Trauma-focused therapy integrated with cognitive behavioural therapy for psychosis for people with post-traumatic stress disorder and psychosis (the STAR trial): a multicentre, pragmatic, randomised trial in the UK.

BACKGROUND: People with psychosis have high rates of post-traumatic stress disorder (PTSD), which is associated with a poor prognosis. We aimed to investigate the effectiveness of a trauma-focused therapy integrated with cognitive behavioural therapy for psychosis (CBTp) in people with psychosis. METHODS: STAR was a rater-blind, parallel-group, pragmatic randomised controlled trial conducted across five UK sites. We randomly assigned (1:1) adults with co-occurring PTSD and psychosis in secondary care to trauma-focused CBTp plus treatment as usual or treatment as usual only, using randomly var

Peer-supported mindfulness-based intervention for managing depressive symptoms in community-dwelling older adults: Protocol for a randomized controlled trial.

BACKGROUND: Depression in old age is a public health concern contributing to individual and societal burdens. Mindfulness-based stress reduction (MBSR) and mindfulness-based cognitive therapy (MBCT) have shown promise in reducing depressive symptoms. However, time commitments associated with these interventions could pose challenges to older adults. To address this, we propose a briefer mindfulness-based intervention (MBI) with peer supporters to engage and support participants experiencing time constraints and distress. This protocol describes a study examining the efficacy of a peer-supporte

Spring PGD versus waiting list control in the treatment of prolonged grief disorder (PGD): protocol for a feasibility randomised controlled trial (RCT).

Background: Prolonged Grief Disorder (PGD) is characterised by persistent longing or preoccupation with a deceased loved one, accompanied by intense emotional pain that lasts six-months or more and significantly impairs functioning. While Cognitive Behavioural Therapy (CBT) with a grief focus is effective, access is limited due to high costs and therapist shortages. Guided digital therapy, which delivers psychological support via an app or website with professional guidance, may offer a scalable solution. Building on the success of a guided digital intervention for post-traumatic stress disord

Single-item patient-rated helpfulness and improvement as an alternative to standardized questionnaires for establishing anxiety and depression treatment efficacy.

Evidence-based psychological treatments for anxiety and depression are widely used, yet roughly half of those treated do not respond. Treatment response prediction could help to optimize patient outcomes and use of clinical resources. However, existing longitudinal studies with potentially valuable predictors are unlikely to include comprehensive, prospective measures of symptoms throughout therapy. Single-item patient ratings of helpfulness and improvement are a potentially cost-effective and efficient alternative, but their relationship with typically used change score measures is unknown. D

Psychological intervention in patients with central sensitization syndromes and comorbid psychosocial trauma: a systematic review and meta-analysis.

The co-occurrence of central sensitization syndromes (CSS) and post-traumatic symptoms (PTSS) or post-traumatic stress disorder (PTSD) exacerbates impairment, and treatment is unsatisfactory in many cases. This systematic review and meta-analysis examined the effectiveness of psychological therapy in individuals with CSS and comorbid PTSS/PTSD. A search of Medline, PsycInfo, Web of Science, Pubmed, and Scopus databases (2000 to 2023) was conducted following PRISMA guidelines. Nineteen articles were selected, including nine for meta-analysis. The main psychological therapies were pain-focused c

Perspectives of digital cognitive behavioural therapy for functional dyspepsia: qualitative interviews using thematic analysis.

OBJECTIVE: Cognitive behavioural therapy (CBT) has shown efficacy in improving mental health and symptoms of functional dyspepsia (FD), a prevalent disorder of gut-brain interaction (DGBI). However, FD-specific CBT is not widely available or scalable. Therefore, this study explored the perspectives of patients with FD and clinicians who treat them on the use of digital CBT-based interventions. METHODS AND MEASURES: This qualitative study involved semi-structured interviews with 21 patients with FD and 10 clinicians. Iterative, inductive thematic analysis was conducted. RESULTS: Three patient

Mindfulness-based cognitive therapy for perceived stress and psychosomatic symptoms in Chinese adolescent girls: a mixed-methods school-based study of parent and teacher perceptions.

