Sepsis in the Emergency Department: Different Faces, Same Fate? A Sex-Based Prognostic Analysis.

We aimed to evaluate the clinical characteristics of male and female patients with sepsis and to investigate their potential sex-specific differences in the prognostic weight. Retrospective analysis, including all patients with sepsis admitted to the Emergency Department High-Dependency Unit, between June 2008 and October 2025. Clinical data were collected using a standardized template. The endpoints were day-7 and day-28 mortality rates. The study population included 1527 patients, with a mean age of 74 ± 13 years, and female sex comprising 44%. Women were older (77 ± 13 vs. 73 ± 13 yrs, p ≤

Assessment of Artificial Intelligence-based Translation Tools for Emergency Department Discharge Instructions.

INTRODUCTION: Emergency departments (ED) in the United States serve as a safety net for millions, including those with limited English proficiency (LEP). Eight percent of individuals living in the United States have LEP, placing them at risk for language barriers that can adversely affect the quality and safety of their care. Many hospitals lack language-concordant care, especially at the time of discharge. Miscommunication at discharge can lead to adverse health outcomes, including medication errors, poor compliance, and unnecessary return visits to the ED. Our objectives in this study were t

Risk Factors for Invasive Infection in Febrile Oncology Patients Related to Cancer Type.

OBJECTIVE: To identify clinical and laboratory risk factors for invasive infection in febrile oncology patients seen in the Pediatric Emergency Department, depending on the type of cancer. METHODS: We conducted a prospective observational study of febrile oncology patients seen between 2016 and 2023 at the Pediatric Emergency Department of a tertiary teaching hospital. Invasive infection was defined microbiologically (isolation of a bacterial pathogen in sterile fluid or a commensal bacterial species in 2 different blood cultures) or clinically (death, meeting Phoenix criteria for sepsis, rec

Analytically Confirmed Opioid Detections From the Emerging Drugs Network of Australia.

OBJECTIVE: We describe analytically confirmed opioid-related presentations to emergency departments (EDs) in Australia. METHODS: Presentations with analytically confirmed opioid exposure, between January 2021 and December 2024, were identified from the Emerging Drugs Network of Australia (EDNA) Clinical Registry. Demographic data, clinical features, outcomes, naloxone dosing, and analytical toxicology results were extracted. RESULTS: Opioids were detected in 793 ED presentations. The majority were male (568, 71.6%) with a median age of 40 years (Q1-Q3: 30-48 years). Patients were unwell (Aus

Emergency surgical management of nontraumatic pathologic spinal pain diagnosed in the emergency department.

BACKGROUND: This study aimed to evaluate patients who presented to the emergency department with non-traumatic back or lumbar pain, without new-onset neurological deficits or cauda equina syndrome. Because of inadequate response to medical treatment, patients underwent imaging of the affected region and subsequently required emergency neurosurgical intervention. METHODS: We included patients aged ≥18 years who presented to the emergency department with back or lumbar pain, experienced no pain relief despite analgesic treatment, underwent imaging of the affected region, and were referred for e

Vertical Displacement Index and Early Treatment-Related Physiological Improvement in Acute Respiratory Failure: An Exploratory Ultrasound-Based Study.

Objective: Rapid assessment of early treatment-related physiological improvement in emergency department (ED) patients with respiratory failure (RF) remains challenging. Blood gas analysis is informative but invasive and not ideal for repeated use. The vertical displacement index (VDI), an ultrasound-derived parameter based on pleural motion, may provide dynamic bedside information on early physiological change. This study evaluated whether changes in VDI are associated with early physiological improvement in ED patients with RF. Methods: This prospective observational study was conducted in t

Isolated deep orbital haemangioma in an infant presenting to the emergency department with right-sided proptosis.

Proptosis in early childhood can be an alarming and stressful situation for parents and paediatricians. Different types of anatomical structures can be found in the orbit, such as nerves, muscles, veins, arteries, glandular and connective tissue and hence the differential diagnosis of unilateral proptosis can be vast. In some cases, orbital disease has the potential to cause substantial morbidity and mortality. Appropriate medical history, examination and diagnostic evaluation with imaging can assist with a prompt diagnosis and management. In young infants, a heightened level of suspicion shou

Evaluating GPT-4o for emergency disposition of complex respiratory cases with pulmonology consultation: a diagnostic accuracy study.

