Reconstruction of a Rare Split-Level Bilaminar Patellar Tendon Rupture With Concomitant Multiligament Knee Injury Using Peroneus Longus Autograft: A Case Report.

INTRODUCTION: Patellar tendon ruptures (PTRs) are critical causes of knee extensor mechanism failure. Although typically isolated, a split-level bilaminar rupture pattern, in which different layers of the PTR at different anatomical levels, is extremely rare in adults, particularly when associated with complex multiligamentous injuries (occurring in only 0.02%-0.2% of orthopedic cases). CASE PRESENTATION: A 60-year-old female presented with acute knee pain and loss of active extension following a fall. Clinical examination revealed patella alta and a palpable infrapatellar gap. Imaging confir

Effects of Extracorporeal Shock Wave Therapy on Tendon Integrity, Biomechanical Strength, Matrix Remodeling, Inflammation, Angiogenesis, and Tenogenic Differentiation in Rotator Cuff Injury.

Extracorporeal shock wave therapy (ESWT) improves tendon-bone healing after rotator cuff injury (RCI), but its underlying cellular mechanisms remain unknown. This study aimed to explore the therapeutic effects of ESWT on RCI, with a particular focus on endothelial cells and tenocytes. The RCI model was established in rabbits by rotator cuff tear surgery, followed by ESWT intervention. Histological evaluation, blood perfusion analysis, biomechanical testing, and assessment of inflammation and extracellular matrix (ECM) degradation were performed in RCI-treated rabbits at week (W) 2, W4, and W8.

Heel-Rise Height Deficit 1 Year After Achilles Tendon Rupture Relates to Changes in Ankle Biomechanics 6 Years After Injury.

BACKGROUND: It is unknown whether the height of a heel-rise performed in the single-leg standing heel-rise test 1 year after an Achilles tendon rupture (ATR) correlates with ankle biomechanics during walking, jogging, and jumping in the long-term. PURPOSE: To explore the differences in ankle biomechanics, tendon length, calf muscle recovery, and patient-reported outcomes at a mean of 6 years after ATR between 2 groups that, at 1-year follow-up, had less than 15% versus greater than 30% differences in heel-rise height. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: Seventeen pati

Efficacy of Adipose-Derived Mesenchymal Stem Cells and Stromal Vascular Fraction Alone and Combined to Biomaterials in Tendinopathy or Tendon Injury: Systematic Review of Current Concepts.

Background and Objectives: Tendon injury and tendinopathy are among the most frequent musculoskeletal diseases and represent a challenging issue for surgeons as well as a great socio-economic global burden. Despite the current treatments available, either surgical or conservative, the tendon healing process is often suboptimal and impaired. This is due to the inherent scarce ability of tendon tissue to repair and return itself to the original structure. Recently, Adipose-derived mesenchymal stem cells (ADSC) and stromal vascular fraction (SVF) have gained a central interest in the scientific c

Inferior Dislocation of Shoulder Complicated with Undisplaced Greater Tuberosity Fracture, Rupture of the Supraspinatus Tendon, and Brachial Plexus Injury in the Elderly: Case Report and Literature Review.

Inferior dislocation of the shoulder, also called luxatio erecta, is a rare form of the otherwise common shoulder dislocation. It appears in less than 0.5% of all shoulder dislocations. An awareness of associated potential axillary artery injury, brachial plexus complications, and rotator cuff tears is important in this rare entity and should be excluded with a high index of suspicion. In our case report, we have an 83-year-old female who inferiorly dislocated her dominant shoulder with brachial plexus injury and musculotendinous injury, which was caused by an accidental fall. The dislocation

Isolated partial tear of extensor digitorum longus tendon with overlying muscle herniation in acute ankle sports injury: role of high resolution musculoskeletal ultrasound.

Lateral Ankle sprain is a common sports-related trauma with the mechanism of injury ranging from inversion to plantar flexion. These injuries commonly affect the ligaments but can also affect the associated soft tissue structures like the eversion muscles and tendons. Prompt and accurate diagnosis of such injuries is warranted so as to ensure early return to play and prevent long-term complications. Lateral ankle sprain injuries in sports may not always be associated with ligament injuries. We report a never before reported case of lateral ankle sprain injury in a soccer player with the unusua

Posterior Tibial Tendon Rupture Associated With Anterolateral Distal Tibial and Medial Malleolar Fracture and a Novel Pattern of Tibiofibular Syndesmotic Injury: A Case Report and Review of the Literature.

A posterior tibial tendon (PTT) rupture associated with ankle fractures is a very rare condition. Ankle pronation and external rotation (PER) movement are the typical traumatic mechanism. This injury is frequently overlooked preoperatively. Early diagnosis and treatment are very important to prevent the serious consequences related to functional PTT insufficiency on biomechanics of the foot. Few cases have been described in the literature that highlight the relationship between PTT rupture and PER type ankle fracture with a medial malleolar fracture. We present a case of a complete PTT rupture

Comparison of proximal turndown of central slip combined with suture of lateral bands versus free tendon grafting for central slip reconstruction after an open finger injury.

We randomized patients with open finger injury and central slip insertion defects into a proximal turn-down group (final n = 28) and a palmaris longus tendon graft group (final n = 20). In the proximal turn-down group, the dorsal central slip of the extensor tendon in the proximal phalanx was split, leaving it attached distally. We turned the strip from proximal to distal and fixed it using the distal joint capsule of the proximal interphalangeal joint as the distal insertion of reconstruction, and the extended central slips were then fixed to the middle phalanx. The dorsal traumatic central s

Functional Outcomes of Arthroscopic Anterior Cruciate Ligament Reconstruction Using Hamstring Tendon Autograft in a Predominantly Chronic Anterior Cruciate Ligament Injury Cohort: A Prospective Study.

INTRODUCTION: Anterior cruciate ligament (ACL) rupture is a common cause of knee instability. While arthroscopic ACL reconstruction is well established, most studies focus on acute or sports-related injuries. Data on functional outcomes following reconstruction in chronic ACL tears, particularly those due to road traffic accidents, are limited. OBJECTIVE: The objective of this study was to evaluate functional outcomes and knee stability following arthroscopic ACL reconstruction using an autologous hamstring tendon graft in patients with chronic ACL injuries. MATERIALS AND METHODS: This prosp

A magnetic resonance imaging safe distance beyond the flexor hallucis longus tendon and the application of the safe arthroscopic approach to prevent neurovascular injury in posterior ankle arthroscopy.

BACKGROUND: The flexor hallucis longus (FHL) tendon is used as a medial landmark for posterior ankle arthroscopy. However, some posterior ankle pathologies require working medial to the FHL. The safe distance beyond the FHL to the neurovascular bundle has never been established. PURPOSE: To report the safe distances beyond the FHL and neurovascular bundle and the incidence of neurovascular injury after using "safe arthroscopic approach". METHODS: The 63 MRIs with posterior ankle pathologies were included. Measurements were the FHL-tibial nerve (TN) and FHL-posterior tibial artery (PTA) dista

Application of Tendon With Z-Lengthening Technique.

Tendon deficiency limits repair and reconstructive options after tendon and nerve injuries of the upper extremity. Current treatment options include intercalary tendon autograft, tendon transfers, and two-stage tenodesis, with sacrifice of the flexor digitorum superficialis. These reconstructive techniques are associated with donor site morbidity and limited in the setting of multiple tendon deficiencies. The tendon with z-lengthening (TWZL) technique is presented here as an alternative treatment method for tendon injuries and tendon transfer reconstruction after nerve injuries. The TWZL techn…
