The functional restoration approach to the treatment of chronic pain in patients with soft tissue and back injuries.

A prospective randomized study of 542 injured workers with continuing pain compared 271 workers who were treated at either one of two clinics that provided functional restoration with a control group of 271 subjects. Chronic pain was caused by low back injury in 78% of patients; 79% of those treated were at work 12 months after completion of treatment compared with 78% of the control subjects. When the patients were divided into subsets, based on the accident date and followed monthly, the duration of absence from work, the compensation costs, the disability award costs, and the total costs we

[The relevance of chest X-rays and respiratory function tests to the assessment of occupational hazards in asbestos factory workers (author's transl)].

The authors have analyzed the medico-legal records of 70 workers from an asbestos factory, who developed pneumoconiosis, sometimes after prolonged exposure. X-ray films were interpreted (though not without some difficulty) according to the I.L.O. classification criteria. Respiratory function tests included spirometry, gas exchange determination and blood gas measurements. There was a correlation between the duration of occupational exposure and the onset of chest lesions visible on X-ray films (pleural thickening, pulmonary fibrosis and later, pleural calcifications) and a closer correlation b

Long-term follow-up of work participation among New Zealand workers following injury: A 12-year national cohort study.

OBJECTIVES: This paper describes work participation trajectories 12-years following injury for a cohort of working age New Zealanders, identifying characteristics of membership of the trajectory groups. METHODS: Participants, injured between 2007-2009, were recruited from New Zealand's universal entitlement Accident Compensation Corporation (ACC) scheme. Trajectories considering work participation at 2- and 12-years following injury were summarised using frequencies and percentages. RESULTS: Four trajectories were defined: sustained participation (69%), sustained absence (2%), interrupted pa

Occupational infection in Korea.

Occupational infection is a human disease caused by work-associated exposure to microbial agents through human and environmental contact. According to the literature, occupational infection was the third leading cause of occupational disease (861 cases, 8.0%), and health care, agricultural, forestry, and fishery workers were risk groups in Korea. In addition, most high-risk groups have not been protected by workers' compensation, which could lead to underestimation of the exact spectrum and magnitude of the problem, and may also result in a lack of development and implementation of occupationa…

Nurse practitioners as attending providers for workers with uncomplicated back injuries: using administrative data to evaluate quality and process of care.

OBJECTIVE: The objectives of this study were 1) to identify quality and process of care indicators available in administrative workers' compensation data and to document their association with work disability outcomes, and 2) to use these indicators to assess whether nurse practitioners (NPs), recently authorized to serve as attending providers for injured workers in Washington State, performed differently than did primary care physicians (PCPs). METHODS: Quality and process of care indicators for NP and PCP back injury claims from Washington State were compared using direct standardization a…

[Professional absenteeism and surgery for carpal tunnel syndrome. Results of a prospective series of 233 patients].

AIM: The aim of our work was to evaluate the influence of the type of patient's insurance ant the return to work interval after carpal tunnel release. MATERIAL AND METHODS: An prospective analysis of 233 patients (18-65 years old, full time workers) undergoing a carpal tunnel release between January and June 1998 was conducted to determine the interval between surgery and return to work. For statistical analysis we used the Ms Works and Status software package. The mean return-to-work intervals were determined for the following and compared for three groups. Group 1: independent worker, n = 8

Pulmonary Aluminosis Diagnosed with In-air Microparticle Induced X-ray Emission Analysis of Particles.

We herein present a case of pulmonary aluminosis diagnosed with in-air microparticle induced X-ray emission (in-air micro-PIXE) analysis. The diagnosis of pulmonary aluminosis was supported by the occupational exposure to aluminum, ground glass opacity and ill-defined centrilobular nodular opacities seen in high resolution CT, and respiratory bronchioles accompanied by pigmented dust by histological examination by in-air micro-PIXE analysis of the lung tissues. The possibility of developing this rare condition should not be underestimated in workers at high-risk jobs. This is an important repo

Prioritizing prevention opportunities in the Washington State construction industry, 2003-2007.

OBJECTIVE: This study compares construction industry groups in Washington State by injury severity and cost, and ranks industry groups according to potential for prevention. METHODS: All Washington State workers' compensation compensable claims with date of injury between 2003 and 2007 were classified into North American Industrial Classification System (NAICS) industry groups. Claims were then aggregated by injury type and industry groups were ranked according to a prevention index (PI). The PI is the average of the rank orders of the claim count and the claim incidence rate. A lower PI indi…

Apportionment in asbestos-related disease for purposes of compensation.

Workers' compensation systems attempt to evaluate claims for occupational disease on an individual basis using the best guidelines available to them. This may be difficult when there is more than one risk factor associated with the outcome, such as asbestos and cigarette smoking, and the occupational exposures is not clearly responsible for the disease. Apportionment is an approach that involves an assessment of the relative contribution of work-related exposures to the risk of the disease or to the final impairment that arises for the disease. This article discusses the concept of apportionme…

Injuries to carpenters.

Study of 208 consecutively treated injuries to carpenters in northwestern Vermont revealed an annual injury rate of 13.3 to 17.2/100 workers … $70, and mean and median total costs of care were $376 and $90. Only one third of payment came from workers' compensation. Self-employed carpenters comprised 17% of injured persons but 36% of those with unpaid bills.

Occupational factors in disability pensions for gastric and duodenal ulcer.

Although a general decline in the prevalence of duodenal ulcer disease during the past 20 to 30 years has been noticed by many authors from different countries, it still lacks a clear explanation. In the present paper a relationship between occupational work load and prevalence of peptic ulcer disease was examined in order to test the hypothesis that ulcer prevalence might be related to occupational work load. The number of disability pensions granted to West German employees between 1979 and 1983 due to gastric and duodenal ulcers served as markers of peptic ulcer morbidity and were used to a

Ultraviolet radiation exposure and melanoma risk among over 2 million Ontario workers.

This study aimed to examine the association between occupational sun exposure and melanoma risk among a large cohort of 2.4 million workers. These workers were identified through accepted compensation claims, which were linked to cancer records to identify melanoma diagnoses from 1983 to 2019 in Ontario, Canada. An existing job exposure matrix, SUNJEM, was used to estimate occupational solar ultraviolet radiation (UVR) exposure as low-intermittent (intermittent/partly shaded exposure), moderate (2 to < 6 h/day), or high (≥6 h/day). The association between occupational solar UVR and melanoma ri

Evaluation of breathlessness in asbestos workers. Results of exercise testing.

We studied 120 asbestos-exposed workers seeking compensation for asbestos-related ventilatory impairment who were referred to us for evaluation of their complaint of dyspnea. We reviewed history, chest radiographs, pulmonary function studies, and exercise tests. The workers were 59.9 +/- 9.5 (mean +/- SD) yr of age and their first asbestos exposure had been 34.4 +/- 10 yr prior to the study; 63% were smokers, 19% were ex-smokers, and 18% were nonsmokers. Chest radiographs were normal in 4%, showed only pleural disease in 35%, only parenchymal diseases in 5%, and pleuroparenchymal disease in 56
