Ankle clonus and soleus medium-latency reflex are stretch-induced responses. Clonus is traditionally considered to be the result of oscillation in the group Ia mediated spinal stretch reflex but the soleus medium-latency reflex response originates mainly from the activation of group II afferents. The medium latency reflex response (MLR) was recorded in soleus muscle by peroneal nerve stimulation and clonus beats were recorded in soleus muscle using EMG in 19 spastic patients. The dorsiflexion (DF) and plantarflexion (PF) times of clonus and the half-period were calculated based on accelerometr…

Humans with spinal cord injury (SCI) modulate locomotor output in response to limb load. Understanding the neural control mechanisms responsible for locomotor adaptation could provide a framework for selecting effective interventions. We quantified feedback and feedforward locomotor adaptations to limb load modulations in people with incomplete SCI. While subjects airstepped (stepping performed with kinematic assistance and 100% bodyweight support), a powered-orthosis created a dorisflexor torque during the "stance phase" of select steps producing highly controlled ankle-load perturbations. Wh…

This review article describes the postoperative magnetic resonance (MR) findings relating to surgery after tendon repair, ligament repair, and Morton's neuroma resection. The normal postoperative tendon is commonly thickened, showing signal changes that are most pronounced 3 to 6 months after surgery. Two years after tendon suture, the signal intensity should be low on T2-weighted images. The focus of the postoperative MR imaging after ankle repair is to detect the normal condition after the various surgical procedures (e.g., Broström, Watson-Jones, Evans, or Chrisman-Snook). The repaired liga…

Entrapment neuropathies of the knee, leg, ankle, and foot are often underdiagnosed, as the results of clinical examination and electrophysiologic evaluation are not always reliable. The causes of most entrapment neuropathies in the lower extremity may be divided into two major categories: (a) mechanical causes, which occur at fibrous or fibro-osseous tunnels, and (b) dynamic causes related to nerve injury during specific limb positioning. Magnetic resonance (MR) imaging, including high-resolution MR neurography, allows detailed evaluation of the course and morphology of peripheral nerves, as w…

The ankle-foot orthosis (AFO) with oil damper, GaitSolution (GS), was previously developed to make heel rocker function possible during the gait of hemiplegic patients. To clarify the characteristics of patients who show adaptation to GS use, a questionnaire was completed by the clinicians responsible for 99 patients with hemiplegia who participated in this study. Clinicians completed items concerning gait data of patients without AFO use and with GS use, adaptation to the use of GS, and patient opinion of GS use. Approximately 60% of patients achieved heel rocker function during the loading r…

BACKGROUND: It is well recognized that the alignment of the lower limb changes during early childhood. The hip-knee-ankle angle is often referred to as the mechanical femoral-tibial angle and is measured on a full-length radiograph of the lower extremity. While several authors have independently reported consistent reference values for the hip-knee-ankle angle in adults, such values have not been well documented for children. The purpose of our study was to establish reference values for the hip-knee-ankle angle and assess the relationship between it and the anatomic femoral-tibial angle in ch…

UNLABELLED: Most evidence for the efficacy of intra-articular corticosteroids is confined to the knee, with few studies considering the joints of the foot and ankle. The aim of this study was to identify the long-term efficacy of corticosteroid injection in foot and ankle joints. All patients undergoing intra-articular corticosteroid injections into foot and ankle joints over a 10-month period were recruited into the study. Patients were asked to complete a foot-related quality of life questionnaire, namely the Foot and Ankle Outcome Score, immediately before intra-articular injection and at s…

BACKGROUND: Usually the rate of lower extremity's injuries in sports such as running is known to be correlated with the height of foot arches. Foot pressure measurement studies have shown controversial results in this issue, mainly due to the complexity of the foot structure. This study aimed to investigate if any relationship exists between the MLA height and the ankle/knee injuries in professional runners. It was also aimed to find out any association between the foot pressure patterns and the clinical navicular drop test in the subjects.
MATERIALS AND METHODS: Forty-seven professional runn…

Injuries to the lateral ligaments of the ankle are common in medical practice. The most commonly injured ligaments are the anterior talofibular (ATFL) and calcaneofibular (CFL) ligaments. When undertaking MRI evaluation of ankle injuries it is important to understand the normal dimensions, appearance and variations of these ligaments. Twenty-eight consecutive patients referred for MRI of the ankle underwent bilateral scanning. The thickness of normal and abnormal ATFL and CFL was determined. The mean thickness of the ATFL was 2.19 +/- 0.6 mm and the CFL measured 2.13 +/- 0.5 mm. One normal ank…

BACKGROUND AND PURPOSE: Postural stability has been shown to be impaired after a lateral ankle sprain (LAS) and after immersion cryotherapy in healthy ankles. This study was performed to determine the effects of cryotherapy on postural stability after LAS.
METHOD: A single-session, repeated measures design was used. Fifteen 18- to 29-year-old males (mean age 21.33 +/- 3.54, height 71.23 +/- 2.50 cm, mass 170.33 +/- 19.77 kg) with relatively recent grade I LAS volunteered. Medial-lateral postural sway variability was assessed during single-leg barefoot stance using a Bertec force platform. Swa…

In this study, an ideal electromechanical KAFO, satisfying stability in the stance and knee flexion in the swing phase during walking, was developed. Biomechanical evaluations were performed on four polio patients by means of three-dimensional gait analyses and energy consumption studies. From the three-dimensional gait analysis on poliomyelitis patients, a considerable amount of knee flexion during the swing phase was observed in controlled-knee gait, which resulted in approximately 33% less energy consumption than in locked-knee gait. The developed electromechanical KAFO in this study was he

Although stabilization of the lateral ankle ligament complex (LALC) with augmented techniques is known to be successful, it is associated with a number of complications. We hypothesize that successful stabilization of LALC can be achieved with a woven polyester tape implant via a minimally invasive procedure, as an alternative to tenodesis. Four men with chronic instability of the ankle underwent a minimally invasive surgical stabilization of LALC with a woven polyester tape. This tape was passed through the distal fibula to the base of the fifth metatarsal and then back to the fibula once mor…

In a previous study, we showed that electroacupuncture (EA) applied to the SI-6 point on the contralateral forelimb produces long-lasting and powerful analgesia in pain caused by ankle sprain in a rat model. To investigate the underlying mechanism of EA analgesia, the present study tested the effects of various antagonists on known endogenous analgesic systems in this model. Ankle sprain was induced in anesthetized rats by overextending their right ankle with repeated forceful plantar flexion and inversion of the foot. When rats developed pain behaviors (a reduction in weight-bearing of the af…
