Confirmation of needle placement within the piriformis muscle of a cadaveric specimen using anatomic landmarks and fluoroscopic guidance.

Of patients presenting to pain clinics, complaints are of low back or buttock pain with or without radicular leg symptoms is one of the most … nited States. Fluoroscopy, however, is more widely available in this setting. This study utilized a cadaveric specimen to confirm proper needle placement for piriformis or peri-sciatic injection utilizing the previously documented landmarks for fluoroscopic guidance as described by Betts. An anteroposterior of the pelvis with inclusion of the acetabular r …

Posterior ankle arthroscopy portal safety regarding proximity to the tibial and sural nerves.

The aim of this cadaveric study was to assess the relative safety of posterior ankle arthroscopy portal sites regarding their distance from the tibial and sural nerves. We dissected 20 embalmed cadaveric lower limbs, carefully exposed the nerves, preserving their original position, and established the entry points of five posterior ankle portals using pins. We measured distances with a digital calliper and used Friedman test and Wilcoxon Signed Ranks tests for statistical analyses. There was unequal safety between the five portals (p = 0.00001). There was no statistically significant differenc…

Functional anatomy of the thumb sagittal band.

PURPOSE: To study the anatomy and kinematics of the thumb sagittal band (SB) and the influence of simulated injury on the stability of the extensor pollicis longus (EPL) tendon. METHODS: Twenty-four below-elbow, cadaveric amputation specimens were used. The study had 3 components: SB anatomy, SB kinematics, and assessment of SB influence on extensor stability through simulated injury by serial sectioning. RESULTS: The radial and ulnar components of the SB originated on their respective sides of the thumb metacarpophalangeal joint from a soft tissue confluence and inserted into the margins of…

Comparison of angled versus AP view radiography for post-operative evaluation of lumbosacral spondylolisthesis: a cadaveric study.

BACKGROUND: The objective of this study was to investigate if angled radiographic views of the L5-S1 junction result in quantitatively better images in patients with lumbosacral spondylolisthesis compared to conventional AP view. METHODS: Grade I lumbosacral spondylolisthesis was simulated in cadaveric specimens and repaired using pedicle screws and posterolateral bone grafting. Angled view AP radiographs were taken at different angles and analyzed at both grade I spondylolisthesis and complete reduction (to normal). RESULTS: The results indicated that angled view radiographs provide better …

Anatomic considerations and the relationship between the piriformis muscle and the sciatic nerve.

UNLABELLED: STATING BACKGROUND: The piriformis syndrome is one of the non-discogenics causes of sciatica. It results from the compression of the sciatic nerve (SN) by the piriformis muscle (PM) in the neutral and piriformis stretch test position. The evidence of the increase in pain in the test position requires a detailed anatomical study addressing the changes that occurred in the SN and PM anatomy during the test position. The aim of this study is to examine this relationship morphometrically. MATERIALS AND METHODS: A total of 20 right and left lower limbs of ten adult cadavers were examin…

The minipterional craniotomy: technical description and anatomic assessment.

OBJECTIVE: To describe a modification of the pterional approach (PT), the minipterional craniotomy (MPT), and compare the anatomic exposure provided by these two approaches. METHODS: The anatomic exposure offered by the MPT and PT were compared in eight sides of cadaver heads using a computerized tracking system, a robotic microscope, and an image-guidance system. The area of surgical exposure, angular exposure, and anatomic limits of each craniotomy were evaluated. Three recently operated clinical cases (EGF) are also reported. RESULTS: There were no statistical differences in the total are…

[Morphometric and morphologic studies in right ventricular hypoplasia].

In hypoplastic right ventricle the dysplasia of the tricuspid valve (TV) has repercussions on the development of the right ventricle. This paper documents the results of an anatomopathologic and morphometric studies of the tricuspid valve and the right ventricle in 25 hearts with this cardiopathy, which were morphologically analyzed using the segmentary sequential system. The following measurements were made: the annulus of the tricuspid valve, the thickness of the walls of the right ventricle, the ventricular septum and the distances from the atrioventricular-apex (AV-A) and from the apex-pul

MR imaging of Liliequist's membrane.

