Apraxia of speech and nonfluent aphasia: a new clinical marker for corticobasal degeneration and progressive supranuclear palsy.

PURPOSE OF REVIEW: To highlight the fact that patients with corticobasal degeneration (CBD) and progressive supranuclear palsy (PSP) can sometimes present with a progressive apraxia of speech, nonfluent aphasia, or a combination of the two disorders. RECENT FINDINGS: Corticobasal degeneration and PSP are neurodegenerative diseases characterized by neuronal loss and gliosis in cardinal brain regions, as well as the abnormal deposition of the microtubule associated protein tau in cell bodies and cell processes. The typical presenting features of CBD and PSP are akinesia and rigidity that are le

Which clinical features differentiate progressive supranuclear palsy (Steele-Richardson-Olszewski syndrome) from related disorders? A clinicopathological study.

The difficulty in differentiating progressive supranuclear palsy (PSP, also called Steele-Richardson-Olszewski syndrome) from other related disorders was the incentive for a study to determine the clinical features that best distinguish PSP. Logistic regression and classification and regression tree analysis (CART) were used to analyse data obtained at the first visit from a sample of 83 patients with a clinical history of parkinsonism or dementia confirmed neuropathologically, including PSP (n = 24), corticobasal degeneration (n = 11), Parkinson's disease (PD, n = 11), diffuse Lewy body disea

Sulfonylurea binding sites in normal human brain and in Parkinson's disease, progressive supranuclear palsy and Huntington's disease.

In human brain, [3H]glibenclamide binds with high affinity (KD about 3.5 nM) to sulfonylurea binding sites which are associated with ATP-sensitive potassium (KATP) channels. Regarding to the important neuromodulatory action of KATP channels in some neuronal populations, sulfonylurea binding sites were measured in several cortical areas (frontal and temporal cortex, hippocampus) and striatum (caudate nucleus and putamen) in controls and patients with Parkinson's disease or progressive supranuclear palsy. There was no modification of [3H]glibenclamide specific binding in the cerebral regions stu

Constant neurofibrillary changes in the neocortex in progressive supranuclear palsy. Basic differences with Alzheimer's disease and aging.

Neocortical neurofibrillary tangles (NFT) revealed by Bodian technique and anti-tau immunolabelling were seen in 5/5 cases of progressive supranuclear palsy (PSP) aged 58-76 years. These lesions differed from Alzheimer's disease or age-related changes: (1) they were most frequent in the precentral gyrus (Brodmann's area 4) whereas associative areas are predominantly lesioned in Alzheimer's disease; (2) they affected mainly large pyramidal neurons and small cells, relatively sparing the cell population selectively involved in Alzheimer's disease; (3) they predominated in layers V and VI of area

Immunocytochemical comparison of neurofibrillary tangles in senile dementia of Alzheimer type, progressive supranuclear palsy, and postencephalitic parkinsonism.

Antiserum raised against a two-cycle-purified human brain microtubule fraction that binds specifically to tangles of senile dementia of the Alzheimer type was tested on the subthalamus, basis pontis, and midbrain sections of two patients with progressive supranuclear palsy. Tangles composed of 15 nm straight fibers in these areas showed positive immunolabeling with the antiserum. The labeling tangles appeared as a fibrous meshwork in neuronal cell bodies and occasionally in short segments of axons. Corresponding sections of brains obtained from patients with idiopathic parkinsonism, striatonig

Disease severity and progression in progressive supranuclear palsy and multiple system atrophy: validation of the NNIPPS--Parkinson Plus Scale.

BACKGROUND: The Natural History and Neuroprotection in Parkinson Plus Syndromes (NNIPPS) study was a large phase III randomized placebo-controlled trial of riluzole in Progressive Supranuclear Palsy (PSP, n = 362) and Multiple System Atrophy (MSA, n = 398). To assess disease severity and progression, we constructed and validated a new clinical rating scale as an ancillary study. METHODS AND FINDINGS: Patients were assessed at entry and 6-montly for up to 3 years. Evaluation of the scale's psychometric properties included reliability (n = 116), validity (n = 760), and responsiveness (n = 642).

