Phosphene-guided transcranial magnetic stimulation of occipital but not parietal cortex suppresses stimulus visibility.

Transcranial magnetic stimulation (TMS) applied over the occipital lobe approximately 100 ms after the onset of a stimulus decreases its visibility if it appears in the location of the phosphene. Because phosphenes can also be elicited by stimulation of the parietal regions, we asked if the same procedure that is used to reduce visibility of stimuli with occipital TMS will lead to decreased stimulus visibility when TMS is applied to parietal regions. TMS was randomly applied at 0-130 ms after the onset of the stimulus in steps of 10 ms in occipital and parietal regions. Participants responded …

Repetitive transcranial magnetic stimulation (rTMS) to treat refractory panic disorder patient: a case report.

Panic disorder (PD) is an anxiety disorder that is highly disruptive to the patient's life and needs new options for effective treatments. In this case report, we present an application of repetitive transcranial magnetic stimulation (rTMS) in a refractory patient resistant to cognitive behavior therapy (CBT) and pharmacotherapy who was treated with a combined protocol of rTMS with a sequential stimulation of right and left dorsolateral prefrontal cortex (DLPFC). The protocol was conducted 3 times per week during 4 weeks, with one month follow-up. The fact that the patient showed improvement s…

Assessing consciousness in coma and related states using transcranial magnetic stimulation combined with electroencephalography.

Thanks to advances in medical care, an increased number of patients recover from coma. However, some remain in vegetative/unresponsive wakef … ciated with behavioral assessment of consciousness. We will then describe a non-invasive technique, transcranial magnetic stimulation combined with high-density electroencephalography (TMS-EEG), which has allowed us to detect the presence or absence of consciousness in different physiological, pathological and pharmacological states. Some potential underlying mechanisms of the los …

Motor cortical excitability assessed by transcranial magnetic stimulation in psychiatric disorders: a systematic review.

BACKGROUND: Transcranial magnetic stimulation (TMS) is a popular neurostimulation technique suitable for the investigation of inhibitory and facilitatory networks in the human motor system. In the last 20 years, several studies have used TMS to investigate cortical excitability in various psychiatric disorders, leading to a consequent improvement in pathophysiological understanding. However, little is known about the overlap and specificity of these findings across these conditions. OBJECTIVE: To provide a systematic review of TMS studies (1985-2013) focusing on motor cortical excitability in…

Therapeutic benefit of repetitive transcranial magnetic stimulation for severe mirror movements: A case report.

Congenital mirror movements retard typical hand functions, but no definite therapeutic modality is available to treat such movements. We report an 8-year-old boy with severe mirror movements of both hands. His mirror movements were assessed using the Woods and Teuber grading scale and his fine motor skills were also evaluated by the Purdue Pegboard Test. A 2-week regimen of repetitive transcranial magnetic stimulation produced markedly diminished mirror movement symptoms and increased the fine motor skills of both hands. Two weeks after the completion of the regimen, mirror movement grades had…

Effects of transcranial magnetic stimulation on visual evoked potentials in a visual suppression task.

Transcranial magnetic stimulation (TMS) can non-invasively modify cortical neural activity by means of a time-varying magnetic field. For example, in cognitive neuroscience, it is applied to create reversible "virtual lesions" in healthy humans (usually assessed as diminished performance in a behavioral task), thereby helping to establish causal structure-function relationships. Despite its widespread use, it is still rather unclear how TMS acts on existing, task-related neural activity, potentially resulting in a measurable effect on the behavioral level. Here, we deliver TMS to early visual …

Using transcranial magnetic stimulation methods to probe connectivity between motor areas of the brain.

Transcranial magnetic stimulation is increasingly used as a tool to explore cortical motor function in healthy subjects and in patients with neurological disease or injury. This review describes a "twin coil" TMS approach that allows investigation of time related changes in functional connectivity between primary motor cortex and other areas in preparation for a forthcoming movement. Investigations into premotor-motor interactions show that these are specific to the type of task that is performed as well as the muscles used to control the movement, allowing us to monitor information flow withi…

Reduction of spasticity with repetitive transcranial magnetic stimulation in patients with spinal cord injury.

OBJECTIVE: Spasticity with increased tone and spasms is frequent in patients after spinal cord injury (SCI). Damage to descending corticospinal pathways that normally exert spinal segmental control is thought to play an important causal role in spasticity. The authors examined whether the modulation of excitability of the primary motor cortex with high-frequency repetitive transcranial magnetic stimulation (rTMS) could modify lower limb spasticity in patients with incomplete SCI. METHODS: Patients were assessed by the Modified Ashworth Scale, Visual Analogue Scale, and the Spinal Cord Injury …

[Functional neuroimaging and the treatment of aphasia: speech therapy and repetitive transcranial magnetic stimulation].