Liliequist's membrane is an arachnoid structure well-known to neurosurgeons. However, the importance of this membrane had been lost until the development of endoscopic third ventriculostomy (ETV). ETV is superior in its minimal invasiveness, but in some subgroups of hydrocephalus, the effectiveness of ETV may be reduced. Liliequist's membrane may block the cerebrospinal fluid (CSF) flow from the defect of the third ventricle floor, which may cause failure of ETV. Liliequist's membrane can be visualized on magnetic resonance (MR) imaging in normal healthy individuals, however, its visibility is

Arteries in the posterior cervical triangle in man.

Due to frequent changes in the anatomical nomenclature of the arteries in the posterior cervical triangle (lateral cervical region), anatomical and surgical papers relating to these topics are sometimes difficult to understand and are hard to compare. These changes, coupled with improper knowledge of the gross anatomy and nomenclature of the arteries in the posterior cervical triangle, have presented difficulties in musculocutaneous flap planning, especially in plastic and reconstructive surgery. As an illustration of this concern, the term, transverse cervical artery (A. transversa colli [cer

Precise localization of motor branching and motor points: a cadeveric study.

In order to facilitate precise localization of motor nerves and motor points, and to increase effectiveness and minimize complications of neurolytic blocks. Locations of motor branching points and motor points of 31 cadavers were measured as relative to medial femoral condyle and mid posterior calf line. Needle insertion points 1.5 centimeters and 0.5 centimeters proximal to the level of medial femoral condyle yielded the best chance (66.25%-76.19%) of finding motor branches to medial gastrocnemius muscles and lateral gastrocnemius respectively. The points with greatest chance of success (67.6…

Anatomy of the presacral venous plexus: implications for rectal surgery.

The presacral venous plexus results from anastomoses between the lateral and median sacral veins, and courses into the pelvic fascia coverin … rum in order to lower the occurrence of such injuries during rectal surgery. The pelvis of 10 fresh cadavers was dissected after injection of a colored resin into the inferior vena cava, and the presacral venous plexus was studied. Four avascular tetragonal areas were common to all the specimens. The corners of a square with a side of 3 cm, centered on th …

[Bronchial arteries: anatomical and clinical studies and strategies in surgical approach].

One of the most important and controversial aspects of thoracic surgery is due to the topographical and surgical anatomy of the vascular structures involving the mediastinum. The knowledge of the so-called "vascular time", in fact, allows to face more complicated surgical situations, typical of the most specialistic thoracic surgery. The bronchial arteries represent a paradigm of what we have just stated. The study of their anatomy (number, position, origin, distribution and relationships with the mediastinal structures) is of relevant importance, not only for the interest the bronchial vascul

[Trapezo-metacarpal arthroplasty by rotation transfer of the trapezo-metacarpal joint. Anatomical study and operative technique].

INTRODUCTION: To correct trapezium dysplasia we propose a new surgical technique. The operation consists of a double osteotomy of the trapezium and of the first metacarpal base, thereby harvesting the trapezo-metacarpal joint en-bloc. Vascularization is via the radial artery pedicle divided at the first web level. The articular block is then rotated 180 degrees on the longitudinal axis of the thumb. METHOD: An anatomic study was performed on six fresh cadaveric upper limbs. The radial artery was injected with colored latex, 5 cm above the wrist. The peri-articular vascular network was studied…

Comparison of posterior approaches to the posterior incisural space: microsurgical anatomy and proposal of a new method, the occipital bi-transtentorial/falcine approach.

OBJECTIVE: Direct surgical approaches to the posterior incisural space, including the pineal region, remain as challenges for neurosurgeons. The purposes of this study were 1) to compare the surgical views in the various posterior approaches to the posterior incisural space and 2) to propose a new approach, which is a modification of the occipital transtentorial approach. METHODS: Ten adult cadaveric specimens (20 sides) were studied, using x3 to x40 magnification, after perfusion of the arteries and veins with colored silicone. Intraoperative views in the posterior approaches to lesions were…
