[Diagnostic features of tuberculosis in extra-pulmonary locations].

Bacteriological examination of patients with extralung tuberculosis is carried out, namely: tuberculosis of urino-genital organs, cerebral membranes and central nervous system, bones and joints, and also of peripheral lymphatic knots. Strains isolated under these diseases were examined for medical sensitivity to antibacterial drugs (ABD), their biochemical characteristics were studied. It was ascertained, that the strains of mycobacteria of tuberculosis (MBT), isolated under tuberculosis of cerebral membranes and central nervous system, tuberculosis of urino-genital organs, tuberculosis of bon

[Results of studying of the pathomorphism of pulmonary tuberculosis in Iakutiia].

The paper summarizes the results of studies into the pathological anatomy and pathomorphism of pulmonary tuberculosis in Yakutia, which have been conducted by the Department of Anatomy, Yakutsk State University Medical Institute, over 36 years. A morphological comparative analysis of autopsies of individuals who died from different forms in the pre-antibacterial and antibacterial periods and surgical materials were used as a major source of data. More than 1,000 autopsies made it possible to define a wide range of behavioral patterns and the morphological features of pulmonary tuberculosis und

Factors associated with drug resistance in pulmonary tuberculosis.

BACKGROUND: Drug-resistant tuberculosis is becoming a formidable foe for the mankind as it is difficult to manage. The present study was aimed at studying the factors associated with drug resistance in tuberculosis. METHODS: One-hundred and nine patients from whom mycobacterial growth was obtained on culture were studied. RESULTS: Treatment default was found to be the most important factor associated with drug resistance. Forty-five of the 48 isolates from patients with history of default showed drug resistance. Travel to a different place was the reason for default in 19 of 45 patients with

Tonsillar tuberculosis associated with pulmonary and laryngeal foci.

Tonsillar tuberculosis is one of the uncommon forms of extrapulmonary tuberculosis. We report a case of tonsillar tuberculosis associated with pulmonary and laryngeal foci. A 23-year-old female was admitted for evaluation of hoarseness and difficulty in swallowing. Bilateral palatine tonsils were enlarged, and a tonsillectomy was performed. Since a histological study revealed tonsillar tuberculosis, antituberculous agents were administered. After the treatment the pulmonary lesions detected with chest computed tomography were improved, and her symptoms were relieved. The possibility of tonsill

[Prevention of development of pulmonary tuberculosis in compromised host].

Recently compromised hosts have increased due to aging of population, advance of medical technology and therapy or changes in the dietary life and social life. Concomitantly the proportion of compromised hosts in the patients with pulmonary tuberculosis has also increased. Taking up diabetes mellitus, hemodialysis, collagen disease and lung cancer as the representatives of compromised hosts, we studied the propriety of chemoprophylaxis to prevent the development of tuberculosis and the standard for the subjects in the case of chemoprophylaxis being given. Diabetics top the patients in the high

Pulmonary tuberculosis in patients with systematic lupus erythematosus.

OBJECTIVE: The purpose of our study was to describe radiologic manifestations of pulmonary tuberculosis in patients with systemic lupus erythematosus. CONCLUSION: The prevalence of pulmonary tuberculosis was high in patients with systemic lupus erythematosus. Imaging of these patients showed miliary dissemination and patchy consolidation. However, cavitation was rare. These findings may reflect impaired immune response against tuberculous bacilli.

[The coexistence of pulmonary tuberculosis and thymoma a case report].

A 33-year-old male was admitted with complaints of cough, dysphagia, and swelling of face and upper extremities. Chest X-ray and CT scan revealed a large mediastinal mass and infiltrates in the right upper lobe. Percutaneous biopsy proved the mediastinal tumor as thymoma with cellular atypia. After irradiation, the tumor was surgically removed. Caseous epitheloid granulomas were found in the dissected mediastinal lymph nodes. AFB (Acid fast bacillus) stain of the patient's gastric fluid was positive for Mycobacterium. The coexistence of these two diseases was incidental, however, this case sug

Efficacy of the conventional diagnostic approach to pulmonary tuberculosis.

