Secondary insults during the interhospital transfer of head-injured patients: an audit of transfers in the Mersey Region.

To assess the incidence of secondary insults and unidentified extracranial injuries a prospective audit of 50 head-injury transfers to a regional neurosurgical unit using a standardized assessment proforma was undertaken. There was wide variability in the quality of transfers. Six per cent of the group were hypoxic on arrival and 15 per cent were hypotensive. In the patients with multiple injuries, 29 per cent had inadequately diagnosed or managed injuries when they arrived. A comparison of this cohort of patients with previous studies is presented. As a result of the audit a set of transfer a

Psychiatric disorders in children and adolescents in the first six months after mild traumatic brain injury.

The objective was to assess the nature, rate, predictive factors, and neurocognitive correlates of novel psychiatric disorders (NPD) after mild traumatic brain injury (MTBI). Children age 5-14 years with MTBI (N=87) from consecutive admissions to five trauma centers were enrolled and studied with semistructured psychiatric interviews soon after injury (baseline), and 70 of these children were assessed again 6 months post-injury. Injury severity; lesion characteristics; pre-injury variables, including psychiatric disorder, family psychiatric history, family functioning, socioeconomic status, ps

The Relationship Between Forms of Childhood Maltreatment and Vascular Function Indices in Black Men and Women.

OBJECTIVE: Examine the association between types of childhood maltreatment and surrogate markers of vascular function, and to test whether sex modifies these relationships. METHODS: Childhood maltreatment and indices of vascular function were assessed in a cohort of healthy Black adults without known CVD (n=404). Maltreatment (physical, sexual, emotional, and general trauma) domains were assessed using the Early Trauma Inventory Short Form. A trauma severity index score was calculated by summing the indexes for each domain. Outcomes of central augmentation index (cAIx) corrected for a heart r

Injuries related to electric scooter and bicycle use in a Washington, DC, emergency department.

OBJECTIVE: E-scooter use has grown rapidly in the United States. Its rise in popularity has coincided with the promotion of cycling in many cities, but more needs to be known about how these transportation modes compare to determine if cycling should serve as an appropriate benchmark for policy decisions and safety expectations regarding e-scooters. METHODS: We examined characteristics of adults seeking treatment in a Washington, DC, emergency department (ED) for injuries associated with riding e-scooters during 2019 (n = 99) or bicycles during 2015-2017 (n = 337). RESULTS: E-scooter inciden

Diagnostic accuracy of the Extended Focused Abdominal Sonography for Trauma (E-FAST) performed by emergency physicians compared to CT.

INTRODUCTION: The diagnostic accuracy of the FAST exam performed by EM residents were shown to be similar to radiology residents. However, in the last 2 decades, an extended-FAST (E-FAST) protocol including thoracic examination to exclude pneumo- and hemothorax was introduced. The accuracy of emergency physicians (EPs) while performing E-FAST is a less studied area, especially in Europe. The aim of this study was to compare the diagnostic accuracy of the E-FAST exam performed by EM residents with the results of CT scan as a gold standard. METHODS: This was a prospective, observational, diagno

Modified stoppa approach for acetabular fractures with anterior and posterior column displacement: quantification of radiographic reduction and analysis of interobserver variability.

OBJECTIVES: To quantify the reduction of acetabular fractures with displacement of the anterior and posterior column by using only a single anterior approach that combines the modified Stoppa approach with the lateral window of the classic ilioinguinal approach. The second objective was to evaluate the interobserver variability of our technique for radiographic evaluation of the reduction. SETTING: Level I trauma referral center. PATIENTS: A consecutive cohort of 17 patients with displaced acetabular fractures treated operatively with use of only an anterior approach that combined the modifi

Vertebral arterial injury due to fatal dog bites.

Dog bite-related injuries and fatalities are major public health problem worldwide. The authors present a case of a Japanese Tosa-dog bite-related fatality in an old woman who died from a vertebral arterial laceration with the C5 vertebral fracture. This was an uncommon type of injury that dog bites injured the vertebral artery only without damage to carotid artery. The identity of the Tosa-dog as an offending dog in this case was unsuccessful by a DNA analysis. Then the detection of the offending dog was made by comparisons of the dental casts of the dog with the victim's wounds. This case re

Injuries significantly associated with thoracic spine fractures: a case-control study.

OBJECTIVE: To determine injuries significantly associated with traumatic thoracic spine (T-spine) fractures METHODS: This was a case-control study undertaken in an adult trauma centre. Cases were patients admitted with a traumatic T-spine fracture between January 1999 and August 2007. Each case had two controls matched for sex, age (+/-5 years) and injury severity classification (major/minor). Data were collected from patient medical records and the trauma service database. Multivariate logistic regression was used to determine injuries significantly associated with T-spine fracture. RESULTS

[Structured radiologic diagnosis in polytrauma].

