Evaluation of a methylation classifier for predicting pre-cancer lesion among women with abnormal results between HPV16/18 and cytology.

BACKGROUND: Although HPV testing and cytology detection are successful for cervical screening in China, additional procedures are urgently required to avoid misdiagnosis and overtreatment. In this multicenter study, we collected cervical samples during screening in clinics. A total of 588 women with HPV16/18+ and/or cytology result ≥HSIL+ (high-grade squamous intraepithelial lesion or worse) were referred to colposcopy for pathological diagnosis. Methylation of S5 was quantified by pyrosequencing. RESULTS: The S5 classifier separates women with ≥HSIL+ from <HSIL with a high area under the cur

Validating assessment of neonatal jaundice with transcutaneous bilirubin measurement.

In response to current trends in maternity care, nurses are assuming a major role in providing quality care for new mothers and their babies postdischarge through follow-up home care. Jaundice has been identified as the most commonly encountered cause of morbidity among healthy infants during the first week of life, and nurses must possess accurate neonatal assessment skills. Unlike nurses who work in hospital settings, those who provide home care do not have ready access to colleagues for consultation and validation of assessments. The result of unrefined assessment skills of nurses may be re

Ventriculoperitoneal shunt infection masquerading as an acute surgical abdomen.

Shunting of cerebrospinal fluid to the peritoneal cavity has brightened the outlook for children with hydrocephalus. Nine hundred sixty-nine primary ventriculoperitoneal shunts were inserted for hydrocephalus between 1970 and 1981. During this same period, 2205 shunt revisions were performed in 847 children, some of whose primary shunt had been inserted prior to 1970 or at other institutions. Nineteen patients with a ventriculoperitoneal shunt infection persented with abdominal pain, fever, and abdominal tenderness; each had acute peritonitis. Three underwent laparotomy with the preoperative d

Acute Myocardial Injury in the Setting of Hypertensive Emergency: A Case Report.

Hypertensive emergency can cause acute myocardial injury characterized by chest pain, elevated cardiac biomarkers, and ischemic electrocardiographic changes in the absence of obstructive coronary artery disease. This presentation reflects a mismatch between myocardial oxygen supply and demand, driven by severe afterload increase, microvascular dysfunction, and subendocardial ischemia. Differentiating this entity from type 1 myocardial infarction is critical, as management priorities differ significantly. Prompt but controlled blood pressure reduction remains the cornerstone of treatment and of

When Safety Promotion Impairs Access to Care: Lessons From Endoscopy Across Canada.

An informal review of existing conscious sedation policies across Canada shows that, following endoscopic procedures, many sites require that a patient be accompanied home and cared for by a support person. Though well-intended, these policies may act as a barrier to care for vulnerable persons or result in unnecessary hospital admissions. We argue that equity requires more flexibility, creativity and risk tolerance in the application or interpretation of safety-related rules in particular situations. Unexamined common practices can sometimes pull against our responsibility to promote greater

Trigeminal Trophic Syndrome: A Case Report and Literature Review.

Trigeminal trophic syndrome (TTS) is a rare neurocutaneous disorder defined by trigeminal anesthesia, facial paresthesia, and crescentic ulceration of the nasal ala. We describe an 86-year-old woman with long-standing trigeminal neuralgia who developed a suspicious ulcerated lesion on the left nasal ala. Although the initial biopsy showed atypical squamous cells suggestive of squamous cell carcinoma, the combined histopathologic, radiologic, and clinical findings supported a diagnosis of TTS. This case underscores the need to distinguish TTS from malignancy, particularly in elderly patients wi

Mimickers of Pathologic Calcifications and Uncommon Hyperechoic Lesions on Neck Ultrasound: A Pictorial Essay.

Ultrasound is essential for distinguishing malignant from benign thyroid nodules by identifying suspicious features like microcalcifications and macrocalcifications. Microcalcifications strongly indicate malignancy, while entirely calcified nodules ≥ 1 cm show an 18.4%-23.3% malignancy risk. However, not all hyperechoic lesions are calcifications. Misinterpretation of these lesions can lead to unnecessary invasive procedures like fine needle aspiration or core needle biopsy. This review highlights hyperechoic structures and lesions and their causes, helping distinguish true pathologic calcific

Generalized lymphatic anomaly involving the pleura and bone in an older male: A case report.

Generalized lymphatic anomaly (GLA) is a congenital malformation of the lymphatic vessels that is often diagnosed in early childhood. Owing to the rarity and heterogeneity of its clinical course, GLA is frequently misdiagnosed, especially in adults. A 67-year-old man was incidentally found to have bone and pleural lesions. Multiple bone lesions detected on magnetic resonance images were mistaken for malignancy, and pathological evaluation led to the diagnosis of GLA. GLA should be considered in the differential diagnosis of multiple bone lesions, and a proactive biopsy to confirm the diagnosis

Acute calcific periarthritis-a commonly misdiagnosed pathology.

Acute calcific periarthritis (ACP) is a self-limiting, monoarticular, peri-articular process of dystrophic mineral deposition and adjacent inflammation. Patients present with a sudden onset of pain, localised swelling, erythema, tenderness and restricted range of motion. Symptoms reduce in severity within 4-7 days and self resolve in 3-4 weeks. ACP is commonly misdiagnosed, in particular, as infective or inflammatory pathologies such as septic arthritis and gout. This condition has specific imaging findings which allows differentiation from other disorders when combined with the clinical prese

An Unusual Presentation of Recurrent Childhood Kimura Disease: Ileocecal Involvement.

