Combining value of information analysis and ethical argumentation in decisions on participation of vulnerable patients in clinical research.

BACKGROUND: The participation of vulnerable patients in clinical research poses apparent ethical dilemmas. Depending on the nature of the vulnerability, their participation may challenge the ethical principles of autonomy, non-maleficence, or justice. On the other hand, non-participation may preclude the building of a knowledge base that is a prerequisite for defining the optimal clinical management of vulnerable patients. Such clinical uncertainty may also incur substantial economic costs. MAIN TEXT: We present the participation of pre-menopausal women with atrial fibrillation in trials of n

Sufficient Sample Size and Power in Multilevel Ordinal Logistic Regression Models.

For most of the time, biomedical researchers have been dealing with ordinal outcome variable in multilevel models where patients are nested in doctors. We can justifiably apply multilevel cumulative logit model, where the outcome variable represents the mild, severe, and extremely severe intensity of diseases like malaria and typhoid in the form of ordered categories. Based on our simulation conditions, Maximum Likelihood (ML) method is better than Penalized Quasilikelihood (PQL) method in three-category ordinal outcome variable. PQL method, however, performs equally well as ML method where fi…

Involving Families with Osteogenesis Imperfecta in Health Service Research: Joint Development of the OI/ECE Questionnaire.

BACKGROUND: Despite the growing interest in understanding the psycho-social impact of rare genetic diseases, few studies examine this concept and even fewer seek to obtain feedback from families who have lived the experience. The aim of this project was to involve families of children living with osteogenesis imperfecta (OI) in the development of a tool to assess the impact of OI on the lives of patients and their families. METHODS: This project used an integrated knowledge translation approach in which knowledge users (clinicians and people living with OI and their families) were consulted t

A model for incorporating patient and stakeholder voices in a learning health care network: Washington State's Comparative Effectiveness Research Translation Network.

OBJECTIVE: To describe the inaugural comparative effectiveness research (CER) cohort study of Washington State's Comparative Effectiveness Research Translation Network (CERTAIN), which compares invasive with noninvasive treatments for peripheral artery disease, and to focus on the patient centeredness of this cohort study by describing it within the context of a newly published conceptual framework for patient-centered outcomes research (PCOR). STUDY DESIGN AND SETTING: The peripheral artery disease study was selected because of clinician-identified uncertainty in treatment selection and diff

Nonhuman Primate Models and Understanding the Pathogenesis of HIV Infection and AIDS.

Research using nonhuman primates (NHPs) as models for human immunodeficiency virus (HIV) infection and acquired immunodeficiency syndrome (AIDS) has resulted in tremendous achievements not only in the prevention and treatment of HIV, but also in biomedical research more broadly. Once considered a death sentence, HIV infection is now fairly well controlled with combination antiretroviral treatments, almost all of which were first tested for efficacy and safety in nonhuman primates or other laboratory animals. Research in NHP has led to "dogma changing" discoveries in immunology, infectious dise…

Between Simians and Cell Lines: Rhesus Monkeys, Polio Research, and the Geopolitics of Tissue Culture (1934-1954).

This essay argues that the racialized geopolitics of the rhesus monkey trade conditioned the trajectory of tissue culture in polio research. Rhesus monkeys from north India were important experimental organisms in the American "war against polio" between the 1930s and 1950s. During this period, the National Foundation for Infantile Paralysis (NFIP) expended considerable effort to secure the nonhuman primate for researchers' changing experimental agendas. The NFIP drew on transnational networks to export hundreds of thousands of rhesus monkeys from colonial and later postcolonial India amid the

Exploiting stem cell therapy: the 3rd meeting of stem cell research Italy.

The study of stem cells is one of the most exciting areas of contemporary biomedical research. During the 3rd Joint Meeting of Stem Cell Research Italy (June 2012, Ferrara, Italy), scientists from different multidisciplinary areas explored new frontiers of basic and applied stem cell research with key lectures and oral presentations. There was a public debate on ethics during the opening ceremony, specifically on the limits and potentialities of adult and embryonic stem cells. Some scientists presented basic research data showing evolutionary aspects, which could be of interest in understandin…

From gene engineering to gene modulation and manipulation: can we prevent or detect gene doping in sports?

During the last 2 decades, progress in deciphering the human gene map as well as the discovery of specific defective genes encoding particular proteins in some serious human diseases have resulted in attempts to treat sick patients with gene therapy. There has been considerable focus on human recombinant proteins which were gene-engineered and produced in vitro (insulin, growth hormone, insulin-like growth factor-1, erythropoietin). Unfortunately, these substances and methods also became improper tools for unscrupulous athletes. Biomedical research has focused on the possible direct insertion …

Towards a gender perspective in qualitative research on voluntary medical male circumcision in east and southern Africa.

The World Health Organization endorsed voluntary medical male circumcision (VMMC) in 2007 as an effective method to provide partial protection against heterosexual female-to-male transmission of HIV in regions with high rates of such transmission, and where uptake of VMMC is low. Qualitative research conducted in east and southern Africa has focused on assessing acceptability, barriers to uptake of VMMC and the likelihood of VMMC increasing men's adoption of risky sexual behaviours. Less researched, however, have been the perceptions of women and sexual minorities towards VMMC, even though the

Classifying Craniosynostosis with a 3D Projection-Based Feature Extraction System.

Craniosynostosis, a disorder in which one or more fibrous joints of the skull fuse prematurely, causes skull deformity and is associated with increased intracranial pressure and developmental delays. Although clinicians can easily diagnose craniosynostosis and can classify its type, being able to quantify the condition is an important problem in craniofacial research. While several papers have attempted this quantification through statistical models, the methods have not been intuitive to biomedical researchers and clinicians who want to use them. The goal of this work was to develop a general…

Research on leukaemia cells surplus to diagnostic needs in children.

The ability to improve diagnosis and refine prognosis in children with acute leukaemia is improving steadily. A growing number of tests can and are being performed on leukaemic cells. These include surface-marker analysis, DNA content, cytogenetics and studies of gene rearrangements. Increasingly large bone-marrow samples, now usually obtained under general anaesthesia, are required to make secure diagnoses. Ethical issues arise from three major areas. 1) Current research on leukaemia cells requested by the Medical Research Council is considered by local research ethics committees, but parents

Remarks on "Temporomandibular Disorders: Priorities for Research and Care": how will Chile advance?

Temporomandibular disorders (TMDs) are complex multi-system disorders for which common traditional dental-centric approaches to research and care unfortunately continue to prevail. A committee appointed by the National Academies of Sciences, Engineering and Medicine (NAM) of the United States of America summarized important recommendations regarding the urgent need to transform, from the predominantly biomedical model, the research, professional education/training, and patient care for TMDs into the biopsychosocial model that is standard in the rest of pain medicine. The release of the Consens

Gradient boosting machine for survival prediction in studies with general failure-time outcome-dependent sampling design.

Outcome-dependent sampling (ODS) designs are widely used in epidemiological and biomedical research as cost-effective alternatives to full cohort studies. While existing research has focused on parameter estimation and inference under specific models, survival prediction in ODS settings remains underexplored. In this paper, we propose a gradient boosting machine tailored for survival prediction in studies conducted using general failure-time ODS designs. Our approach integrates a weighting scheme to correct for sampling bias and improves predictive accuracy. We introduce a weighted empirical r…
