Cancer Caregivers' Prognostic and End-of-Life Communication Needs and Experiences and their Impact.

CONTEXT: Family caregivers of patients with advanced cancer are integrally involved in communications regarding prognosis and end-of-life (EOL) planning and care. Yet little research has examined caregivers' communication experiences or the impact of these experiences on patients and caregivers at EOL. OBJECTIVES: Investigate cancer caregivers' communication experiences and potential impact on patient and caregiver outcomes. METHODS: Semistructured interviews with bereaved family cancer caregivers (N=19) about their communication needs and experiences as their loved one approached EOL and di

Bereaved Caregiver Perspectives on the End of Life in Pediatric Patients With Ventricular Assist Devices.

OBJECTIVES: Ventricular assist devices (VADs) are increasingly used in pediatric heart failure as bridges to heart transplantation, although 25% will die with VADs. Family experiences in this population are not well-described. The objective is to understand bereaved families' perspectives on VAD and end-of-life decision-making. DESIGN: Semistructured interviews with bereaved caregivers of pediatric VAD patients. SETTING: Tertiary children's hospital. PATIENTS: Families of six pediatric VAD patients who died from 2014 to 2020. INTERVENTIONS: Not available. MEASUREMENTS AND MAIN RESULTS: Ap

"Dying with a Smile, Just Knowing that Somebody's Listened to Me": End-Of-Life Care and Medical Assistance in Dying in Canadian Prisons.

Medical assistance in dying (MAiD) has been legal in Canada since 2016 and some incarcerated patients who are at the end of their lives are eligible for the procedure. Interviews with nine incarcerated men at a federal penitentiary in Canada provide insight into some of the ways that people who are navigating aging and end-of-life in prison think about MAiD. Interview themes are organized around: experience with death and dying; possibilities and barriers related to applications for release from prison at end-of-life; experiences of peer-caregiving in a prison palliative care program; support

Burden of diagnostic investigations at the end of life for people with chronic obstructive pulmonary disease.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) is an incurable, chronic condition that leads to significant morbidity and mortality, with most patients dying in hospital. While diagnostic tests are important for actively managing patients during hospital admissions, the balance between benefit and harm should always be considered. This is particularly important when patients reach the end of life, when the focus is to reduce burdensome interventions. AIMS: To examine the use of diagnostic testing in a cohort of people with COPD who died in hospital. METHODS: Retrospective medical r

Systematic review of general practice end-of-life symptom control.

BACKGROUND: End of life care (EoLC) is a fundamental role of general practice, which will become more important as the population ages. It is essential that general practice's role and performance of at the end of life is understood in order to maximise the skills of the entire workforce. OBJECTIVE: To provide a comprehensive description of the role and performance of general practitioners (GPs) and general practice nurses (GPNs) in EoLC symptom control. METHOD: Systematic literature review of papers from 2000 to 2017 were sought from Medline, PsycINFO, Embase, Joanna Briggs Institute and Co

The End-of-Life Experiences of People with Motor Neuron Disease: Family Carers' Perspectives.

BACKGROUND: People with motor neuron disease (MND) face barriers when accessing palliative care. There is a lack of research about how these barriers affect the end-of-life (EOL) and death experiences of people with MND. AIM: This study examined the perceptions of EOL experiences of family carers of people with MND in Western Australia (WA) to identify unmet needs and gaps in EOL support for people with MND and their family carers. DESIGN: Semistructured interviews were conducted with 12 bereaved family carers of people who died from MND in WA. Thematic analysis was used to identify common t

"Why can't I give you my organs after my heart has stopped beating?" An overview of the main clinical, organisational, ethical and legal issues concerning organ donation after circulatory death in Italy.

Donation after circulatory death (DCD) is a valuable option for the procurement of functioning organs for transplantation. Clinical results are promising and public acceptance is quite good in most western countries. Yet, although DCD is widespread in Europe, several problems still persist in Italy as well as in some other countries. This paper aims to describe the main clinical, organisational, ethical and legal issues at stake, bearing in mind the particular situation created by Italian legislation. Currently, as regards DCD, Italy is somewhat different from other countries. Therefore, every

A systematic review of informal caregivers' needs in providing home-based end-of-life care to people with cancer.

AIMS: This paper presents the results of a systematic review examining the practical information needs of informal caregivers providing home-based palliative and end-of-life care to people with advanced cancer. BACKGROUND: Modern hospice care has led to increases in home-based palliative care services, with informal caregivers assuming responsibility for the majority of care. In response, health policy emphasises the provision of palliative care services in which both the patient and carer receive adequate support throughout illness and death. While the emotional needs of carers have been ext

Interventions to enhance adaptation to bereavement.

This paper reviews quantitative evaluations of the efficacy of intervention programs designed to reduce the pain and suffering associated with bereavement. After identifying the psychological and physical health impacts of bereavement and outlining the prevalence of detrimental outcomes, we conclude that a minority of bereaved persons experience severe and sometimes lasting consequences, whereas the majority manage to overcome their grief across the course of time. We detail criteria for establishing the efficacy of bereavement intervention and examine the impact of intervention according to t

Answering the relatives of lung cancer patients.

