Biomechanical assessment of soft tissue cervical spine disorders and expert opinion in low speed collisions.

The multidisciplinary research of injury mechanisms and injury prevention requires the assessment of the technical and biomechanical circumstances of a collision; moreover, the causality assessment in the individual cases is facilitated by taking these aspects into account. In fact, only specially trained engineers and biomechanical experts are in a position to evaluate these relevant basic facts. In many crucial court cases, important technical factors such as collision angle, structural stiffness, extent of intrusion and the vehicle's velocity change are often ignored. The purely medical cau

The effects of staged static cervical flexion-distraction deformities on the patency of the vertebral arterial vasculature.

STUDY DESIGN: Thirty-five fresh frozen cervical spine specimens underwent vertebral artery cannulization and angiography to determine the static influence of the four stages of subaxial flexion-distraction injuries as described by Allen et al on vertebral artery patency. OBJECTIVES: To evaluate the degree of vertebral vessel deformation and potential injury in staged static flexion-distraction deformities of the cervical spine. SUMMARY AND BACKGROUND DATA: Asymptomatic vertebral artery injury is found in up to 19% of all patients who incur trauma to the lower cervical spine. This incidence i

Biomechanical evaluation of a new modular rod-screw implant system for posterior instrumentation of the occipito-cervical spine: in-vitro comparison with two established implant systems.

Posterior instrumentation of the occipito-cervical spine has become an established procedure in a variety of indications. The use of rod-screw systems improved posterior instrumentation as it allows optimal screw positioning adapted to the individual anatomic situation. However, there are still some drawbacks concerning the different implant designs. Therefore, a new modular rod-screw implant system has been developed to overcome some of the drawbacks of established systems. The aim of this study was to evaluate whether posterior internal fixation of the occipito-cervical spine with the new im

Endoscopic foraminotomy using MED system in cadaveric specimens.

STUDY DESIGN: Four cadavers had cervical foraminotomies performed at noncontiguous levels using either the standard open technique or the microendoscopic technique. OBJECTIVES: To evaluate the feasibility of using a minimally invasive technique for posterior decompression of cervical disc disease. SUMMARY OF BACKGROUND DATA: Even though the anterior approach is more commonly performed for the treatment of cervical disc disease, the posterior approach has distinct advantages in selected cases of foraminal stenosis and posterolateral disc herniation. Current technique, however, requires extens

MRI findings of isolated beta-2 microglobulin amyloidosis presenting as a cervical spine mass. Case report and review of literature.

A patient without prior history of renal failure or hemodialysis presented with weakness of the extremities. On magnetic resonance imaging (MRI), a peri-odontoid soft-tissue mass hypointense to muscle on both T1- and T2-weighted images was noted without obvious bony destruction. Pathological examination revealed degenerated fibrocartilage which stained for amyloid. Further, peroxidase staining for beta-2 microglobulin was positive. We describe the first case of a beta-2 microglobulin peri-odontoid amyloidoma without preceding history of hemodialysis and a review of literature is performed.

Image-guided anterior cervical corpectomy. A feasibility study.

STUDY DESIGN: A feasibility study was performed to determine the efficacy of using image-guided frameless stereotaxy to perform anterior corpectomy of the cervical spine. OBJECTIVE: To assess the feasibility of using image-guided stereotaxy in performing anterior cervical corpectomy. SUMMARY OF BACKGROUND DATA: Anterior cervical decompression including discectomy and corpectomy is a commonly performed procedure. Particular concern about invasion of the vertebral artery arises while performing this procedure to gain maximal lateral decompression. At present, surgeons have only landmarks and e

Non-traumatic acute paraplegia associated with cervical disc herniation: a case report.

BACKGROUND: Acute paraplegia attributable to disc herniation is known to occur most frequently at the thoracic level. We report a rare case of non-traumatic acute paraplegia caused by disc herniation at the cervical level. Preoperative magnetic resonance imaging (MRI) of the present case demonstrated a spinal cord lesion as a cause of paraplegia. Although this symptom is believed to disappear rapidly after surgical treatment, there have been a few reported cases with poor neurologic recovery and permanent deficits. CASE DESCRIPTION: A 61-year-old female with a history of minor neck pain suffe

A potential complication of laparoscopy--the surgeon's herniated cervical disk.

