In fact, there is evidence to suggest that a sizable proportion of older people who are at substantially increased risk from the effects of medications acting in the central nervous system are indeed prescribed opioids, antipsychotics, and other drugs that may cause delirium as a side effect. In many instances, this prescription occurs in a dearth of evidence for any worthwhile benefit, and, at the same time, in full knowledge of the fact that the medications may cause harm. Even worse is that, in some cases, the older people who are given these medications are commonly affected by disorders o

Intracranial volume, measured through magnetic resonance imaging and/or estimated from head circumference, is heritable and correlates with cognitive traits and several neurological disorders. We performed a genome-wide association study meta-analysis of intracranial volume (n = 79 174) and found 64 associating sequence variants explaining 5.0% of its variance. We used coding variation, transcript and protein levels, to uncover 12 genes likely mediating the effect of these variants, including GLI3 and CDK6 that affect cranial synostosis and microcephaly, respectively. Intracranial volume corre

Psychobiotics are used to treat neurological disorders, including mild cognitive impairment (MCI) and Alzheimer's disease (AD). However, the mechanisms underlying their neuroprotective effects remain unclear. Herein, we report that the administration of bifidobacteria in an AD mouse model improved behavioral abnormalities and modulated gut dysbiosis. Bifidobacterium breve CCFM1025 and WX treatment significantly improved synaptic plasticity and increased the concentrations of brain-derived neurotrophic factor (BDNF), fibronectin type III domain-containing protein 5 (FNDC5), and postsynaptic den

Spinocerebellar ataxias (SCA) constitute of a group of degenerative and progressive disorders that can be identified on a molecular and cellular basis. Along with histological changes, the clinical presentation of SCA differs between subtypes. In addition to basic cerebellar dysfunction symptoms, patients with SCA develop gait ataxia, dysphagia, dysarthria, oculomotor disturbances, pyramidal and extrapyramidal disease signs, rigidity, bradycardia, sensory deficits, and mild cognitive and executive function decline. MRI scans have confirmed reduction in mass of frontal, temporal, and parietal p

Synaptic dysfunction and altered calcium homeostasis in the brain is common to many neurodegenerative disorders. Among these, Huntington disease (HD), which is inherited in an autosomal dominant fashion, can serve as a model for investigating these mechanisms. HD generally manifests in middle age as a disorder of movement, mood and cognition. An expanded polymorphic CAG repeat in the HTT gene results in progressive neurodegeneration that impacts striatal spiny projection neurons (SPNs) earliest and most severely. Striatal SPNs receive massive glutamatergic input from cortex and thalamus, and t

Research in neuroscience reveals that the brain constructs multiple representation of space. Here, we primarily focus on peripersonal space (PPS) representation, the region of space immediately surrounding our bodies and in which objects can be grasped and manipulated. We review convergent results from several generations of studies, including neurophysiological studies in animals, neuropsychological investigations in monkeys and brain-damaged patients with spatial cognition disorders, as well as recent neuroimaging experiments in neurologically normal individuals. Collectively, these studies …

A first reason for the psychiatrist to be interested in music, musicians and their artistic work, comes from the strong biographical and scientific evidence linking mood disorders and in particular, bipolar disorder, to artistic creativity. Moreover, a family association between psychopathology and creativity has been found in several studies. Important changes in mood, but also cognition, personality and behaviour can occur during all phases of manic-depressive illness and these changes have potentially important effects on creativity and productivity. Those changes are usually opposite in ma

This study evaluated the internal structure and convergent and discriminant evidence for the Colorado Learning Difficulties Questionnaire (CLDQ), a 20-item parent-report rating scale that was developed to provide a brief screening measure for learning difficulties. CLDQ ratings were obtained from parents of children in 2 large community samples and 2 samples from clinics that specialize in the assessment of learning disabilities and related disorders (total N = 8,004). Exploratory and confirmatory factor analyses revealed 5 correlated but separable dimensions that were labeled reading, math, s

Huntington's disease (HD) is a genetic brain disease characterized by loss of capacity in movement control, cognition, and emotional regulation over a period of about 30 years. Since it is well established that clinical impairments and brain atrophy can be detected decades prior to receiving a clinical diagnosis, functional neuroimaging efforts have gained momentum in HD research. In most brain disorders, there is accumulating evidence that the clinical manifestations of disease do not simply depend on the extent of tissue loss, but represent a complex balance among neuronal dysfunction, tissu

In this article, we present the results of a literature survey on case reports of the unexpected return of mental clarity and memory shortly before death, which we have called "terminal lucidity." We focus specifically on terminal lucidity in mental disorders, of which we have found 81 case references. Of these, we were able to retrieve 49 case reports, most of which had been recorded before 1849. Thereafter, comparatively few reports of terminal lucidity have been published. Some more recent publications referred to terminal lucidity in patients suffering from schizophrenia and dementia. We d

