BACKGROUND AND OBJECTIVES: Improvement of transfusion security in sub-Saharan countries requires the determination of priorities taking into account the specific context.
PATIENTS AND METHODS: One hundred and forty patients with sickle cell disease (SCD) from one clinical centre for SCD in Kisangani, DRC were tested for HBsAg, anti-HIV antibodies, anti-HCV antibodies and for alloantibodies against red blood cells and human leucocyte antigens (HLA).
RESULTS: Thirteen patients had not been transfused and were free of HBV, HIV or HCV infection. HBV, HIV and HCV infections were detected in 2/127

Anti-HLA immunisation has been studied in 130 patients treated by haemodialysis between 1969 and 1978. Until 1977 blood transfusions were restricted (number of transfusions per patient averaged 3.42 units). However 50% of the patients were found to be immunised. The frequency of immunisation was higher in women patients (63%) than in men (39.7%). This difference shows a good correlation with the frequency of pregnancies (r = 0.849). The percentage of immunisation increases in parallel with the number of months of dialysis (r = 0.95). The capacity to eliminate HBS Ag seems to be related to the …

Intrauterine transfusion of the fetus is described in 165 pregnancies. The overall fetal survival rose from 28 per cent in 1964 to 1969 to 42-5 per cent in 1973 to 1975. Apart from technical complications of the procedure itself, the factors most likely to affect fetal survival were the gestational age and amniotic fluid optical density difference before the first intrauterine transfusion, the birth weight and the cord blood haemoglobin level.

Transfusion-related acute lung injury (TRALI) is a frequent cause of transfusion-associated morbidity and mortality. Severe TRALI is often due to antibodies in blood components directed against the human neutrophil alloantigen-3a (HNA-3a). We show here that the HNA-3a antigen arises from a nucleotide polymorphism in the choline transporter-like protein-2 gene (SLC44A2), with the resulting variation at amino acid position 154 determining the reactivity of the protein with HNA-3a-specific antibodies; the variant with an arginine at this position, rather than a glutamine, constitutes the HNA-3a a

We investigated the seroprevalence of human immunodeficiency virus (HIV), hepatitis B virus (HBV) and hepatitis C virus (HCV) infection among 258 clergymen-in-training (age range 18-39 years) who represented a donor population that fulfilled the criteria for safe blood transfusion. In all, 15.1% of the men were positive for HBsAg, 4.3% were positive for anti-HCV and 2.7% were HIV-positive; 22.1% were infected with at least one of these viruses. Co-infection with HIV and HBV was found in 0.4% of the subjects, HBV and HCV in 0.4%, and HIV and HCV in 0%. This underscores the importance of routine…

Peri-operative bleeding is associated with invasive surgery and has traditionally been compensated for by blood transfusion. Concerns about the risk of transfusion-transmitted disease have led to an increasing interest in synthetic haemostatic agents. Desmopressin (1-deamino-8-D-arginine vasopressin), a synthetic analogue of vasopressin, has been shown to be of benefit in the peri-operative management of von Willebrand's disease or mild haemophilia A. This paper addresses the role of desmopressin and bleeding during invasive surgery, particularly during cardiopulmonary bypass. Clinical trials …

In May 1994, the Blood Transfusion Centre, Sheffield, introduced a new screening test for Immunoglobulin A (IgA) deficiency. The test is performed in parallel with routine donor ABO grouping and Rh typing on two Olympus PK 7200 autoanalysers. A panel of IgA-deficient donors is required to provide blood products for IgA-deficient patients in whom the presence of anti-IgA antibodies may cause serious anaphylactic transfusion reactions (Welborn & Hersch, 1991). Detection rates during high summer became noticeably inconsistent and since a relationship between IgA deficiency and hypersensitivity st…

We report on the adaptation of a 'Walkaway' instrument (ES 600, Boehringer Mannheim) to be used in a blood transfusion service for donor screening of HBsAG and anti-HIV 1 + 2. Anti-CMV is obtainable, and anti-HCV will be available in the near future. Sample processing is done by a Tecan 8051 ID from barcoded primary tubes into ES 600 carousels. The capacity of this instrument is limited to 150 specimens and 600 tests per run. After mounting of reagents and control specimens, the tests are performed fully automatically without a technician. Regarding our experiences, the instrument is unanimous…

Important current issues in transfusion medicine in Norway are discussed. Current patient legislation specifically defines blood donors as patients, and blood and blood products are defined as drugs. Donor selection is controversial, especially deferral of all persons born in, or having lived for more than one year in areas with high prevalence of infections that are transmitted by blood. The threshold for becoming a blood donor is high, but registered donors donate frequently, e.g. 2,4 whole blood donations per year on average. Some blood banks have specialized in multicomponent aphereis tech

BACKGROUND: Thalassemia is a hereditary disease, and severe anemia is the main phenotype of major thalassemia. Furthermore, the most important method in the management of this disease is red blood cell transfusion. Regular transfusions administered 1 or 2 times every month improve prognosis and survival. However, there is higher risk of infections and iron overload, especially in transfusion-dependent thalassemia (TDT). Infections are the second leading cause of death in adult TDT, after heart failure. Higher risk of infection is also influenced by multiple blood transfusions which causes alte…

A deferral takes place when donors fail to meet the eligibility criteria for donating blood during their visit to a blood collection site. Deferral periods, which can be either permanent or temporary, are implemented to protect the well-being of both the donor and the recipient. This study aimed to investigate the frequency of deferrals and the various factors contributing to them. A retrospective analysis was conducted at the Transfusion Medicine Unit of Hospital Universiti Sains Malaysia (USM), utilizing data obtained from blood donors during the period from January 2022 to June 2023. Background/Objectives: Preoperative anemia and iron deficiency increase transfusion risk in cardiac surgery. We evaluated outcomes associated with a Patient Blood Management (PBM) pathway and assessed the robustness of these associations to baseline imbalance and secular trends. Methods: This single-center retrospective cohort study compared PBM patients treated in 2019, 2022, and 2023 with historical controls from 2017-2018. Outcomes included red blood cell (RBC) transfusion, length of stay, readmission, mortality, safety events, and costs. Analyses included multivariable regression, propensi

BACKGROUND: Convalescent plasma therapy for COVID-19 relies on transfer of anti-viral antibody from donors to recipients via plasma transfusion. The relationship between clinical characteristics and antibody response to COVID-19 is not well defined. We investigated predictors of convalescent antibody production and quantified recipient antibody response in a convalescent plasma therapy clinical trial.
METHODS: Multivariable analysis of clinical and serological parameters in 103 confirmed COVID-19 convalescent plasma donors 28 days or more following symptom resolution was performed. Mixed-effe

Sickle cell disease (SCD) remains a critical global health issue, with high child mortality in low-resource regions. Early diagnosis is essential for improving health outcomes, but conventional diagnostic methods are unsuitable for widespread use due to the high costs of laboratory equipment. There is an urgent need for portable, cost-effective, and user-friendly point-of-care diagnostic tools that can quickly assess blood health. Here, we explore two new biomarkers enabled by acoustic probing for rapid SCD diagnosis: cell membrane stability from measuring red blood cell (RBC) lysis temperatur
