Emergency cervical suture: the obstetrician's dilemma.

We studied the perinatal mortality and morbidity associated with emergency cervical suture at Royal North Shore Hospital over 7 years. There were 23 patients who had an emergency cervical suture inserted and they were divided into 3 groups for analysis, Group 1: patients with cervical dilatation initially detected on routine 18-20 week ultrasonography and later confirmed on clinical examination, Group 2: cervical dilatation < or = 3 cm and Group 3: cervical dilatation >3 cm at presentation. The median delay in delivery in each group was 6, 5 and 3 weeks respectively. The perioperative membrane

Arthroscopic transglenoid suture of Bankart lesions.

Arthroscopic transglenoid suture of Bankart lesions was performed in 31 patients from 1988 to 1992. The diagnosis in all patients was recurrent traumatic anterior luxation, and a Bankart lesion was found in all cases. Mean time for clinical follow-up was 43 months (ranging from 25 to 76 months). A telephone review of all cases was obtained two years later. Five patients experienced postoperative wound problems posteriorly, where the sutures were tied over the fascia of the infraspinatus. One transient suprascapular nerve palsy was seen. There was a recurrence of complete dislocation in eight p…

[Mechanical digestive system suturing devices].

Between 1994 (December)-1996 (May) 150 patients have been operated on using one or many stapling devices. The staplers disposable to us were the "Linear Cutter" or GIA (Gastrointestinal Anastomosis), "Linear Stapler" (TA) and "Intraluminal Circular Stapler" or EEA (end-to-end anastomosis) types, produced by ETHICON (Johnson and Johnson Ltd. Company). The principles operations performed were various digestive resections, intervisceralis anastomosis and interventions of reconstructions (in oesophagus surgery, ileal pouch etc.). The advantages of staplers applications are: a) the reduction of the

Intracranial pressure in single-suture craniosynostosis.

The indications for surgical correction of craniosynostosis in which there is involvement of only one of the cranial vault sutures have traditionally been based upon the cosmetic merits of the deformity alone. Whilst it is now appreciated that intracranial hypertension is commonly associated with the more complex forms of craniosynostosis, this aspect has not previously been addressed in detail among cases of single-suture craniosynostosis. This retrospective study reports our experience of overnight subdural intracranial pressure monitoring in 74 children with premature closure of a single cr…

Bone suture anchors in hand surgery.

The recent development of small bone suture anchors has created potential applications in reconstructive surgery of the hand and wrist. A combined laboratory and clinical study was devised to evaluate their use. 16 paired fingers (32 in all) from eight cadaveric hands were disarticulated at the MP joint with a 10 cm tail of FDP tendon. The FDP insertion was released in all specimens. In 16 fingers reinsertion was performed with the classic Bunnell technique; in the paired 16 fingers, the repair utilized Acufex 2 mm anchors. There was no significant difference between the two groups regarding l

Surgical correction of metopic suture synostosis.

Premature fusion of the metopic suture is an uncommon form of craniosynostosis, historically reported with an incidence of less than 10% among the various forms of craniosynostoses. Despite its infrequency, it is the most obvious deformity associated with premature fusion of a single suture with its prominent frontal keel, narrow forehead, and close-set eyes. This article discusses the timing, long-term results, and recent advances of surgical techniques.

Pyloric exclusion. Suture material of choice.

Pyloric exclusion was first reported in 1977 as a temporary method of providing duodenal decompression in an attempt to protect the duodenum … e-established. A comparison of polypropylene (PP), polyglycolic acid (PGA), and polydioxanone (PDS) sutures in the dog model suggests, however, that only PDS reliably accomplishes this goal. The low (approximately 5-10%) but finite incidence of fistula formation reported with pyloric exclusion using PGA may be improved by using PDS instead.

[Mechanical suture in bile duct surgery].

The authors describe results of using "one-agraffe" suturing devices and an original "many-agraffes" apparatus for suturing a wound of the common bile duct, papillosphincteroplasty and creation of bile-eliminating anastomoses in experiments in 47 dogs and clinical application in 104 patients. It was shown that these devices can considerably facilitate and accelerate the performing of operations, increase the hermetic properties of the suture and anastomoses, thus giving better remote results as compared with manual sutures with synthetic and catgut threads.

Suture technique for caudal septal deviations.

The "buckled" or severely deviated nasal septum has resisted a variety of techniques devised for its correction. This paper will present a new technique which the author has found useful in many of these cases. This method utilizes in reverse the procedure popularized by Mustardé for managing the protruding ear. Instead of bending the cartilage into a harmonious shape, the bent cartilage is straightened with submucosal permanent sutures. The paper will illustrate this technique.

[Circular suture of the cervix in placenta praevia].

In the accidents of placenta praevia a modified cervixcerclage was used to 25 women during gestation period of 20 to 35 weeks. The circulare suture goes under the bundles of uterine oessels. This inables to escape the lesions of oessels and dystrophy of the cervix uteri. The circulare suture helps to reduce a number of premature labours. The labours were delayed for 23 days and fetal mortality reduced to 10,53%. Cervixcerclage is indicated. Fetal mortality to prevent fetal mortality and to prolonge the pregnancy.

Biomechanical comparison of different suture anchors used in rotator cuff repair surgery-all-suture anchors are equivalent to other suture anchors: a systematic review and network meta-analysis.

PURPOSE: Suture anchors are commonly used to repair rotator cuff tendons in arthroscopy surgery, and several anchor materials have been created to maximize pull-out strength and minimize iatrogenic damage. We hypothesized that all-suture anchors have biomechanical properties equivalent to those of conventional anchors. Our purpose is to compare the biomechanical properties of different anchors used for rotator cuff repair. METHODS: The Embase, PubMed, Cochrane, and Scopus databases were searched for biomechanical studies on various suture anchors. The search keywords included rotator cuff tea

Hybrid Sequential Meniscal Suture Technique.

Meniscal preservation is a priority in the treatment of knee injuries because of the fundamental role of the meniscus in load absorption, joint stability, and chondral protection. Several repair techniques have been described, including inside-out, outside-in, and all-inside approaches. More recently, continuous suturing techniques have emerged as a promising alternative, allowing multiple fixation points with shorter operative time and reduced costs. We describe a hybrid continuous meniscal repair technique that combines initial all-inside fixation with additional outside-in sutures using the…

Modified Technique for Sutured Intraocular Lens.

PURPOSE: To describe a novel scleral fixation technique for the Akreos AO60 intraocular lens using Gore-Tex sutures buried within the sclera to minimize long-term exposure-related complications and improve cosmetic outcomes. METHODS: Four eyes of four patients underwent scleral fixation of the Akreos AO60 lens using the modified ab externo approach. Outcomes included best-corrected visual acuity, intraocular lens centration, and suture-related complications. RESULTS: Preoperative best-corrected visual acuity ranged from 20/60 to 20/300. Postoperative best-corrected visual acuity ranged from …