BACKGROUND: Mindfulness-Based Cognitive Therapy (MBCT) integrates mindfulness practices with cognitive strategies to alleviate perceived stress and its associated symptoms. While well studied in adults, evidence for school-based MBCT among adolescents with psychosomatic complaints-particularly in China-remains limited. METHODS: We conducted a quasi-experimental, mixed-methods study in three public secondary schools in DaZhou (Sichuan, China). Female students aged 14-16 were screened (N = 1,200) using the Psychosomatic Complaints Scale (PCS; cutoff > 45) and DSM-5-informed clinical interviews;

Evaluation of a virtual reality-directed brain-gut behavioural treatment inpatient program for patients with inflammatory bowel disease: protocol for a pilot feasibility trial.

INTRODUCTION: Pain is one of the most bothersome symptoms that affects patients with inflammatory bowel disease (IBD) but is often inadequately treated. Inadequate pain control in the inpatient setting not only impacts patients' experience but increases opioid use and hospital length of stay. Opioids are often considered first-line treatment for severe pain but are associated with significant morbidity and mortality in IBD. Non-steroidal anti-inflammatory drugs are a non-opioid analgesic option, but concerns regarding their contribution to IBD flares have limited their use. Brain-gut behaviour

The impact of cognitive bias modification of interpretation on hostile attribution bias, reactive aggression and neural mechanisms.

Hostile Attribution Bias (HAB) has been demonstrated to exert adverse impacts on reactive aggression (RA). Consequently, it is essential to investigate its prevention strategies and the brain-level alterations induced by these strategies. Cognitive Bias Modification of Interpretation (CBMI) has exhibited potential in modifying HAB. The current objective of this research is to explore its influence on individuals' hostile attributions bias and reactive aggression, and conduct an exploratory study on the underlying brain change mechanisms associated with behavioral alterations. The college stude

Smartphone-based approach-avoidance bias modification training for anorexia nervosa - A randomized clinical trial.

BACKGROUND: Effective treatments for anorexia nervosa (AN) are available, yet many patients do not respond to treatment or experience relapse. Cognitive bias modification aims to ameliorate cognitive biases that are assumed to contribute to the development and maintenance of AN. This study examines the efficacy of a novel mobile approach-avoidance bias modification training (ABMT) with food cues for AN. METHOD: In this RCT, 90 inpatients with AN received six sessions of active or sham ABMT with food cues or no training, alongside treatment-as-usual. Primary outcome was self-reported eating di

Severity of Psychotic-Like Experiences in Help-Seeking German Adolescents: An Exploration of Psychosocial Predictors and Psychological Treatment Outcomes.

INTRODUCTION: Psychotic-like experiences (PLEs) occur transdiagnostically in young people and are linked to an increased risk of severe psychopathology in adulthood. However, the psychosocial factors influencing PLEs severity and their distribution across diagnoses remain under-researched. Updated knowledge of these aspects is crucial for clinical understanding and treatment. This study aims to expand existing research by investigating the prevalence and psychosocial factors associated with PLEs in help-seeking adolescents, examining differences in their severity across diagnoses and multimorb

Evaluating the effectiveness of a focused CBT training for panic disorder: a randomized parallel trial.

BACKGROUND: Recovery rates for panic disorder in NHS Talking Therapies (NHSTT) services in the United Kingdom do not match those in randomized trials. Previous research has found that training therapists in 'focused cognitive behavioral therapy' (CBT) improves outcomes. The primary aim was to examine whether focused CBT delivered by trained psychological well-being practitioners (PWPs) can improve treatment outcomes for panic disorder. An exploratory aim was to evaluate the potential impact of a novel component of focused CBT, which includes the use of 'approach-supporting behaviors' (ASBs) wh…