Almost all operations that were traditionally performed by open laparotomy operations can be done laparoscopically; however, surgeons may experience several negative health effects. A 37-year-old gynecologic laparoscopist had a herniated intervertebral disk at C5-6 level. Due to a negative trauma history, a possible explanation may be the nonergonomic posture that he held while performing laparoscopic surgery for many hours. To reduce the risk of this complication, we recommend that surgeons' spatial orientation and hand-eye coordination for laparoscopy be improved by sequential phases of trai

[Gait disturbance without motor and sensory involvement in cervical myelopathy--clinical course and radiological findings].

Among 100 patients with cervical myelopathy, we found 7 patients with gait disturbance in which motor or sensory involvements were absent (group A). In these patients, the gait was unsteady, but not ataxic nor spastic. Some of them showed mild hyperreflexia, but no one had Romberg's sign. We compared the effect of neck traction and radiological findings in group A with these in 25 patients who had gait disorders as well as other symptoms associated with cervical myelopathy (group B). Mean age in group A (mean 83.9 +/- 7.9 ys) was older than that in group B (mean 76.6 +/- 5.7 ys). Gait disorder

Traumatic interdural arachnoid cyst in the upper cervical spine. Case report.

The case of an interdural arachnoid cyst of traumatic origin at the C3-5 level in an 18-year-old man was admitted to our hospital with a 1-year history of progressive weakness in left limbs and numbness below the clavicles is reported. He had had a C-2 fracture at the age of 9 years without definite neurological deficits. Magnetic resonance imaging revealed a cystic lesion in the C3-5 level. Laminectomy was performed, and an interdural cyst was found. Histological examination revealed fibrous thickening of the arachnoid membrane. A cyst located in the interdural space of upper cervical spine i

Complications in children managed with immobilization in a halo vest.

Thirty-seven patients who were three to sixteen years old were managed with immobilization in a halo vest between 1987 and 1993. Twenty-four patients (65 per cent) had the halo vest applied in conjunction with operative arthrodesis of the cervical spine; the remaining thirteen patients (35 per cent) had the halo vest applied to immobilize the cervical spine after trauma. Complications occurred in twenty-five patients (68 per cent). Pin-site infections were the most common complications, developing in twenty-two patients. Grade-II infections (purulent drainage) developed more frequently in chil

"Great mimicry" in a patient with tetraparesis: a case report.

The patient is a 63-year-old Chinese man who presented with tetraparesis and urinary incontinence. The initial diagnosis was cord compression from cervical spondylosis. The patient relapsed 3 months after cervical laminectomy. The transverse myelitis picture, left optic atrophy and suggestive brainstem evoked potentials led to treatment of a presumptive demyelinating process. The presence of vitiligo, however, led to detection of high titers of antinuclear antibodies (ANA) and presence of anti-nonhistone antibodies. The patient was then diagnosed to have a lupus (SLE)-like disease, which has n

[Techniques for intubation when head and neck cannot be moved].

The inability to extend the head may be due to a blocked cervical spine or to any cervical instability imposing to maintain the head straight. Exposure of the glottis during intubation may be difficult and can be ameliorated by a stable general anesthesia, some pressure on the larynx and by charging the epiglottis. When mouth aperture is superior to 40 mm, a lighted stylet, a laryngoscope with a prism, a fiberoptic laryngoscope (Bullard) or the PCV laryngoscope represent a possible alternative to the Mac Intosh laryngoscope. If mouth aperture is superior to 20 mm but inferior to 40 mm, a ENT o

CT appearance of an unusual type of comminuted fracture/dislocation of C1 and C2.

An unusual type of traumatic comminuted fracture/dislocation of C1 and C2 is presented. The fracture differs significantly from previously described types of C1-2 complex fracture. Anatomically contoured titanium loops were used for stabilization of the occipitocervical junction. To the best of the author's knowledge this is the first description of comminuted fracture/dislocation of the bodies and laminae of C1 and C2 in the English literature.
