A case of ulnar nerve palsy due to a conduction block in the deep motor branch at the wrist is reported. The cause was a rapidly growing synovial cyst. Ultrasonography and computed tomography were performed to determine the exact location of the cyst, which was punctured and injected with corticosteroid. Function promptly returned to normal after this procedure.

We present a case of forearm soft tissue mass caused by an accessory muscle, distal and deep to flexor carpi ulnaris muscle. Imaging studies, mainly magnetic resonance and ultrasound, allow a specific diagnosis, and obviate unnecessary surgery. In this case, the symptoms associated with ulnar compression led to surgery and confirmed the preoperative diagnosis.

The mean latency value of somatosensory evoked potentials (SEP) recorded at the Erb's point, spinal and cortical level, was assessed in 20 normal subjects by percutaneous stimulation of the peripheral nerves (median, ulnar, superficial radial, tibial and peroneal). At the Erb's point and spinal cervical level, we distinguished the classically described potentials (N9, N13, N14) and the cortical "far-field" potentials such as: P17, N20, P24, N35 and sometimes P45. The central conduction time was calculated by determination of the interpeak latency N9-N13 and N13-N20 (for the upper limb) and LP-

Nerve compression syndromes may cause postburn morbidity that can often be difficult to recognize and manage. This study reviewed patients in the authors' institution who needed nerve decompression secondary to thermal or electrical burns. The objective was to evaluate the timing of nerve decompression in the burn population. A 4-year review of the authors' institution's database found 22 patients who underwent peripheral neuroplasty. This patient population included both thermal and electrical burn patients. Two patients were excluded from the study because they underwent rapid forearm amputa

OBJECTIVE: Motor conduction velocity may yield false-negative results in mild ulnar nerve entrapment at elbow (UNE). There is evidence that the clinical heterogeneity of UNE may be due to the different involvement of fascicles. We hypothesized that, if fibres to FDI are more damaged than fibres to ADM, a relative slowing of motor conduction velocity (CV) at the segment across the elbow recording from FDI (FDI-CV) versus CV at the same segment recording from ADM (ADM-CV) would occur.
METHODS: We calculated the ratio between FDI-CV and ADM-CV (IN-RATIO) in 60 consecutive UNE patients, 40 norms,

The purpose of the present study was to identify the incidence of hand symptoms related to pregnancy in a Danish population. Three hundred and thirty-five consecutive postpartum patients were interviewed by questionnaire. Hand symptoms had been noted by 16%, among these 30% described a classic median-nerve symptom distribution (carpal tunnel syndrome) and 24% of patients described an ulnar nerve distribution. Most symptoms were bilateral, commenced in the third trimester and resolved soon after delivery. There was a significant correlation of hand symptoms in pregnancy with the presence of swe

PURPOSE: The purposes of this study were to determine whether delayed-onset ulnar neuritis (DOUN) after elbow contracture release can be prevented and to compare the efficacy of ulnar nerve decompression versus subcutaneous transposition.
METHODS: A retrospective study of 563 consecutive arthroscopic elbow contracture releases was conducted. The prophylactic efficacy of (1) subcutaneous transposition, (2) ulnar nerve decompression, (3) limited ulnar nerve decompression (7 to 8 cm), and (4) mini-decompression (4 to 6 cm) was assessed prospectively. The efficacy of prophylactic strategies (tran

The compound muscle action potential from the abductor digiti minimi muscle is bi-lobed, and its second peak is formed by far-field potentials (FFPs). We investigated their origin in two patients with ulnar neuropathy at the wrist that spared the hypothenar muscles. FFPs were lost or distorted, which indicated that the deep motor branch-innervated muscles, such as the interossei, mainly contributed to the FFPs, especially to their initial N1 and steep following P1 components.

