Functional Neurological Symptom Disorder Obscured by Incomplete Diagnostic Communication: A Case Report.

Functional neurological disorder (FND) is a diagnosis grounded in positive clinical signs, yet it remains vulnerable to underdocumentation and incomplete communication across care transitions, particularly when it is suspected but not formally recorded. In this case, a 24-year-old woman with a history of major trauma (three gunshot wounds sustained four years earlier) presented with sudden-onset left lower extremity (LLE) weakness, pain, and tingling coinciding with the anniversary month of her injury. Extensive neuroimaging and serologic workup were unrevealing aside from an incidental disc p

Improving OSA diagnosis: An AI-powered approach for analyzing DISE videos to assess obstruction site and severity.

BACKGROUND: Obstructive sleep apnea (OSA) is a prevalent sleep disorder, Drug-induced sleep endoscopy (DISE) offers direct visualization of upper airway obstruction and collapse. Unlike awake fiberoptic examinations, DISE allows for direct visualization of airway collapse during sleep, simulating natural sleep conditions under intravenous anesthesia. This is crucial for identifying obstruction sites and patterns, especially before surgical interventions or after failed CPAP treatment. While the VOTE classification system is commonly used to assess collapse severity at different anatomical site

Dexmedetomidine Preemptive Analgesia on Intra- and Postoperative Pain in Noncardiac Surgery: A Randomized, Double-Blind, Controlled Study.

BACKGROUND: Inadequate pain management affects over half of the 230 million major surgeries performed annually. Preemptive analgesia aims to prevent peripheral and central sensitization. Dexmedetomidine (DEX) offers non-opioid analgesia and opioid-sparing effects. This study evaluated the efficacy of preemptive DEX using the nociception index (NOX) for objective intraoperative monitoring and assessed its impact on perioperative outcomes. METHODS: In this single-center, double-blind, randomized controlled trial, 220 eligible patients were assigned to receive either intravenous DEX (1.5 µg·kg-1

Impact of Heating Inspired Gases During Mechanical Ventilation on Resolution of Hypothermia in a Porcine Model.

BACKGROUND: Hypothermia is a common finding following prehospital trauma with important consequences. Treatment includes removal of the patient from exposure and application of a vapor barrier and blankets. The role of elevated inspired airway temperatures for rewarming has been advocated for decades with little evidence of benefit. We sought to determine if warming of inspired gas up to 44°C will increase core temperature to normothermia after hypothermia as compared with standard of care warming. METHODS: Female swine, 39.0 ± 2.5 kg, were orally intubated and placed on a mechanical ventilat

Conservative Management of Negative Pressure Pulmonary Edema: A Case Report.

Negative pressure pulmonary edema (NPPE) represents a less frequently encountered, albeit critical, etiology of non-cardiogenic pulmonary edema, manifesting subsequent to acute upper airway obstruction. It is most often seen in young, healthy adults with strong inspiratory effort, usually triggered by post-extubation laryngospasm. Although the presentation can be dramatic, most patients recover completely when the condition is recognized and managed early. A young adult man underwent a laparoscopic appendectomy under general anesthesia. During emergence, he developed acute laryngospasm while b

Association Between Long-Term ACEI/ARB Use and Postoperative Pain in Hypertensive Patients: A Retrospective Cohort Study and Genetic Validation.

BACKGROUND: Angiotensin-converting enzyme inhibitors (ACEIs) and angiotensin receptor blockers (ARBs) are commonly used for hypertension management. Emerging evidence suggests that these agents may influence pain perception through neuroinflammatory and hormonal pathways. However, their impact on acute postoperative pain has not been well characterized. AIM: This study aimed to explore the hypothesis that long-term ACEI/ARB use is associated with reduced acute postoperative pain in hypertensive patients undergoing non-cardiac surgery. METHODS: We conducted a hybrid study combining a retrospe

Probe-Guided Intersphincteric Lateral Internal Sphincterotomy: A Modified Approach for Chronic Anal Fissures.

BACKGROUND: Lateral internal sphincterotomy is a well-established treatment for chronic anal fissures. Building on this, probe-guided intersphincteric lateral internal sphincterotomy uses a probe-assisted paramedian incision to integrate fissurectomy with sphincterotomy while prophylactically addressing potential crypt pathology. This anatomically guided, standardized approach broadens its applicability to both typical and complex fissures while remaining safe, reproducible, and technically accessible, particularly for less experienced surgeons. IMPACT OF INNOVATION: This modified technique i

Pain Management in Minimally Invasive Cardiac Surgery: A Review of Current Clinical Evidence.

Compared with conventional sternotomy, minimally invasive cardiac surgery (MICS) is associated with significant advantages such as reduced tissue trauma, faster recovery, and shorter hospital stay. However, the management of postoperative pain caused by intercostal nerve injury, pleural irritation, and tissue retraction remains a major challenge. Despite the less invasive nature of MICS, patients often report experiencing pain similar to that experienced following conventional cardiac surgery, particularly during the acute postoperative period. Effective pain management is essential for optimi
