trends in the surgical management of SUI in Australia in the context of recent medicolegal and media attention on transvaginal mesh devices. MATERIALS AND METHODS: Data were obtained from the Australian Government Department of Human Servic

Vaginal Mesh Exposure After Single-Incision vs Full-Length Midurethral Sling Placement: A Retrospective Cohort Study.

INTRODUCTION AND HYPOTHESIS: Midurethral slings (MUS) are the standard surgical treatment for stress urinary incontinence, but real-world data comparing vaginal mesh exposure risk across sling types are limited. We hypothesized that sling type, single-incision versus full-length, would be independently associated with vaginal mesh exposure and postoperative complications. METHODS: We conducted a retrospective cohort study of 369 women who underwent MUS placement by a single fellowship-trained Urogynecology and Reconstructive Pelvic Surgeon at a tertiary care institution from 2017 to 2023. Results of Stress Incontinence Surgery in Octogenarian Women.

Stress urinary incontinence (SUI) is common among octogenarian women, often in association with significant comorbidities that can include functional and cognitive impairment. There is limited evidence in the literature on the efficacy and safety of current surgical options for treating SUI in octogenarians. The few studies available on this topic mainly focus on mid-urethral slings and reveal similar efficacy and safety profiles for octogenarian women in comparison to younger women. These studies are mostly retrospective, with small sample sizes and short follow-up. What data there are sugges

A Randomized Comparison of Transobturator Tape with the Plication of Urethral Ligaments in the Treatment of Stress Urinary Incontinence.

INTRODUCTION AND HYPOTHESIS: The new urethral ligament plication (ULP) operation for curing SUI is tape free. Instead of using a tape, the pubourethral ligament (PUL) is prevented from lengthening to cause urine loss on effort by a single collagen-creating polyester suture that confines all four branches of the PUL and adds new collagen to strengthen the ligaments. Its anatomical basis is identical to that for the midurethral sling (MUS). METHODS: The 108 patients were randomized by computer, 54 in each arm. Informed consent was obtained. The transobturator tape (TOT) technique was standard o

No mess with mesh; vaginal natural orifice transluminal endoscopic surgery (vNOTES) autologous posterior rectus fascia mesh for vaginal urinary incontinence procedures.

Urinary incontinence affects 25-45 % of women with the gold standard surgical approach being placement of mid-urethral synthetic slings; tension-free vaginal tape (TVT) and trans-obturator tape (TOT). Due to the controversies regarding vaginal mesh the last decade, an increasing demand has evolved for incontinence treatment without vaginal synthetic mesh. The short term results of autologous rectus fascia sling for TOT surgery have shown similar success rates compared to those after the use of synthetic mesh, but the harvesting of the mesh is less minimally invasive and is associated with long

Pelvic ultrasound finding of late-onset bladder erosion after transobturator tape for female stress urinary incontinence: A case report.

RATIONALE: Mid-urethral sling is a gold-standard procedure for stress urinary incontinence because of its high efficacy and low complication incidence. Moreover, mesh erosion into the bladder is a rare complication. PATIENT CONCERNS: The 63-year-old patient visited our gynecology clinic with complaints of gross hematuria and was diagnosed with bladder erosion by ultrasound 6 months after transobturator tape procedure. DIAGNOSES: The 2D ultrasound found the sling in the bladder wall perforation, which can lead to the formation of bladder stones. Meanwhile, 3D ultrasound showed the left side o

Surgical treatment of stress urinary incontinence: comparison of robotic-assisted Burch urethropexy with retropubic midurethral sling.

Synthetic retropubic midurethral slings (RMUS) and robotic-assisted Burch urethropexies (RA-Burch) are common surgical treatment options for stress urinary incontinence (SUI). Few data exist comparing the success of these two retropubic surgeries. This retrospective cohort study of RA-Burch and RMUS procedures compared the proportion of patients with subjective cure after RA-Burch compared to RMUS at our institution between 2016 and 2020. Subjective cure was defined as reporting no symptoms of SUI at longest follow-up. Chi-square, Fisher's exact, Mann-Whitney U tests, logistic regression, and
