OBJECTIVE: Surveillance for patients undergoing thoracic endovascular aortic repair (TEVAR) for blunt thoracic aortic injury (BTAI) varies. Annual chest computed tomographic angiography (CTA) is often recommended but concerns about the risks and costs have emerged. The aim of this study was to examine the optimal follow-up frequency based on 11-year outcomes and surveillance experience.
METHODS: Seventy-six patients with BTAI received TEVAR from May 2002 to July 2013. Demographics, cardiovascular risk factors, Injury Severity Score (ISS), types, sizes, timing, and outcomes of stent grafts wer

BACKGROUND: Accurate measurement of true aortic luminal diameter (ALD) is critical for endograft sizing in endovascular treatment of blunt thoracic aortic injury (BTAI), but ALD is dynamic and changes with respect to patients' hemodynamic status. This study aimed to characterize how ALD at the time of diagnosis of BTAI compares with ALD at the time of endovascular repair and later at follow-up.
METHODS: This is an Institutional Review Board-approved, single-institution retrospective analysis of prospectively obtained data. Patients were included who presented between July 2007 and December 20

OBJECTIVE: Previous studies describe a high incidence of acute kidney injury after open thoracic aortic surgery. Findings may be confounded by patient selection, including surgery with deep hypothermic circulatory arrest only or emergency procedures. We studied incidence and risk factors of acute kidney injury in patients undergoing thoracic aortic surgery.
METHODS: We reviewed 851 patients undergoing elective thoracic aortic surgery with and without deep hypothermic circulatory arrest between 2000 and 2007, focusing on clinical outcome and acute kidney injury defined by consensus RIFLE (Risk

The Society for Vascular Surgery® pursued development of clinical practice guidelines for the management of traumatic thoracic aortic injuries with thoracic endovascular aortic repair. In formulating clinical practice guidelines, the Society selected a panel of experts and conducted a systematic review and meta-analysis of the literature. They used the Grading of Recommendations Assessment, Development and Evaluation methods (GRADE) to develop and present their recommendations. The systematic review included 7768 patients from 139 studies. The mortality rate was significantly lower in patients

OBJECTIVE: Ischemic preconditioning has been found to protect various organs from a subsequent longer ischemic insult. We investigated whether the late phase of ischemic preconditioning reduces spinal cord injury from occlusion of the descending thoracic aorta.
METHODS: Twenty-four pigs (27 to 30 kg) were randomly divided in four groups: group I (n = 4) underwent a sham operation, group II (n = 4) underwent aortic occlusion for 20 minutes, group III (n = 8) underwent aortic occlusion for 35 minutes, and group IV (n = 8) underwent aortic occlusion for 20 minutes and, 48 hours later, aortic occ

STUDY DESIGN: A case of thoracic spinal cord injury without radiographic abnormality in a skeletally mature patient is reported.
OBJECTIVES: To report, for the first time in the literature, a case of thoracic spinal cord injury without radiographic abnormality or any ligamentous injury in a skeletally mature patient, and to propose a unique mechanism of spinal cord traction injury resulting from associated bilateral high-energy lower limb injuries. SUMMERY OF BACKGROUND DATA: Thoracic spinal cord injury without radiographic abnormality is extremely uncommon in skeletally mature patients. Hype

STUDY DESIGN: Case report.
OBJECTIVES: To describe a rarely reported type of upper thoracic spinal cord injury without vertebral bony lesion in two cases with multiple trauma.
SUMMARY OF BACKGROUND DATA: Because it is supported by the stiffness of the rib cage, the upper thoracic spine has greater stability than the cervical and lumbar regions, and thus its fracture or fracture dislocation is less frequent. Nevertheless, when fracture or fracture dislocation of upper thoracic spine occurs, spinal cord involvement and severe concomitant injuries are frequently associated.
METHODS: Two cases

The spinal cord is rarely transected after spinal cord injury. Dysfunction of surviving axons, which traverse the site of spinal cord injury, appears to contribute to post-traumatic neurological deficits, although the underlying mechanisms remain unclear. The subpial rim frequently contains thinly myelinated axons which appear to conduct signals abnormally, although it is uncertain whether this truly reflects maladaptive alterations in conduction properties of injured axons during the chronic phase of spinal cord injury or whether this is merely the result of the selective survival of a subpop

One hundred five hemodynamically stable patients with penetrating thoracic trauma were prospectively evaluated for occult cardiac injury. All patients underwent transthoracic echocardiography (ECHO) and subxiphoid exploration (SXE). Those with positive SXE underwent exploration. Results are calculated based on operative findings of significant injuries. For the entire group, SXE was more sensitive than ECHO (sensitivity, 100% vs. 56%; specificity, 92% vs. 93%; and accuracy, 92% vs. 90%, respectively). The SXE missed no injuries in this group. Eight of 17 patients, however, with positive SXEs h

We describe 270 patients with spinal injuries who were treated by spring alloplasty between 1971 and 1985. The radiological results were compared with the mechanism of injury. The best outcome was obtained in patients with a spinal dislocation or flexion fracture. The technique is also recommended in compression fractures with wedging of the vertebral body, where there is a smaller proportion of good radiological results, but a gratifying improvement in neurological and functional outcome. The method is only cautiously advocated in cases of facet joint fracture and is generally contraindicated

Spinal cord injury following operations on the descending thoracic or thoracoabdominal aorta remains a major problem. In certain subsets of patients, the risk of postoperative spinal cord injury is substantial. Although several adjuncts have been employed clinically to eliminate or reduce the frequency of this complication, none have proven to be completely effective. An important reason for this is the failure of these techniques to reliably and noninvasively localize the level of origin of arteries from the aorta that are critical to spinal cord circulation. Since postoperative spinal cord i
