BACKGROUND: Screening for latent tuberculosis infection (LTBI) in migrants is important for elimination of tuberculosis in low-incidence countries, alongside the need to detect blood-borne infections to align with new guidelines on migrant screening for multiple infections in European countries. However, feasibility needs to be better understood.

A woman in her mid-30s presented with a 3-month history of dry cough, profound weight loss and anorexia. Evaluation revealed pulmonary tuberculosis microbiologically confirmed by bronchoalveolar lavage cartridge-based nucleic acid amplification testing (CBNAAT) together with histopathological evidence from endobronchial biopsy, with extensive dissemination involving the pleura, pericardium, peritoneum, omentum, liver and colon. Bronchoscopy demonstrated multiple nodular endobronchial lesions while bronchoalveolar lavage CBNAAT detected Mycobacterium tuberculosis without rifampicin resistance.

The complications of isoniazid (INH) were studied in 1033 patients, who had received INH for at least 18 months, with or without other drugs. We examined surgical liver specimens from 52 patients with hepatitis C virus-related cirrhosis. All patients underwent orthotopic liver transplantation at Paul Brousse Hospital. Systemic infections occasionally present with ocular involvement. Prompt diagnosis and management, aided by an internist, may lead to resolution of the infection without severe ocular sequelae. A case of brucellosis associated with peritonitis, intestinal obstruction, granulomatous hepatitis, inappropriate antidiuretic hormone (ADH) secretion and meningitis is reported. Initially, the patient was diagnosed as a case of disseminated tuberculosis and treated accordingly. However, the serologic tests for brucellosis were strongly positive and the patient was subsequently treated as a case of brucellosis and recovered fully. The gastrointestinal manifestations of brucellosis are reviewed.
