Epidemiology of menstrual-related absenteeism in 44 low-income and middle-income countries: a cross-sectional analysis of Multiple Indicator Cluster Surveys.

BACKGROUND: Menstrual-related absenteeism from work, school, or social activities is an important functional indicator of poor menstrual health that disrupts women's and girls' daily lives and exacerbates gender inequality. We sought to estimate the prevalence of and factors contributing to menstrual-related absenteeism across low-income and middle-income countries. METHODS: We analysed cross-sectional data from 47 nationally or subnationally representative Multiple Indicator Cluster Surveys from 2017 to 2023, which comprised 3 193 042 individuals from 555 869 households across 44 countries;

Supply-side and demand-side factors affecting allopathic primary care service delivery in low-income and middle-income country cities.

Most people in low-income and middle-income countries (LMICs) now live in cities, as opposed to rural areas where access to care and provider choice is limited. Urban health-care provision is organised on very different patterns to those of rural care. We synthesise global evidence to show that health-care clinics are plentiful and easily accessible in LMIC cities and that they are seldom overcrowded. The costs that patients incur when they seek care are highly variable and driven mostly by drugs and diagnostics. We show that citizens have agency, often bypassing cheaper facilities to access p

Adaptation of an mHealth Solution for the Nutritional Management of Diabetes in a Low- and Middle-Income Country: Pre-Post Mixed Methods Pilot Study.

BACKGROUND: Carbohydrate counting (CC) is vital for individuals living with type 1 diabetes mellitus (T1DM); yet, formal training is often lacking in many contexts. To bridge this gap, the parents of a person living with diabetes and a team at the Geneva University Hospital (HUG) developed WebDia, a free-access app that helps patients with T1DM assess meal carbohydrates and make informed decisions regarding insulin dosage. In the context of Peru, where dietary patterns and meal compositions may differ, customizing WebDia to suit the components of the Peruvian diet becomes particularly relevant

Avoidable pitfalls on the path to health financing self-reliance in low-income and middle-income countries.

Low-income and middle-income countries (LMICs) are facing an urgent and complex challenge: how to transition to greater self-sustainability in health financing amid declining donor support. While this shift is inevitable, the policy responses it elicits carry significant implications for health system equity and access. This commentary highlights four policy choices increasingly observed in LMICs that we argue are unacceptable in the pursuit of sustainability. These include: (1) shifting the financial burden to out-of-pocket payments; (2) over-reliance on contributory health insurance schemes;

Surgical indicators for obstetrics and family planning in routine health information systems: a landscape analysis.

Strengthening use of high-quality data for surgical obstetrics and family planning is important for improving maternal and perinatal health outcomes. Routine health information systems (RHIS) represent an important data source for indicator tracking. This landscape analysis aims to describe and compare surgical obstetric and family planning indicators put forth by global multi-stakeholder groups and those that are currently captured in RHIS in nine low- and middle-income countries. The analysis focused on five indicator topics: (i) caesarean delivery, (ii) peripartum hysterectomy, (iii) female

Fertility Care in Low and Middle Income Countries: Policy, Politics, and Macro-Level Influences on Implementation in Uganda.

BACKGROUND: Infertility is a reproductive disease affecting millions globally. In Sub-Saharan Africa, the burden is considerably higher, affecting one in four couples. The psychosocial and economic impacts of infertility remain severe. Furthermore, restricted access to affordable fertility services is justified by international population reduction agendas and limited resources, resulting in inequitable access. Treatment, when available, is primarily through private sector clinics, at catastrophically high costs. For this reason, low-cost IVF (LCIVF) technologies have been developed to simplif

Birth defects services in low resource settings: implications from a community-based study in India.

Birth defects, or congenital anomalies, are important causes of child mortality, morbidity and disability. Guidelines for public health activities to address birth defects is currently unavailable for low- and low-middle- income countries leading to a critical policy and service gap. We used the results of a study conducted in eight villages under a Primary Health Centre in Pune district of India to determine the needed public health interventions to address these disorders. The study identified 90 children with congenital anomalies per 10,000 children under five years [95% CI 62.2-126.8]. Con

How inclusive were strategies to prevent the spread of COVID-19 for people with disabilities? Evidence from qualitative research in eight low- and middle-income countries.

BACKGROUND: From the outset of the pandemic there were calls to ensure people with disabilities were included in prevention and response measures, given their increased risk of health consequences from COVID-19 infection. This study sought to explore people with disabilities' experiences of inclusion in the response to the COVID-19 pandemic, to understand how such responses can be more inclusive in the future. METHODS: Qualitative interviews were conducted with 372 people with disabilities and their caregivers in Bangladesh, Ghana, India, Peru, Thailand, Türkiye (with Syrian refugees), Viet N

Early life exposure to economic shocks and association with childhood malnutrition: a pooled analysis of 230 nationwide surveys from 68 low-income and middle-income countries.

BACKGROUND: The relationship between economic growth and nutrition is not yet fully understood in the context of the new nutrition reality where most low-income and middle-income countries (LMICs) face an increasing double burden of malnutrition. We aimed to investigate the association between early life exposure to economic shocks and multiple forms of childhood malnutrition in LMICs. METHODS: We pooled cross-sectional data on children younger than 5 years from the Demographic and Health Surveys from 1990 to 2022 and longitudinal income data from the World Inequality Database. An economic sh

Building upon the core elements of antifungal stewardship: practical recommendations for effective antifungal stewardship in resource-limited settings.

INTRODUCTION: Despite the crucial importance of effective AFS in resource-limited settings, such settings remain comparatively underserved and underrepresented in terms of resource-setting-specific guidance and research. Further practical contextualization and application of current AFS best practices is thus necessary. AREAS COVERED: A panel of leading experts from diverse countries (India, Nigeria, Spain, and the US) was brought together to provide recommendations for practical and effective implementation of AFS in resource-limited settings. We have adapted and contextualized the Centers f

Adaptations for Urogynecologic Surgery in Limited-Resource Settings: How to Do More with Less.

INTRODUCTION AND HYPOTHESIS: Pelvic floor disorders affect up to 50% of women in limited-resource settings (LRS) but are severely under-treated. Historically, attention has focused on urogenital fistulae, but pelvic organ prolapse (POP) is emerging as a growing issue, especially for women engaged in manual labor. Women in these regions often endure their conditions in silence owing to social stigma and mental health impacts, compounded by health care access barriers. Delivery of urogynecological services in LRS requires adaptable surgical models and skills because of limited tools and equipmen