The purpose of this study was to investigate the deformation and displacement of the normal median nerve in the carpal tunnel during index finger and thumb motion, using ultrasound. Thirty wrists from 15 asymptomatic volunteers were evaluated. Cross-sectional images during motion from full extension to flexion of the index finger and thumb were recorded. On the initial and final frames, the median nerve, flexor pollicis longus (FPL), and index finger flexor digitorum superficialis (FDS) tendons were outlined. Coordinate data were recorded and median nerve cross-sectional area, perimeter, aspec

Background/Objectives: The transradial approach is widely used for vascular access in many disciplines. Radial artery occlusion (RAO) is a frequent sequel, and hand/arm pain affects 7.8% of patients. We aimed to elucidate whether RAO or ulnar artery occlusion (UAO) causes impaired neural blood flow and, thus, if symptoms may be attributable to claudication or nerve damage. Methods: Forty healthy volunteers (73% female), with a mean age of 38 years and without clinical or sonographic signs of carpal tunnel syndrome, were included. All underwent a standardized ultrasound examination (Aplio i800

Controversy surrounds the reliability of methods of treating ulnar nerve compression at the elbow. The effectiveness of submuscular anterior nerve transposition was evaluated in 33 limbs of 31 patients. The flexor-pronator Z-lengthening technique, without internal neurolysis, was used. Results were determined by patient chart reviews. Severity of preoperative nerve compression was measured using Dellon's classification. Of the 33 limbs, 6 had mild preoperative nerve compression; 7, moderate; and 20, severe. Overall outcome was evaluated using a modification of the Bishop rating system. At a me

Eight patients undergoing limb lengthening by the Ilizarov method had near-nerve needle electrode conduction studies when they developed clinical signs of nerve entrapment. The near-nerve technique was chosen because it is highly sensitive to configurational changes which appear in the action potential as injury occurs. In 10 out of 13 symptomatic limbs the near-nerve potential was found to be abnormally small and its usually smooth contour had become notched and irregular. In the remaining 3 limbs the potentials were normal initially but became abnormal as further constriction of the nerve to

Tennis injuries are common in both the upper and lower extremities. The most common, and often most difficult, upper extremity injuries are shoulder tendinitis and tennis elbow (lateral and medial). Key considerations in the treatment of tendinitis include an understanding of the injury process and the resultant character and quantity of the pathologic spectrums. Tendon degeneration rather than tendon repair is the primary pathologic entity secondary to intrinsic muscle-tendon overload. For best treatment results, the protocols of treatment, both surgical and nonsurgical, must be individualize

Magnetic resonance (MR) imaging is an effective method for helping determine the cause of wrist pain by demonstrating a broad spectrum of abnormalities, including those of bone, cartilage, ligaments, and tendons. MR imaging is useful in the detection, characterization, and staging of osseous injury and disease, although computed tomography provides superior detail in the depiction of bone. MR imaging may demonstrate irregular cartilage loss in noninflammatory arthropathies such as osteoarthritis, and its superior soft-tissue contrast makes it the method of choice for evaluating the synovial pr

BACKGROUND: Cubital tunnel syndrome (CuTS) is a prevalent compressive neuropathy with various surgical treatments but no established consensus on the optimal technique. This study compared outcomes and electromyography (EMG) findings between in situ decompression (ISD) and anterior ulnar nerve transposition (AT) and assessed correlations between preoperative EMG parameters and muscle strength.
MATERIALS AND METHODS: We retrospectively analyzed 81 patients with unilateral idiopathic CuTS. Thirty-three underwent ISD and 48 underwent AT, determined by intraoperative ulnar nerve stability. Outcom

OBJECTIVE: A randomized controlled trial was reported recently, in which simple decompression and anterior subcutaneous transposition were compared for treatment of ulnaropathy at the elbow. Clinically, both surgical options seem to be equally effective. The objective of this study is to compare the costs, from a societal standpoint, of simple decompression versus anterior subcutaneous transposition in Euros.
METHODS: Published and unpublished data from a randomized controlled trial that compared simple decompression and anterior subcutaneous transposition were used. The investigators tabulat

OBJECTIVES: In the present study we set out to obtain normative values for radial nerve F-waves, with surface recording from the extensor indicis muscle.
METHODS: Forty-nine patients with unrelated complaints were tested. Surface recording electrodes were placed on the extensor indicis muscle. This was found by asking the patient to extend the second digit against resistance. The active surface recording electrode was placed over the most distal portion of the muscle, near the radial border of the ulnar bone near the wrist. Stimulation was performed near the lateral epicondyle between the rad
