In recent years, several techniques for the treatment of more complex forms of faecal incontinence have emerged. These consist of either transposition of skeletal muscle (gracilis muscle or gluteus maximus muscle) around the anal canal, implantation of an artificial sphincter or implantation of a neuromuscular stimulator having previously transposed gracilis muscle. The latter method seems to hold most promise.

Although combined glottography has potential as a clinical tool, it has had limited use in the monitoring of laryngeal function before and after treatment. There is no data about the standardization nor reproducibility of the procedure. An indication of the reproducibility of combined glottography is paramount if changes in laryngeal function are to be measured accurately. Using a standardized technique, two complimentary studies examined the reproducibility of combined glottography within one sitting (Study 1) and over time (Study 2). The components of variance were analysed (ANOVA). In Study

In the past few years the following methods have been accepted as the treatment of choice in trigeminal neuralgia: transcutaneous electrical nerve stimulation (TENS), drug treatment--especially carbamazepine, controlled thermocoagulation, percutaneous retrogasserian rhizotomy with glycerol, microvascular decompression. The attending physician will prefer the treatment from which he expects the best results. However, the choice of treatment should take into consideration the age and general condition of the patient, the possible side effects, the quality of pain and treatments already performed.

Neurogenic urinary tract dysfunction is characterized by inadequate voiding and urinary incontinence. The aim of therapy nowadays is adequate bladder emptying and control of urinary incontinence. Neurogenic urinary incontinence can be caused by (a) detrusor hyperreflexia, (b) sphincter hypo- or areflexia, (c) a combination of both, or also (d) detrusor hyporeflexia with consequent overlow incontinence. Based on a simple urodynamic classification the current treatment strategies are presented. (a) Detrusor hyperreflexia can be transformed into hypo- or are-flexia pharmacologically with potent d

This study examined the effects of transmural nerve stimulation, acetylcholine, adrenoceptor agonists and several peptides on the contractility of strips of human gallbladder in vitro. Acetylcholine caused concentration-related contractions of the tissues and the sensitivity to acetylcholine was similar in gallbladders with mild and severe chronic cholecystitis. Noradrenaline and adrenaline relaxed gallbladder strips, probably via beta 2-adrenoceptor stimulation. Transmural nerve stimulation always caused contractions, but in the presence of atropine inhibitory responses were demonstrable and

Tooth pulp stimulation in the rabbit produces a complex rhythmic lick/chew response (LCR). Because of the effect of fentanyl on the afferent and efferent components of oral function we examined its effect on this response. Intravenous fentanyl attenuates the LCR in a dose dependent manner with the ED50 occurring at 0.1 mg/kg. Concurrently a different response to tooth stimulation appears consisting of a slow, non-rhythmic lateral jaw movement (LJR). The effects of fentanyl on both LCR and LJR are blocked by naloxone (0.1 mg/kg). The dopamine antagonist, chlorpromazine (10 mg/kg iv) also has th

In form of a survey is demonstrated which contributions make, apart from clinic and routine ECG, the storage ECG, the load ECG as well as special ECG derivation techniques, such as the oesophagus ECG, intracardiac ECG derivations with electrostimulation as well as the endocardiac mapping technique for the analysis of various forms of supraventricular tachycardias including the preexcitation syndromes. Also the discussion of medicamentous and electrotherapeutic methods is performed above all under the clinical aspects of applicability, in which cases the hierarchy and combination in therapy are

In this paper we employ the data described in the previous paper (I) to derive the current-voltage (I-V) relations of the basolateral membrane, the amiloride-insensitive "leak" pathway across the apical membrane, and the parallel pathways across rabbit descending colon. The results indicated that: a) The resistance of the basolateral membrane is independent of the electrical potential difference across that barrier over the range -8 to 67 mV and averaged 195 omega cm2. The electromotive force across this barrier averaged 50 mV under control conditions and 48 mV in the presence of amiloride. Th

The effect of early movement on flexion return in the repaired deep flexor tendon of 85 chicken toes was assessed. Postoperatively, the leg was immobilized in plaster. Beginning 18 days after surgery, minimal flexion of the third toe was induced by electrical stimulation of the deep flexor muscle in 38 chickens. Thirty-five days after surgery the plaster was removed and during the next 25 days the flexion was measured 15 times and compared with preoperative findings in the same chicken. In birds with early movement, averages of the last 4 daily measurements of total flexion were significantly

Prolonged, bright luminescent glows in Porichthys photophores are elicited by administration of 2,4-dinitrophenol (DNP) and potassium cyanide (KCN). Ultrastructural alterations of varicose nerve endings precede photocyte changes during such luminescent activity. Common alterations of nerve profiles include mitochondrial disruptions, flattening and depletion of synaptic vesicles, formation of large vacuolar cisternae, and invaginations in the contour of axolemma. Protracted luminescent activity in response to DNP results in depletion of photocyte vesicle material while vesicle and ER membranes

The phoniatric-logopedic therapeutical possibilities and results of the two most frequent causes of paralysis following a Strumectomy are presented here. The major aspect of this treatment is the application of electric therapy. Its correct usage will be explained. This "conservative" therapy should precede every operative measure and should follow again once the wound has healed. A new post-operative phoniatric therapy method following Glottis widening surgery will also be described. This therapy is based upon the functioning of the Ventricularis muscle, which operates independent of the Recu

A percutaneous technique is described for implanting a dorsal or ventral column stimulator for pain control with minimal surgical trauma. If the patient does not respond to percutaneous on-surface spinal stimulation, a percutaneous cordotomy can be carried out, as the needle is already in the correct position. Stimulation of the cord at the C1-2 level can be used for treatment of trigeminal neuralgia.