Functional imaging has provided new evidence of the neurobiological impact of the treatment of aphasia, including speech therapy, through the alteration of the activated language neural network. In such a way, speech therapy has proved its impact. The role of each hemisphere is still very unclear. Some of the authors link the left-lateralisation of activations to the therapeutic improvement of language and the right-activated network to a maladaptative strategy, whereas others consider the latter as a useful compensatory network for speech disorders. Treatment-emergent mania in unipolar and bipolar depression: focus on repetitive transcranial magnetic stimulation.

This review focused on the treatment-emergent mania/hypomania (TEM) associated with repetitive transcranial magnetic stimulation (rTMS) treatment of depression. English-language literature published from 1966-2006 and indexed in Medline was searched. Ten of 53 randomized controlled trials on rTMS treatment of depression specifically addressed TEM. The pooled TEM rate is 0.84% for the active treatment group and 0.73% for the sham group. The difference is not statistically significant. Along with case reports, a total of 13 cases of TEM associated with rTMS treatment of depression have been publ…

Improvement of cervical dystonia: possible role of transcranial magnetic stimulation simulating sensory tricks effect.

Idiopathic cervical dystonia (ICD) is the most common form of focal dystonia. A characteristic and unique diagnostic feature is the presence … he supplementary motor area and the primary sensorimotor cortex (contralateral to head turn). Since transcranial magnetic stimulation (TMS) is an experimental device with the ability to excite or depress the neural circuits, we hypothesize that the use of TMS of specific parameters to specific brain areas (as above) may produce an effect similar to sensory tricks resulting in the …

[Effect of slow repetitive transcranial magnetic stimulation on depression in patients with Parkinson disease].

Parkinson's disease (PD), that has usually been associated with movement disorders, is also associated with depression in about 40% of patients [1-9]. Transcranial magnetic stimulation (TMS) is a new non-invasive technique for direct stimulation of the cerebral cortical neurons [1]. Several open studies have shown that repetitive TMS (rTMS) at both rapid (rapid rTMSi > 1 Hz) and low frequencies (slow rTMSi < 1 Hz) may have antidepressant action [2-6]. The study included 8 patients diagnosed as PD fulfilling the DSM-IV criteria for major depression (5 patients) and dysthymia (3 patients). Magne…

Resetting of essential tremor and postural tremor in Parkinson's disease with transcranial magnetic stimulation.

We studied the effects of transcranial motor cortex stimulation on the electromyographic characteristics of tremor in 9 patients with familial essential tremor and in 12 patients with postural tremor associated with Parkinson's disease. Transcranial magnetic stimulation reset both types of tremor equally. The resetting depended on the stimulus intensity, but was most closely correlated with the duration of the electromyographic silent period that followed the stimulus-induced motor evoked potential. Tremor resetting was present bilaterally even after focal, unilateral stimulation. Transcranial…

Transcranial magnetic stimulation in cocaine use disorder and the risk of seizure: a review of the evidence.

Transcranial magnetic stimulation (TMS) has emerged as a promising intervention for cocaine use disorder (CUD). However, a key concern when employing TMS in CUD is the potential risk of seizures. Our goal was to assess seizure risk in individuals with CUD undergoing TMS and to propose parameters that could mitigate it. Our review of the literature indicated that seizures are primarily associated with high-dose cocaine use necessitating urgent medical care - and that the risk is likely low outside of this setting. Thus, to mitigate potential seizure risks during TMS sessions, we suggest an asse…

Cellular Mechanisms of Transcranial Magnetic Stimulation in Climbing Fibers and Purkinje Neurons in the Cerebellum.

Although transcranial magnetic stimulation (TMS) is widely used for brain stimulation, fundamental issues about its underlying mechanisms remain unresolved. We investigated some of these issues experimentally using an intact isolated turtle cerebellum in vitro, employing a novel chamber designed to deliver precisely calibrated induced electric fields along cortical depth. Our results show that single-pulse TMS can directly activate Purkinje cells and climbing fibers, and synaptically activate Purkinje cells via climbing fibers - all within the first 1.2 ms. Specifically, current source density…

Feasibility of phase-locked transcranial magnetic stimulation of cerebellum for the treatment of essential tremor.

Objective.Cerebellar transcranial magnetic stimulation (TMS) has been proposed to suppress limb tremors in essential tremor (ET), but mixed results have been reported so far, both when pulses are applied repetitively TMS (rTMS) and in bursts. We aim to investigate the cellular effects of TMS on the cerebellum under ET through numerical simulations.Approach.A computational model of the olivo-cerebello-thalamocortical pathways exhibiting the main neural biomarkers of ET (i.e. circuit-wide tremor-locked neural oscillations) was expanded to incorporate the effects of TMS-induced electric field (E-…
