In the past year, 1200 consecutive eczematous patients were tested with cocamidopropylbetaine 1% aq. Contact allergy was evinced in 46 subjects (3.8%), while irritant reactions (slight erythema only) were observed in 15 cases (1.25%). 30 out of 46 patients with allergic reactions were subsequently tested with the substances used in the synthesis of cocamidopropylbetaine, together with a sample of cocamidopropylbetaine declared by the supplier to possess a greater purity. To elucidate the importance of hand eczema in the population of Gothenburg, a questionnaire was sent to 20,000 individuals aged 20-65 years, randomly selected from the population register of the city. After two reminders, a response rate of 83% was obtained. Those individuals considering themselves to have had hand eczema within the previous 12 months were invited to a dermatological examination including patch testing. 1385 persons (71%) participated. During a 3-year period 3332 patients were patch tested with ICDRG standard series including 4 rubber mixes. Additional tests were performed with individual rubber chemicals and suspected solid rubber products. Rubber sensitivity was detected in 158 (4.7%) patients. Thiuram mix caused most of the positive reactions (73) followed by carba mix (51), PPD mix (40) and mercapto mix (19). 853 hard metal workers were examined and patch tested with 20 substances from their environment, including nickel and cobalt. Nickel sensitivity was found in 2 men and 38 women. 88% of the nickel-sensitive individuals had developed a jewelry dermatitis prior to employment in the hard metal industry or before the appearance of hand eczema. 29% of the hard metal workers gave a history of slight irritant dermatitis. The frequency of type-I reactions (contact urticaria) to plaice, whiting, common dab, witch, codfish, Norwegian haddock, herring and halibut has been examined in 26 individuals; 23 of these carried through all the investigations. 65% of the volunteers had a positive scratch patch test to one or more species of fish. Delayed type (Type IV) hypersensitivity is commonly manifested in clinical practice as allergic eczematous contact dermatitis (AECD). This reaction can be transferred by lymphoid cells but not serum and parallels other T lymphocyte immune responses. A randomized double-blind left-right comparative trial was carried out between a new steroid Desoximetason 0,25% (Ibaril) and fluocinolone acetonide 0,025% cream. Evaluation of symptoms on 50 patients suffering from endogenous eczema was recorded after 1, 2, 3 and 4 weeks. There was a significantly better effect of Ibaril compared to Fluocinolone-acetonide as judged by the observer during the 1st, 3rd and 4th week of treatment. After the 2nd week treatment Ibaril showed a better effect, however, a significancy could not be established.