PURPOSE: Development of a concept for structured diagnostic imaging in patients with multiple trauma. MATERIAL AND METHODS: Evaluation of data from a prospective trial with over 2400 documented patients with multiple trauma. All diagnostic and therapeutic steps, primary and secondary death and the 90 days lethality were documented. Structured diagnostic imaging of multiple injured patients requires the integration of an experienced radiologist in an interdisciplinary trauma team consisting of anesthesia, radiology and trauma surgery. Radiology itself deserves standardized concepts for equipme

Validating new summary indices for the Childhood Trauma Interview: associations with first onsets of major depressive disorder and anxiety disorders.

Childhood and adolescent adversity is of great interest in relation to risk for psychopathology, and interview measures of adversity are thought to be more reliable and valid than their questionnaire counterparts. One interview measure, the Childhood Trauma Interview (CTI; Fink et al., 1995), has been positively evaluated relative to similar measures, but there are some psychometric limitations to an existing scoring approach that limit the full potential of this measure. We propose several new summary indices for the CTI that permit examination of different types of adversity and different de

Pelvic fracture from major blunt trauma. Outcome is determined by associated injuries.

Pelvic hemorrhage has been implicated as the cause of death in 50% of patients who die following pelvic fractures. To establish correlates of morbidity and mortality from pelvic fractures due to blunt trauma, we reviewed 236 patients treated during 4 years. The average age of the 144 men and 92 women was 31.5 years, the average Injury Severity Score was 21.3, the average blood requirement was 5 units, and the average hospital stay was 16.8 days. One hundred fifty-two patients (64.4%) were injured in motor vehicle accidents, 33 (14%) had motor vehicle-pedestrian accidents, 16 (6.8%) had crush i

Associations between post-traumatic stress disorder severity, drinking motives, and unhealthy alcohol use among women veterans.

OBJECTIVE: Unhealthy drinking, defined as patterns of alcohol use that increase risk for adverse consequences, is common among US military veterans. Post-traumatic stress disorder (PTSD) severity has been positively associated with alcohol-related consequences and, less consistently, alcohol consumption among veterans. PTSD is highly heterogeneous, with specific symptom clusters showing varying patterns of association with indices of unhealthy drinking. Drinking motives, particularly coping-related motives, have also been associated with greater alcohol-related consequences and PTSD severity.

Introducing the Surgical Therapeutic Index in trauma surgery: an assessment tool for the benefits and risks of operative fracture treatment strategies.

BACKGROUND: The Surgical Therapeutic Index (STI) has been described as an indicator of the benefits and risks of surgical treatment. The index is calculated by dividing the cure rate of an operative treatment by the complication rate. This study introduces the STI in trauma surgery by comparing the indices for surgical plate fixation (PF) and intramedullary fixation (IMF) of displaced midshaft clavicular fractures. METHODS: In a previously reported, randomized controlled fashion, 120 patients were assigned to PF or IMF. Cure was defined by a Disabilities of the Arm, Shoulder and Hand score of

Analysis of Concomitant and Isolated Venous Injury in Military Lower Extremity Trauma.

BACKGROUND: The implications of major venous injury to the lower extremity are not well established. We aimed to determine the significance of concomitant and isolated femoropopliteal venous injury and assess the impact of surgical management strategies on limb outcomes. METHODS: The Fasciotomy and Vascular Injury Outcomes database was queried for limbs sustaining femoropopliteal arterial, venous, or concomitant injuries in Iraq or Afghanistan between 2004 and 2012. Demographics, injury patterns and severity, interventions, and outcomes were compared between patients sustaining isolated arter

The effectiveness and safety of LMWH for preventing thrombosis in patients with spinal cord injury: a meta-analysis.

BACKGROUND: Unfractionated heparin (UFH) and low molecular weight heparin (LMWH) are commonly used for preventing venous thrombosis of the lower extremity in patients with traumatic spinal cord injury. Although, LMWH is the most commonly used drug, it has yet to be established whether it is more effective and safer than UFH. Further, a comparison of the effectiveness of LMWH in preventing thrombosis at different locations and different degrees of spinal cord injury has also not been clearly defined. MATERIALS AND METHODS: Cohort studies comparing the use of LMWH and UFH in the prevention of l

The closed eye harbours a unique microbiome in dry eye disease.

Dry eye affects millions of individuals. In experimental models, dry eye disease is associated with T helper cell 17-mediated inflammation of the ocular surface that may cause persistent damage to the corneal epithelium. However, the initiating and perpetuating factors associated with chronic inflammation of the ocular surface remain unclear. The ocular microbiota alters ocular surface inflammation and may influence dry eye disease development and progression. Here, we collected serial samples of tears on awakening from sleep, closed eye tears, during a randomized clinical trial of a non-pharm

A Rule of Thumb for Hand Burns: Categorization and Mapping of Proportional Surface Area Involvement.

Hand burns are common and often complex injuries, requiring referral to specialist centers. The patient's thumbprint is a rapid means of accurately assessing hand burn surface area. This study aimed to establish categories and evaluate sites of hand burn surface area in order to facilitate comparison of hand burns. Sixteen burns involving the hand and wrist among 14 patients referred to a burns center were retrospectively categorized by burn mechanism, burn thickness, and hand burn surface area. The burn surface area in the clinical record was compared with that calculated by the thumbprint me
