Biomechanics-based Gradient Nano-surface Implants Screening and Its Adoption in Dental Implant Repair.

BACKGROUND: this study aimed to screen the micro/nano surface of pure titanium implant gradient for performance analysis, and to explore its role in dental implant repair. METHODS: after treatment with different concentrations of hydrofluoric acid and varying etching times, titanium plates with micro/nano gradient surfaces were selected and divided into four groups: polished, b, c, and d. The microscopic morphology of the titanium surfaces was observed, and the contact angle was measured. One implant was inserted into the femoral metaphysis on both sides of 28 SD rats. Histological sections w

Mandibular reconstruction with non-vascularized bone graft in a double bridging technique.

BACKGROUND: Reconstruction of the mandible following tumor resection or trauma can be challenging due to associated functional and esthetic problems. Various options have been described in the literature, but non-vascularized bone graft remain a viable option in middle- and low-income countries, Nigeria inclusive. We hereby report our experience with the use of non-vascularized bone graft with mandibular reconstruction plates in a double bridging technique for mandibular reconstruction in our institution. METHODOLOGY: Patients who had mandibular defect reconstruction with non-vascularized bon

Patellar Tendon Allograft Reconstruction.

BACKGROUND: Chronic patellar tendon injuries can cause significant functional deficits due to anterior knee pain, extension weakness, and extensor lag. With chronic injuries, the tissue is inadequate and of poor quality. Chronic injuries require autograft or allograft for augmentation or reconstruction. We present reconstruction of a chronic patellar tendon injury with Achilles tendon allograft. INDICATIONS: Chronic patellar tendon injuries causing functional deficits, including knee extension weakness, extensor lag, and anterior knee pain. TECHNIQUE DESCRIPTION: A longitudinal incision is m

A Case of Femoral Neck Insufficiency Fracture due to Tumor-Induced Osteomalacia.

Tumor-induced osteomalacia (TIO) is a rare skeletal disease caused by hypersecretion of fibroblast growth factor 23 (FGF-23) from neoplasms of mesenchymal origin; patients with TIO present with insufficiency fractures, progressive bone pain, and delayed fracture unions. Herein, we report the case of a 48-year-old man with an insufficiency fracture in his left femoral neck associated with TIO. The causative tumor located in the patient's maxillary sinus had been resected; however, complete resection was impossible due to the location of the tumor. Therefore, the patient's osteomalacia persisted

Treatment of aseptic nonunion after medial opening-wedge high tibial osteotomy.

BACKGROUND: Nonunion after medial opening-wedge high tibial osteotomy (OWHTO) is a rare but serious complication with very limited data regarding its treatment. The aim of this study was to analyze the healing rate after operative treatment of nonunion after OWHTO. METHODS: We performed a single-center, retrospective study that included 14 patients with nonunion after OWHTO between 2010 and 2018. The treatment for all patients consisted of local debridement and cancellous bone grafting at the osteotomy gap. Revision osteosynthesis due to a loss of correction/loosening of the locking screws or

Lower limb lengthening over an intramedullary nail: a long-term follow-up study of 28 cases.

BACKGROUND: Limb lengthening using an external fixator requires a long period of external fixation and may be associated with several complications such as axial deformity, fracture of the regenerated bone, and joint stiffness. With the goal of reducing the time of external fixation as well as some of these complications, we performed femoral or tibial lengthening over an intramedullary nail, according to Paley's technique, in 28 patients, followed up after a mean period of 8 years. MATERIALS AND METHODS: Twenty-eight patients treated for lower limb discrepancy by limb lengthening over an int

Reconstruction of mastoidectomy defect with titanium mesh.

OBJECTIVE: Intact-canal-wall mastoidectomy procedures leave an unsightly depression in the postauricular area. Until now, there have been few reports of successful reconstruction of mastoidectomy defects, and none using titanium mesh. When secondary mastoidectomy is not anticipated, as in endolymphatic sac shunt procedures, the postauricular defect resulting from mastoidectomy can be eliminated by reconstruction using titanium mesh. This is a retrospective study of 14 patients who underwent reconstruction of a mastoidectomy defect with titanium mesh. MATERIAL AND METHODS: All 14 patients unde

Percutaneous Imaging-Guided Screw Fixation of Osteoporotic Transverse Fractures of the Lower Sacrum with Cement Augmentation: Report of 2 Cases.

Osteoporotic fractures of the sacrum usually involve the sacral ala and can be managed with percutaneous cementoplasty if conservative therapy failed to achieve bone consolidation. On the other hand, isolated transverse fractures of the lowest sacrum are more rare, with little literature focusing on their management in the osteoporotic population. If pseudoarthrosis occurs in this location, sacroplasty is not an optimal therapeutic option because of the poor biomechanical resistance of cement to multi-directional stresses. Hence, we report two cases of chronic unhealed transverse fractures of

Construct Failure in an Atypical Femoral Fracture treated with Intramedullary Nailing: A Case Report.