Kimura disease commonly presents as an isolated swelling over the head and neck region. Intestinal involvement by Kimura disease in children is uncommonly reported. We report a 10-year-old boy who had presented with an ileocaecal mass with peripheral blood eosinophilia and elevated immunoglobulin E levels. The histopathologic examination from the ileocaecal mass was suggestive of Kimura disease. He had 2 recurrences once in the left axillary region and once in the bilateral cervical region. Ileocaecal involvement in Kimura disease is an uncommon presentation in childhood. Careful evaluation of

ChILD: A Pictorial Review of Pulmonary Imaging Findings in Childhood Interstitial Lung Diseases.

Childhood interstitial lung disease (chILD) is a group of lung disorders characterized by lung remodeling leading to abnormal gas exchange. ChILD is classified differently from adult interstitial lung disease and encompasses 2 broad categories: "disorders more prevalent in infancy" (<2 years) and "disorders not specific to infancy" (>2 years). High-resolution computed tomography can play an important role in the evaluation of chILD by narrowing the differential diagnosis and preventing unnecessary invasive procedures if typical imaging patterns are recognized. Thus, the pediatric radiologist s

Acute calcific periarthritis secondary to calcium hydroxyapatite crystal deposition in the wrist: a case report.

Acute calcific periarthritis (ACP) is an unusual cause of monoarticular pain characterised by the deposition of calcium hydroxyapatite in the peri-articular and intra-articular tissues. Although the most commonly affected joint is the shoulder, other joints may be involved, including the wrist. This case report describes a 57-year-old female presenting with wrist pain and swelling associated with amorphous calcification overlying the lunate. The patient improved with the use of non-steroidal anti-inflammatory drugs and splinting. Clinician awareness of the clinical presentation and radiographi

Dopamine Agonist-Associated Hiccup in Parkinson's Disease: A Case Report.

Hiccup is described as the sudden involuntary contraction of the diaphragm, and the intercostal muscles followed by the immediate closure of the glottis. Corticosteroids, benzodiazepines, and antibiotics may cause drug-induced hiccups. Dopamine agonist-induced hiccups were reported in patients diagnosed with Parkinson's Disease (PD) in small number of cases. Here we report a patient diagnosed with PD who had severe hiccups with the use of two dopamine agonists in treatment, however hiccup was not reported with the use of Levodopa. This information may help to manage the treatment of PD, and av

Fibrodysplasia ossificans progressiva: a current review of imaging findings.

OBJECTIVE: Fibrodysplasia ossificans progressiva is a rare genetic disorder characterized by congenital skeletal deformities and soft tissue masses that progress to heterotopic ossification. Deformities of the great toes are distinctive, and heterotopic ossification in the soft tissues follows an expected anatomic and temporal pattern. In addition to heterotopic ossification, osteochondromata, middle ear ossification, demyelination, lymphedema, and venous thrombosis are characteristic. Awareness of this constellation of findings is important to early diagnosis and surveillance. CONCLUSIONS: R

Acute Management of Paradoxical Vocal Fold Motion (Vocal Cord Dysfunction).

Paradoxical vocal fold motion disorder, also commonly termed vocal cord dysfunction, is a poorly understood cause of acute upper airway obstruction. Patients with paradoxical vocal fold motion frequently present to the emergency department (ED) with acute respiratory distress and stridor. Lack of familiarity with this disorder may lead to delayed diagnosis or misdiagnosis and unnecessary intubations or surgical airway procedures. Although long-term management of paradoxical vocal fold motion is well described, there is a paucity of information about acute evaluation and management. This articl

Extrapancreatic findings of IgG4-related disease.

IgG4-related disease is a systemic fibro-inflammatory condition, which includes autoimmune pancreatitis as part of the disease spectrum. Imaging has been demonstrated to play a major role in the diagnosis of autoimmune pancreatitis. Recognizing the wide spectrum of extrapancreatic manifestations of IgG4-related disease coupled with a high clinical index of suspicion will allow for an accurate and timely diagnosis to be made, thus avoiding unnecessary invasive procedures and ensuring that early effective corticosteroid therapy is commenced. This review aims to serve as a concise reference tool

Computed tomography and magnetic resonance imaging of cat-scratch disease: a report of two cases.

Cat-scratch disease (CSD) is a bacterial infection that typically causes swelling of the lymph nodes. The patients usually have a history of being scratched, bitten, or licked by cats and often by kittens. Understanding the characteristic radiological and clinical features may lead to a noninvasive diagnosis and prevent unnecessary invasive procedures. We report two cases of CSD with computed tomography and magnetic resonance imaging showing nodules or masses of lymphadenopathy with subcutaneous edema and fat infiltration in the lymphatic drainage area. The imaging features and clinical manife

Febrile myoclonus: an underreported, benign condition in infancy often misinterpreted as febrile seizures.

Massive myoclonic jerks, often presenting in a dramatic fashion during a febrile illness, have rarely been reported and have usually been related to as febrile seizures. Febrile myoclonus is usually a benign phenomenon with no neurological sequelae. However, it may be impressive enough to provoke unnecessary diagnostic workup and treatment in these patients. Thus, its recognition by the emergency room or by the primary care physician is important to provide reassurance to the parents or caregivers and to prevent unwarranted hospitalizations and diagnostic procedures such as lumbar puncture and

Fine needle aspiration cytology of adenomatoid tumour--a case report with review of literature.

Adenomatoid tumours are neoplasms of male and female genital tract with the epididymis being the most common site. They also occur in uterus, fallopian tube, and ovary. These benign tumours are asymptomatic or cause mild symptoms and a palpable mass. Fine needle aspiration of these tumours is very useful to differentiate malignant from benign lesions and helps to avoid unnecessary aggressive surgical procedures. FNAC of these benign epididymal tumours is diagnostic, rapid, reliable, conclusive and cost effective. We are reporting a case of adenomatoid tumour of epididymis in a 41 year old male