BACKGROUND: Large Language Models (LLMs), such as GPT-4o, are increasingly investigated for clinical decision support in emergency medicine. However, their real-world performance in disposition prediction remains insufficiently studied. This study evaluated the diagnostic accuracy of GPT-4o in predicting ED disposition-discharge, ward admission, or ICU admission-in complex emergency respiratory cases requiring pulmonology consultation and chest CT, representing a selective high-acuity subgroup of ED patients. METHODS: We conducted a retrospective observational study in a tertiary ED between N

Emergency Department Boarding, Inpatient Census, and Interhospital Transfer Acceptances.

IMPORTANCE: Referral hospitals in the US are experiencing unprecedented levels of crowding, leading them to increasingly refuse interhospital transfer (IHT) requests. Crowded hospitals are dangerous, but refusing IHTs undermines the role of referral hospitals and may cause harm. OBJECTIVE: To measure associations of hospital crowding measures (emergency department [ED] boarding and inpatient census) with IHT acceptances overall and for prioritized conditions. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study from January 2019 to May 2023 analyzed data from the only academic and l

Implementing best practice for peripheral intravenous cannula use in Australian emergency departments: a stepped-wedge cluster-controlled trial and health economic analysis protocol.

INTRODUCTION: Over one billion adults attend emergency departments (EDs) internationally every year, including 6.6 million in Australia. Up to half of these patients have a peripheral intravenous catheter (PIVC) inserted. Although healthcare workers believe that placing a cannula is helpful ('just in case'), PIVCs often remain idle. PIVC insertion is painful for patients, takes clinicians' attention away from other care, has adverse outcomes and causes major economic and environmental burden. Our aim is to codesign an implementation toolkit to reduce unnecessary PIVC insertions and improve oth

The role of POCUS in diagnosing acute heart failure in the emergency department: A meta-analysis.

The role of point-of-care ultrasound (POCUS) in diagnosing acute heart failure is not fully established. We evaluated the diagnostic accuracy of cardiac, lung, and inferior vena cava (IVC) POCUS, individually and combined, in the emergency department (ED). We systematically searched PubMed, Embase, Web of Science, and Google Scholar for English-language studies published from inception to November 2024. Statistical analyses were conducted using STATA 16, and study quality was assessed with the QUADAS-2 tool. Fifteen studies involving 2751 dyspneic patients in the ED were included. Lung ultraso

Management of tracheostomal hemorrhage complicated by obstructing clot in a post-laryngectomy patient presenting to a rural emergency department.

Total laryngectomy (TLE) results in the permanent separation of the respiratory and digestive tracts, requiring all airway interventions to occur exclusively via a neck stoma. Although airway obstruction in post-laryngectomy patients is uncommon, it can rapidly become fatal without prompt recognition and understanding of the altered anatomy. Here, we report the case of a patient with a recent TLE for squamous cell carcinoma, who presented to a rural Emergency Department (ED) in acute respiratory distress. Emergency Medical Service (EMS) providers reported profuse tracheostomal bleeding and hyp

Continuous erector spinae plane block for abdominal visceral cancer analgesia in the emergency department: the case for catheters.

Abdominal visceral cancer pain is a challenging oncology presentation to treat in the ED. Current emergency oncologic pain management strategies focus largely on parenteral opioid administration, although ultrasound-guided regional anaesthesia has the potential to provide more targeted, superior and long-lasting analgesia without comparable adverse side effects. In particular, a continuous erector spinae plane block (ESPB) performed between T6 and L2 represents a promising analgesic tool in this patient population. Anterior spread of local anaesthetic from the erector spinae plane to the parav

Enhancing the emergency department experience for older adults: Study protocol for the implementation of a comfort menu and cart.

INTRODUCTION: The aging of the population is a global phenomenon, with projections indicating a significant increase in the proportion of individuals aged 65 years and older by 2050. This demographic shift requires adapting emergency department (ED) services to meet the specific demands of older patients, who often present with multiple comorbidities and face challenges such as sensory and cognitive difficulties. EDs, traditionally designed for acute illness and injury management, may not be adequately equipped to meet the unique needs of this vulnerable population. This can result in suboptim
