The effect of acarbose on the colonic transit time of elderly long-term care patients with type 2 diabetes mellitus.

BACKGROUND: Constipation is common in elderly patients with diabetes mellitus (DM); its prevalence is estimated as up to 60% among patients with diabetic neuropathy. Acarbose, an alpha-glucosidase inhibitor, has a beneficial role in controlling DM, although one of its side effects is diarrhea. This study evaluates the efficacy of acarbose in improving constipation using transit time (TT) studies in elderly long-term care (LTC) patients. METHODS: Twenty-eight patients with type 2 DM and constipation were recruited for the study. TT was measured by radiopaque markers and was calculated separate

Using Multistate Models and Qualitative Interviews to Comprehensively Characterize Substance Use Disorder Care Transitions in a US Health Care System: Protocol for a Mixed-Methods Study.

BACKGROUND: Many substance use disorder (SUD) care pathways exist in health care systems. However, patients with SUD often report poor care experiences, particularly regarding timely follow-up, clinician and general satisfaction ratings, and care communication. As SUD care pathways involve transitions across clinicians and venues of care, adequate treatment requires coordination across clinicians and settings to ensure unmet needs are addressed, and appropriate SUD care is delivered. Surprisingly little is known about the real-world care pathways patients engage in once identified as having SU

The transition of adolescents with juvenile idiopathic arthritis or epilepsy from paediatric health-care services to adult health-care services: A scoping review of the literature and a synthesis of the evidence.

Young people with long-term health conditions (LTCs) can face challenges when making the transition to adult health services. This paper sought to identify studies that assess and explore transitional care for young people with LTCs. Two conditions were used as exemplars: juvenile idiopathic arthritis (JIA) and epilepsy. A scoping review of the literature was conducted by using search terms to search for papers in English between 2001 and 2016 concerning transitional care on four databases. Qualitative papers were reviewed and synthesized using thematic analysis. Quantitative papers using heal…

International recommendations for child to adult care transition in rare neurological diseases: A scoping review and Delphi consensus study.

AIM: To produce recommendations for specific neurological diseases, including ataxias, hereditary spastic paraplegias, leukodystrophies, dystonias, and choreas. METHOD: Our study consisted of two related phases. First, we conducted a scoping review to identify essential elements of transitional care models. These elements were included in a questionnaire that was subsequently evaluated by an international group of experts using a modified web-based Delphi consensus process. Results were analysed according to the RAND/UCLA methodology. RESULTS: Fifty-nine statements of transitional care were …

Stakeholder Perspectives on the Use of Telehealth to Support Deprescribing for Older Patients Transitioning From the Hospital to Home Health Care.

BACKGROUND: Multimorbidity affects most older adults in home health care (HHC), leading to inappropriate polypharmacy and increased risks for adverse outcomes. While deprescribing can mitigate these risks, fragmented communication during hospital-to-home transitions remains a major barrier. This study explored stakeholder perspectives on using telehealth to facilitate deprescribing for older adults with multimorbidity while receiving post-acute HHC. METHODS: We conducted semi-structured interviews with 44 stakeholders across 12 U.S. states, including 14 HHC patients and 30 clinicians (physici

Evaluating the Joint Effects of Dementia and Frailty on Burdensome Transitions Among Long-Term Care Residents in Ontario, Canada.

BACKGROUND: Transfers from long-term care (LTC) to hospital near the end of life can be burdensome for LTC residents. Dementia and frailty are common in this population, yet their relationship to burdensome transitions remains unclear. This study examined the joint associations between dementia, frailty, and end-of-life burdensome transitions among LTC residents. METHODS: We conducted a population-based retrospective cohort study using health administrative data, capturing LTC residents aged ≥ 65 who died in Ontario, Canada, January 1, 2015-March 31, 2020. Dementia was identified using a vali

A continuum of disconnection: the impacts of safer supply program closures on continuity of care, transition, and total loss of service access.

OBJECTIVE: In March 2025, the expiration of federal pilot funding for Safer Supply Programs (SSPs), combined with restrictive provincial legislative changes to harm reduction service provision in Ontario, disrupted care for thousands of clients at risk of toxic drug poisoning. This study examines the impacts of these changes on client health, social stability, and overdose risk across a continuum of access. METHOD: We conducted 30 semi-structured virtual interviews between September and November 2025 with clients who were enrolled in an SSP prior to March 31, 2025. Participants were recruited

Prediction of unplanned readmission in older adults during hospital stay: a data-driven approach to support care transition.

PURPOSE/AIMS: Accurately predicting 30-day unplanned readmission in older adults is critical for improving care transitions and reducing preventable hospitalizations. Most existing models rely only on data available at discharge, limiting early intervention. This study aimed to develop and evaluate machine learning models to predict 30-day unplanned readmissions using information from the first 48 h of hospitalization, and to compare their performance with discharge-based models. METHODS: We analyzed data from 14,140 older adult admissions to a quaternary hospital. We built ML algorithms to p

Two Patients, One Plan: Redesigning Transitions of Care in High-Risk Pregnancy with Mom-Baby Dyadic Models.

Maternal and neonatal care in the United States (US) is often delivered separately, creating gaps during the critical transition from hospital to home. Fragmented maternal and neonatal care contributes to missed postpartum visits, delayed recognition of complications, and suboptimal outcomes, particularly in high-risk populations. Mom-Baby dyad care models integrate transitions of care by approaching the mother and infant as a single care unit. Evidence from high-risk obstetric populations, including patients with preeclampsia, gestational diabetes, and infants requiring NICU care, suggests th

Bridging the gap in familial hypercholesterolemia: An adult care perspective on transitioning emerging adults toward lifelong cardiovascular prevention.

BACKGROUND: Familial hypercholesterolemia (FH) confers lifelong elevation in low-density lipoprotein-cholesterol (LDL-C) and markedly increases the risk for premature atherosclerotic cardiovascular disease. Because atherosclerosis begins early in FH, timely initiation of statin therapy reduces lifetime LDL-C exposure, slows disease progression, and improves cardiovascular event-free survival. SOURCES OF MATERIAL: Relevant studies were identified through PubMed and Embase searches (2010-2025) using the terms FH, transition of care, and lipid management, supplemented by guidelines and consensus

Transition From Adolescent to Adult Care for Young People Living With HIV: A Systematic Review of Needs, Barriers, and Interventions.

Young people living with HIV (YPLHIV) aged 15-24 years represent over 25% of new HIV infections globally. They face significant challenges in transitioning from pediatric to adult care. Despite guidelines from major health organizations, many YPLHIV experience unstructured transitions, leading to gaps in care and reduced viral suppression. This systematic review examines barriers and facilitators during this transition and evaluates interventions aimed at improving the transition to adult care for YPLHIV. This systematic review follows Preferred Reporting Items for Systematic Reviews and Meta-