OBJECTIVE: The aim of this study was to evaluate the efficacy of the current clinical approach to the diagnosis of culture positive pulmonary tuberculosis (PTB) in hospitalised adult patients. METHOD: We examined the case records and chest X-rays (CXR) of 199 adult patients with culture positive PTB diagnosed from 1993 to 1995. Patients were divided into two groups: early treatment (ET) and delayed treatment (DT) of PTB. DT were patients who received treatment only after respiratory specimens had returned with positive culture results. We also compared the CXR of the DT group with a group of

[Pulmonary and laryngeal tuberculosis. Study of 26 patients].

A description is reported of tuberculosis cases (TBC) with symptomatic laryngeal involvement diagnosed and treated from 1982 to 1994. Twenty-six out of 2,800 (0.9%) patients diagnosed with TBC had laryngeal symptoms. Twelve patients underwent laryngeal biopsy and the disease was diagnosed in 11; the remaining 15 patients were diagnosed on the basis of the typical lesions at laryngoscopy and resolution with specific therapy; in all of them there was a pulmonary TBC associated. The mean time of clinical laryngeal symptoms was five months and the most common symptom was dysphonia. There were fact

[Clinical studies of the incidence of pulmonary and extrapulmonary tuberculosis].

On the whole 797 patients (mean age 49.5 +/- 18.6 years) with tuberculosis who were hospitalized during 1987 and the first half of the year 1994 were investigated. 148 (18.6%) of these patients were foreigners. Regarding the age, the patients of German origin were mostly above 40 years (70.5%). Within the group of foreigners, 61.5% were below 40 years. 83.3% of the patients suffered from pulmonary and 11.4% from extrapulmonary and 11.4% from extrapulmonary tuberculosis. 5.3% of these patients had both manifestations. The tuberculin tine test was positive in 95.7% and negative in 4.3%. In 63.9%

[Pulmonary tuberculosis in infants. Apropos of 6 cases].

BACKGROUND: The present increased incidence of tuberculosis in children can be explained by contacts with infected adults and/or late BCG vaccination. PATIENTS: Six infants, 3 to 18 months-old (mean: 8 months), were admitted from November 1990 to May 1992 for various reasons; only two were admitted with a diagnosis of tuberculosis based on tuberculin test and only one was given a BCG vaccine. The disease produced a broad range of symptoms. Diagnosis was based on tuberculin test and radiographic examination showing lymph node enlargement of mediastinum and segmental consolidation lesions. Perinatal outcome in pregnancies complicated by pulmonary tuberculosis.

OBJECTIVES: To assess the perinatal outcome of pregnancies complicated by active pulmonary tuberculosis. METHODS: The perinatal outcome of 79 gravidas with pulmonary tuberculosis was compared with that of 316 normal gravidas of similar age, parity and socioeconomic status. RESULTS: The mean birthweight of infants (2649 g) born to tuberculous mothers was 215 g less than that of control group (P < 0.001). Pulmonary tuberculosis was associated with an approximate 2-fold increase in prematurity (22.8% vs. 11.1%, P < 0.01), small for gestational age (20.2% vs. 7.9%, P < 0.005) and low birthweight

[The radiological pictures of pulmonary tuberculosis in HIV+ patients].

Mycobacterial infections can be found in 10% of AIDS patients. Seventy cases with HIV+ infection were examined: they exhibited clear alterations of the chest and contemporaneous isolation and identification of mycobacteria. Primary tuberculosis is the most frequent manifestation in these patients. Alveolar parenchymal mono/bilateral infiltrates are the radiographic patterns of this pulmonary condition; they usually present without excavation (45.7%). Hilar and/or mediastinal lymph nodes are usually associated, and no pleural effusions. The main problem is the differential diagnosis with pneumo

[Doppler-echocardiographic examination of patients with pulmonary tuberculosis].

Echocardiography of 110 newly diagnosed patients with pulmonary tuberculosis was undertaken to find anatomic changes and certain blood flow indices in the right heart. These changes were represented by an enlarged chamber of the left ventricle in over half of the patients and that of the left atrium in 1/3 of them (including myocardial hypertrophy also detected most commonly in the ventricular septum in 1/3 of the cases); fibroplastic changes in the anterior wall and ventricular septum; and induration of the cusps and semilunums of the tricuspid and pulmonary valves with subvalvular structures