The aim of this paper is to document a rare case of construct failure in a 68-year old Filipina who sustained an atypical femoral fracture (AFF) in her left subtrochanteric area. The patient previously had a 40-month history of alendronate 70mg + vitamin D 5600u therapy for osteoporosis and underwent closed intramedullary nailing for the AFF. Six months postoperatively, she began to experience progressive pain in her operated thigh. Radiographs revealed a broken nail at the proximal screw hole and non-union of the AFF. The patient was treated with exposure of the fracture site, removal of the

Posterolateral elbow dislocation with entrapment of the medial epicondyle in children: a case report.

INTRODUCTION: Elbow dislocations in children are uncommon injuries. Dislocations with associated fractures or so-called complex dislocations of the elbow can be challenging to diagnose and treat. CASE PRESENTATION: A 14-year-old male had a posterolateral elbow dislocation after a fall. Closed reduction with traction was performed. Radiographs after initial reduction showed a fragment entrapped into the humero-cubital joint. Computerized tomography scan showed the fragment belonging to the medial epicondyle. Open reduction and internal fixation with a 3.0 millimeter cannulated screw was perfor

Arthroscopic biceps tenodesis.

Surgical treatment of symptomatic pathology of the long head of the biceps tendon generally consists of either biceps tenotomy or tenodesis. Biceps tenodesis is generally recommended for younger patients and has been well described using open techniques. With advancements in arthroscopic ability and equipment, new arthroscopic techniques have recently been reported. These techniques can be especially useful when used in conjunction with other arthroscopic procedures such as distal clavicle resection, rotator cuff repair, and subacromial decompression. We present a modification of the technique

[Carpal perilunar dislocation. Clinical study (apropos of 39 cases)].

Carpal perilunar dislocations are hidden lesions with uncertain outcome and treatment which is not yet standardized. This retrospective study including 39 cases of carpal perilunar dislocations helps clarify these issues. For fresh dislocations, the diagnosis was established in 66%. Surgical treatment was performed in 26 cases. Anatomic results showed nonunion, necrosis and residual instability. Function was always decreased and was worse in open forms or after orthopedic treatment. Scaphoid screwing results were better than those from pinning. Lateral wrist x rays must always be ordered in ca

Surgical technique of low-profile dual plating for midshaft clavicle fractures.

OBJECTIVE: The aim of this surgical technique is fracture healing with anatomical alignment and less implant irritation due to smaller, low-profile plates. Equal to superior stability is provided compared to single superior- or anterior-based plates. INDICATIONS: The same general indications for surgical stabilization of clavicle fractures apply for low-profile double plating and include fracture displacement of one or more shaft width, shortening of more than 1 cm in length, and patients with high physical activity levels. Double plating is especially suitable for fractures in the midportion

Occipital Plate Fixation in the Pediatric Population.

BACKGROUND: Occipital plate fixation has been shown to improve outcomes in cervical spine fusion. There is a paucity of literature describing occipital plate fixation, especially in the pediatric population. The authors reviewed a case series of 34 patients at a pediatric hospital who underwent cervical spine fusion with occipital plate fixation between 2003 and 2016. This study describes how occipital plates aid the cervical spine union in a case series of diverse, complex pediatric patients. METHODS: Our orthopaedic database at our institution was queried for patients undergoing an instrume

Three-Dimensional Planning and Patient-Specific Drill Guides for Repair of Spondylolysis/L5 Pars Defect.

BACKGROUND: Incomplete ossification of the pars interarticularis will result in a pars defect, a common cause of low back pain in youth and strongly associated with participation in high-impact sports. If left untreated, it can result in spondylolisthesis, causing dynamic canal stenosis, low back pain, and radiculopathy. The treatment of pars defect was first described by Bucks in 1970, who used screws in the lamina placed through an upward and outward direction. However, because of the multiple inclusion and exclusion criteria and narrow margin of error, the Bucks pars repair technique is not

Innovative use of single-incision internal fixation of distal clavicle fractures augmented with coracoclavicular stabilisation.

INTRODUCTION: The management of displaced fractures of the distal clavicle remains controversial, particularly in younger patients where there is no consensus as to which surgical intervention is best. Each surgical method has unique surgical complications and rates of persistent pain and post-traumatic arthritis. We report an innovative surgical technique using a plate fixation augmented with minimally invasive tension slide coracoclavicular fixation using a cortical tenodesis button (8.5 mm). METHODS: A single-surgeon series, comprising of eleven cases, underwent retrospective review. A low