Of 230 bronchial carcinoma patients, 30 underwent resection and 13 (5.6%) survived 5 years. Some of the domestic implications of the diagnosis were sought through study of the 200 non-surgically treated patients and by interviews with 191 of their relatives. Seventy-eight per cent of the relatives said that the illness had not been as bad as they had anticipated. Fifty-five per cent of patients died within 4 months of their first examination and 28.5% of relatives were unprepared for the suddenness of the death. Five per cent of patients died of a massive haemoptysis. For two-thirds of a subgr

[The role of neurolytic therapy in blocking cancer pain].

Here is a presentation of the role played by neurolesive procedures in the management of cancer pain. The authors suggest that these procedures should be considered secondary to the analgesic drugs and should be used only in a very limited number of situations (20/25%) due to the changes in neurophysiology of nerve transmission, caused by these interventions. Open neurolesive surgical treatments have a restricted indication because they are very traumatizing and the risk-benefit ratio is scarcely positive. Generally the chemical and thermal neurolesive procedures which are more widely used are

Gestalt therapy with the dying patient: integrative work using clay, poetry therapy, and creative media.

This is a report on death therapy with a 36-year-old cancer patient. Gestalt therapy and creative media (clay, poetry, and colors) were used to facilitate an integration of life and to give the person a sense of balance with life. The author tries to communicate his thoughts and feelings as they were during the course of the therapy, to show that counseling the dying means walking along a stretch of the path together. The companion-therapist cannot avoid his own perplexity and confusion by simply falling back on his professional role. Once he has become involved in an interpersonal relationshi

Developing a decision support tool for the continuation or deprescribing of antithrombotic therapy in patients receiving end-of-life care: Results of a European Delphi study.

INTRODUCTION: To develop a European shared decision-support tool (SDST), a two-round Delphi process was used to achieve consensus on aspects relating to the antithrombotic therapy (ATT) deprescribing discussions and process in end-of-life cancer patients. METHODS: Conducted between September 2024 and March 2025, the Delphi survey was developed by a multidisciplinary 24-member steering committee (SC), including medical specialists in oncology, hematology, palliative care, primary care, geriatrics, and vascular medicine. The survey involved 188 experts from these specialties across eight Europe

Values and wishes at the end of life among women with advanced cancer: An analysis using the Go Wish Cards Game.

OBJECTIVE: To understand how the Go Wish Cards Game (GWCG) can support the expression of values, wishes, and preferences at the end of life among women living with advanced breast and/or gynecological cancer. METHODS: This descriptive qualitative study was conducted as part of a larger randomized clinical trial. Participants were recruited from a leading cancer center in Brazil and invited to sort the GWCG cards into three categories: "very important," "somewhat important," and "not important." The 10 cards rated as "very important" were discussed individually to explore their meanings. At th

How nurses in China navigate end-of-life communication: A qualitative study.

BACKGROUND: End-of-life communication involves navigating diverse beliefs, values and expectations while supporting individuals and families during a highly vulnerable and emotionally charged stage of life. In China, where family-centred decision-making and taboos surrounding death are still common, nurses face distinctive challenges in navigating these conversations. OBJECTIVE: To explore and describe how experienced registered nurses learned and adapted to engaging in end-of-life communication within the Chinese cultural context, drawing on their retrospective accounts. DESIGN: A qualitati

Deus Ex Machina: An Ethnographic Exploration of Technology, Death and Decision-Making in Respiratory Care.

Noninvasive advanced respiratory support (NARS) is widely used in acute respiratory failure, including at the end of life, where its use is ethically and emotionally fraught. This ethnographic study examines how decisions to initiate, sustain or withdraw NARS are negotiated within the institutional and moral complexities of a UK hospital. Drawing on fieldwork including nonparticipant observation, reflective fieldnotes and composite narratives, this paper explores how dying is not simply recognised but discursively produced through clinical interactions. Guided by Foucault's concept of power/kn

Providing spiritual care at the end of life: nurses' roles and perspectives.

Identifying and meeting patients' spiritual needs is integral to nursing in end of life care. However, spirituality, spiritual care and assessment models are still not fully embedded across the UK. While nurses often collaborate with chaplains or other spiritual care professionals to support patients, ambiguity and misunderstanding between spirituality and religion can cause challenges in delivering effective, person-centred care. This article explores the role of spirituality in nursing practice and introduces structured reflections and practical tools to support spiritual care in end of life

Shared decision-making and deprescribing to support anti-thrombotic therapy (dis)continuance for persons living with cancer in their last phase of life: A realist synthesis.

INTRODUCTION: Patients with cancer are at increased risk of thrombotic complications from both the disease and its treatments, with antithrombotic therapy (ATT) usually continued in the last phase of life where the benefit is less clear and there is high risk of harms. Physiological changes toward the end of life increase the risk that ATT will cause serious bleeding events, but discussion between clinicians and patients of ATT risks and benefits is sub-optimal. This realist synthesis explores shared decision-making (SDM) to: (a) provide insights into why prescribing continues in end-of-life c