Magnetic Resonance Imaging in the Neuroimaging of Progressive Supranuclear Palsy-Parkinsonism Predominant: Limitations and Strengths in Clinical Evaluation.

Progressive Supranuclear Palsy (PSP) is an atypical Parkinsonism, pathologically described as a four-repeat tauopathy. The contemporary criteria for diagnosis of PSP indicate akinesia, oculomotor dysfunction, postural instability, and language/cognitive impairment as core symptoms. Among these features, the first two are linked to PSP-Parkinsonism predominant (PSP-P). PSP-P is the second most common subtype of PSP, following PSP-Richardson's syndrome (PSP-RS), and is associated with a more gradual deterioration, beneficial course, and longer life expectancy after diagnosis. It is also problema

Can Progressive Supranuclear Palsy Be Accurately Identified via MRI with the Use of Visual Rating Scales and Signs?

Introduction: Neurodegenerative diseases like progressive supranuclear palsy (PSP) present challenges concerning their diagnosis. Neuroimaging using magnetic resonance (MRI) may add diagnostic value. However, modern techniques such as volumetric assessment using Voxel-Based Morphometry (VBM), although proven to be more accurate and superior compared to MRI, have not gained popularity among scientists in the investigation of neurological disorders due to their higher cost and time-consuming applications. Conventional brain MRI methods may present a quick, practical, and easy-to-use imaging rati

Frequency of anti-IgLON5 disease in patients with a typical clinical presentation of progressive supranuclear palsy/corticobasal syndrome.

Serum anti-IgLON5 antibodies, which were tested in 223 patients meeting the diagnostic criteria for progressive supranuclear palsy/corticobasal syndrome (PSP/CBS), were negative in all patients. Our study suggests that the frequency of anti-IgLON5 disease is extremely rare in patients with typical presentation of PSP/CBS.

The Progressive Supranuclear Palsy Clinical Deficits Scale accurately reflects functional as well as patient- and caregiver-reported outcomes.

BACKGROUND: Progressive supranuclear palsy (PSP) is a common atypical parkinsonian disorder, but a significant gap in clinical and research practice has been the unavailability until very recently of a brief but accurate scale to rate its clinical severity. The 7-item PSP Clinical Deficits Scale (PSP-CDS) offers an efficient method to evaluate PSP clinical severity and can be administered relatively rapidly. However, further validation of this new tool is needed. METHODS: Patients with PSP (n = 110; 55.5 % with Richardson syndrome [PSP-RS]) were consecutively recruited at a quaternary centre

Inflammatory and Neurotrophic Factors and Their Connection to Quality of Life in Progressive Supranuclear Palsy-Single-Center Study.

Progressive supranuclear palsy (PSP) is a condition classified as atypical parkinsonism. Pathologically, it is a four-repeat tauopathy; clinically, it is a disease comprising oculomotor dysfunction, postural instability, akinesia, and cognitive/language disorders. Its pathogenesis is not fully recognized; however, neuroinflammation is considered to likely be a significant aspect, though it is not known whether inflammation is a cause or consequence of neurodegeneration. In this study, the authors analyzed the association between inflammatory/neurotrophic factors and parameters linked to qualit

Differences in the Regional T1/T2-Weighted Ratio Index Between Healthy Controls and Patients With Progressive Supranuclear Palsy.

BACKGROUND: Progressive supranuclear palsy (PSP) is a four-repeat tauopathy, and the differentiation of PSP from other neurodegenerative disorders is a common clinical problem. We applied a T1-/T2-weighted ratio imaging technique based on standard clinical MRI to investigate PSP-related changes in neuroimaging data. METHOD: Twenty patients with PSP and 34 healthy participants underwent an examination to obtain T1- and T2-weighted images at our hospital. We retrospectively computed each subject's T1/T2-weighted ratio index to use as a proxy for the brain's myelin content, and we analyzed the d

Clinical Diagnosis of Progressive Supranuclear Palsy (PSP): A Clinicopathological Comparison of Patients with Confirmed PSP and Clinical Mimics.