This selective review examines the lack of an explanation for the sharply increasing prevalence of autism, and the lack of any synthesis of the proliferating theories of autism. The most controversial and most widely disseminated notion for increasing prevalence is the measles-mumps-rubella/thimerosal vaccine theory. Less controversial causes that have been proposed include changes in autism diagnostic criteria, increasing services for autism, and growing awareness of the disorder. Regardless of its causes, the increasing prevalence of autism has put pressure on the field of autism research to

We describe a new hypothesis for the development of autism, that it is driven by imbalances in brain development involving enhanced effects of paternally expressed imprinted genes, deficits of effects from maternally expressed genes, or both. This hypothesis is supported by: (1) the strong genomic-imprinting component to the genetic and developmental mechanisms of autism, Angelman syndrome, Rett syndrome and Turner syndrome; (2) the core behavioural features of autism, such as self-focused behaviour, altered social interactions and language, and enhanced spatial and mechanistic cognition and a

The concept of the overvalued idea (OVI) has been debated since German psychiatrist Karl Wernicke coined the term in the late nineteenth century, describing it as an emotionally exaggerated yet psychologically comprehensible belief arising from normal cognitive processes that becomes central to an individual's mental life. Since that time, the construct has been variably reinterpreted through competing theoretical lenses, ranging from Kraepelin's biological nosology to contemporary cognitive-behavioral and multidimensional models. Despite its clinical relevance across disorders (e.g., anorexia

In the twenty-first century, cognitive impairment is a significant health problem. Function is substantially impaired by a number of neuropsychiatric and neurodegenerative diseases, such as Parkinsonism, schizophrenia, major depressive disorder, Alzheimer's disease and other types of cognitive impairment, cerebrovascular disabilities, seizure-related disorders, and brain traumas. Over time, a number of chemical messengers and signaling molecules have been identified as potential targets for treatment, and tests have been performed against these targets using both conventional and novel chemica

Self-regulation (SR) difficulties are implicated in a wide range of disorders which develop in childhood, including attention deficit hyperactivity disorder (ADHD), oppositional defiance disorder (ODD), anxiety and depression. However, the integration of the existing research evidence is challenging because of varying terminology and the wide range of tasks used, as well as the heterogeneity and comorbidity within and across diagnostic categories. The current study used the Research Domain Criteria (RDoC) framework to guide the examination of different SR processes in young children showing a

Cognitive control is associated with impulsive and harmful behaviours, such as substance abuse and suicidal behaviours, as well as major depressive disorder (MDD) and borderline personality disorder (BPD). The association between MDD and BPD is partially explained by shared pathological personality traits, which may be underpinned by aspects of cognitive control, such as response inhibition. The neural basis of response inhibition in MDD and BPD is not fully understood and could illuminate factors that differentiate between the disorders and that underlie individual differences in cross-cuttin

AIMS: Bipolar disorders are clinically complex, chronic and recurrent disorders. Few treatment options are effective across hypomanic, manic, depressive and mixed states and as continuation or maintenance treatment after initial symptom remission. The aim of this review was to provide an up-to-date overview of research on the efficacy, tolerability and cognitive effects of electroconvulsive therapy (ECT), transcranial magnetic stimulation (TMS), transcranial direct current stimulation (tDCS), magnetic seizure therapy (MST), deep brain stimulation (DBS) and vagus nerve stimulation (VNS).

Depressive disorders in elderly persons are frequent and can either first be manifested at a younger age and show a recurrent pattern during later life (ICD 10, F33.*) or have an onset beyond 60 years, which is termed late onset depression (LOD). This LOD has a higher tendency to chronification and implies an increased risk of progression, especially to vascular dementia. The multifactorial genesis of depression in old age includes psychosocial, vascular and metabolic factors and requires multimodal therapy modules at the biological and psychosocial level, which is now increasingly being empir

OBJECTIVE: To determine the efficacy of an Internet-based clinician-assisted cognitive behavioural treatment program (the Panic program) for panic disorder (with or without agoraphobia).
METHOD: Fifty-nine individuals meeting diagnostic criteria for panic disorder with agoraphobia were randomly assigned to a treatment group or to a waitlist control group. Treatment group participants completed the Panic program, comprising six on-line lessons, weekly homework assignments, received weekly email contact from a psychiatry registrar, and contributed to a moderated online discussion forum with oth

Studying Klinefelter syndrome (47,XXY), a genetically defined disorder characterized by the presence of an additional X chromosome, can reveal insights into genotype-phenotype associations. Increased vulnerability to psychiatric disorders characterized by difficulties in social interactions, such as schizophrenia and autism, has been reported for this population. The reported social difficulties in 47,XXY men may arise as a consequence of impairments in the processing of social and emotional information. The present study is the first investigation of social-emotional information processing in
