Data analysis of my experience with the implantation of the artificial sphincter shows an overall success rate of 84%. These results are in 139 females ranging from 5 to 84 years of age. The surgery is difficult primarily because the urethral-vaginal septum is not a surgical plane. Even partial injury to these tissues can lead to failure with erosion of the device into the urethra or vagina. Psychodiagnostic studies were conducted in 100 women with urinary incontinence. The VNPI test proposed by Regel and Schulze, a neurosis screening test, was adopted. Fifty-two of the patients studied revealed test results which were either pathological or called for further elucidation. Pathological VNPI findings were obtained in 23% of the women with stress incontinence, in 48% of those with combined incontinence, and in 85% of the patients with urgency incontinence. The present study suggests that psychodiagnosis is necessary beside comprehensive somatic urinary incontinence diagnosis.

A new technique for the treatment of female stress urinary incontinence is described. It consists of colposuspension obtained by suprapubic placement of two double monofilament sutures in a spiral fashion on the lateral vaginal walls, which are fixed proximally to a Cooper's ligament and tied distally one to the other over the rectus fascia. This technique has been successfully employed in a preliminary series of 26 patients and it appears to be easy to perform and with a wide application range.

Over a one year period (September 1981 - September 1982) the authors carried out the subcervical injection of Teflon in 26 cases, 20 of which were suitable for inclusion in the present study. All the patients had true stress incontinence. Nine had undergone previous surgery, 8 had sphincter incompetence with an intact perineum and 3 were obese and/or in poor general condition. Using the Millar Microtransducer system urodynamic measurements were made before and 1 year after 100 operations for urinary stress incontinence. Seventy-eight of the 100 patients were completely continent 1 year after surgery. The results were compared to the findings in 44 normal women. The urodynamic data obtained little relation to the clinical findings because of a large scatter around the mean value. The motor unit fibre density in the external anal sphincter has been measured in 35 women with genuine stress incontinence and in 14 continent women. The mean (SD) fibre density in the normal women was 1.5 (0.15) and in the incontinent women it was 1.96 (0.34) (P less than 0.002). These data provide evidence of peripheral denervation of the external anal sphincter and imply synchronous denervation of the pelvic floor muscles. It is suggested that denervation is an aetiological factor in genuine stress incontinence.

Four hundred and eleven women with urinary incontinence were studied and treated over a 12-year period. The surgical procedures undertaken included 45 colporrahies with Kelly stitches, 12 interposition operations, 49 fascial slings, 49 Zoedler slings using Mersilene, 66 slings using dura, 136 retropubic colpourethrocystopexies, and 54 other operations. Retropubic operations, such as colpourethrocystopexy, should be considered the primary operation for stress incontinence, for if correctly executed, they give the best long-term cure rate (97%).