BACKGROUND: Several neurodegenerative diseases can mimic the clinical presentation of progressive supranuclear palsy (PSP), limiting accurate diagnosis during life. This has important implications for clinical practice, biomarker studies, and therapeutic trial design. OBJECTIVES: To investigate whether the proportion of clinically diagnosed PSP patients with non-PSP pathology (termed 'mimics') has changed over the past 20 years since last examined in this cohort, and to identify clinical features that distinguish confirmed PSP from these mimics. METHODS: We reviewed the clinical records of p

Machine learning approach effectively discriminates between Parkinson's disease and progressive supranuclear palsy: Multi-level indices of rs-fMRI.

AIM: Parkinson's disease (PD) and progressive supranuclear palsy (PSP) present similar clinical symptoms, but their treatment options and clinical prognosis differ significantly. Therefore, we aimed to discriminate between PD and PSP based on multi-level indices of resting-state functional magnetic resonance imaging (rs-fMRI) via the machine learning approach. MATERIALS AND METHODS: A total of 58 PD and 52 PSP patients were prospectively enrolled in this study. Participants were randomly allocated to a training set and a validation set in a 7:3 ratio. Various rs-fMRI indices were extracted, f

Peripheral immune profile and neutrophil-to-lymphocyte ratio in progressive supranuclear palsy: Case-control study and meta-analysis.

BACKGROUND AND PURPOSE: Peripheral inflammation is probably involved in the pathogenesis of progressive supranuclear palsy (PSP) and it may be a common feature with Parkinson's disease (PD). The peripheral immune profile in PSP remains unclear, as well as whether the inflammatory pathways differ from those in PD. The neutrophil-to-lymphocyte ratio (NLR) has been proven to be a well-established biomarker of systemic inflammation. This study aimed to evaluate the peripheral immune profile in PSP compared with PD. METHODS: A cross-sectional study was conducted including patients with PSP and PD

[Clinical features and effects of shunt surgery in patients with progressive supranuclear palsy and idiopathic normal pressure hydrocephalus].

This study aimed to retrospectively review the frequency and clinical features of 13 patients with progressive supranuclear palsy (PSP) and idiopathic normal pressure hydrocephalus (iNPH). All patients were found to have PSP-Richardson's syndrome (PSP-RS). Shunt surgery was effective in 5 of 11 patients (45.5%). A comparison of these 5 patients who responded to shunt surgery versus the remaining 6 patients revealed a significant difference in the reduction of frontal lobe blood flow on cerebral perfusion single-photon emission computed tomography (SPECT) (P = 0.018). These results suggest that

The association of vagal atrophy with parameters of autonomic function in multiple system atrophy and progressive supranuclear palsy.

BACKGROUND: Vagal atrophy is a hallmark of Parkinson's disease (PD) and has been found to be associated with autonomic dysfunction, while analyses of the vagus nerve (VN) in atypical Parkinsonian syndromes (APS) have not yet been performed. We here investigate the characteristics of the VN in multiple system atrophy (MSA) and progressive supranuclear palsy (PSP) and, in a second step, its potential as a possible biomarker for orthostatic dysregulation. OBJECTIVES: The aim was to compare the VN pathology in MSA and PSP with healthy individuals and patients with PD as a differentiating factor a

Case report: Backward gait training combined with gait-synchronized cerebellar transcranial alternating current stimulation in progressive supranuclear palsy.

Progressive supranuclear palsy (PSP) is characterized by recurrent falls caused by postural instability, and a backward gait is considered beneficial for postural instability. Furthermore, a recent approach for rehabilitation combined with gait-oriented synchronized stimulation using non-invasive transcranial patterned stimulation could be promising for balance function. Here, we present a case of PSP with backward gait training combined with gait-synchronized transcranial alternating current stimulation (tACS). A 70-year-old woman with PSP-Richardson's syndrome underwent backward gait trainin
