Functional outcome in limb-salvage surgery for soft tissue tumours of the foot and ankle.

Purpose. This paper describes the functional and oncologic outcome of 30 cases (in 29 patients) treated with limb-salvage surgery for localized soft tissue sarcoma (STS) or fibromatosis of the foot and ankle.Subjects. Patients were eligible for the study if they had a STS or fibromatosis in the distal one-third of the tibia or the foot such that ablative surgery would require a below-knee amputation; had no metastatic disease at presentation; and had a minimum of 2 years of follow-up.Methods. Function was prospectively evaluated using the modified Enneking functional rating scale (MSTS) at 3, …

Plantar-advancement skin flap for central ray resections in the foot: description of a technique.

A surgical technique using an advancement plantar skin flap to maintain the resection gap closure when performing central ray resections is described and illustrated. The concept, applicable to central polydactyly or polysyndactyly of the foot, was developed with goals of providing a skin tether to maintain a normally narrow foot as well as to improve the appearance of the surgical scar.

Musculofascial flaps based on the dorsalis pedis vascular pedicle for coverage of the foot and ankle.

Soft-tissue reconstruction of the foot and ankle has long presented challenging problems for the plastic surgeon. Limitations of available local tissue, the need for specialized tissue, and donor site morbidity restrict the options available to the reconstructive surgeon. In an effort to solve these difficult problems, we have begun to use musculofascial flaps based on the branches of the dorsalis pedis artery. We present our early experience of 5 patients treated with an extensor digitorum brevis muscle flap with fascial extensions often containing the contents of the first web space. Our pat…

Varus malalignment of the talar neck. Its effect on the position of the foot and on subtalar motion.

We performed an in vitro study on twelve specimens of the foot and ankle from cadavera to determine whether varus malalignment of the talar neck alters the position of the foot and subtalar motion. An osteotomy of the talar neck was performed, and the specimens were studied with and without removal of a medially based wedge of bone. Removal of the wedge produced an average varus malalignment of the talar neck of 17.1 +/- 2.4 degrees (range, 12.5 to 21.0 degrees). In the coronal plane, the average arc of motion of the subtalar joint decreased from 17.2 +/- 3.3 degrees before the osteotomy to 11…

African swine fever interference with foot-and-mouth disease infection and seroconversion in pigs.

Initial oral infection of pigs with either highly virulent (L-60) or moderately virulent (DR-2) African swine fever virus (ASFV), followed in 3 days with exposure to foot-and-mouth disease virus (FMDV) (tongue inoculation and contact), failed to cause FMDV infection or seroconversion in 18 of 22 L-60-infected pigs and 13 of 34 DR-2-infected pigs. Of the 13 DR-2-infected pigs remaining free of foot-and-mouth disease (FMD), 2 pigs survived to 24 days without antibody to FMDV, despite constant contact with clinically infected pigs with FMD. Three other DR-2-infected pigs never developed FMD lesio…

The influence of footwear on the prevalence of flat foot. A survey of 1846 skeletally mature persons.

We analysed the static footprints of 1846 skeletally mature individuals to establish the influence of the age at which shoe-wearing began on the prevalence of flat foot. The incidence was 3.24% among those who started to wear shoes before the age of six years, 3.27% in those who began between the ages of 6 and 15 and 1.75% in those who first wore shoes at the age of 16 (p < 0.001). Flat foot was highest in those who, as children, wore footwear for over eight hours each day. Obese individuals and those with ligament laxity had a higher prevalence of flat foot (p < 0.01 and p < 0.0001, respectiv…

Foot shock-induced changes in blood and brain serotonin and related substances in rats.

The effects of electric foot shock on peripheral and central serotonergic systems in rats have been studied. We have focused on the time course alterations with particular attention being paid to changes in 5-HT, 5-HIAA, tryptophan concentrations and 5-HIAA/5-HT ratios in blood and various parts of the brain, observed within 1 h following stress application. Blood and brain (7 regions) samples were taken immediately after electric foot shock, 30 min, 1 and 24 h later. In the blood stress induced a rise in tryptophan level as well as rises in 5-HT, 5-HIAA levels and 5-HIAA/5-HT ratio within 1 h…

Salvage of a foot by free transplants from the contralateral leg in a 5-year-old.

A 5-year-old girl had a traumatic amputation of her left thigh and on the right side loss of the posterior part of the talus and parts of the posterior tibial nerve and artery. These defects were treated with free transplantation of the corresponding part of the talus from the amputated left leg and a vascularized graft from the left posterior tibial nerve and artery. 2 years later the girl was able to walk without crutches, the ankle mobility was good, and so was the sensibility in the sole of the foot.

Selection of recipient vessels for free flaps to the distal leg and foot following trauma.

The recipient vessels are of prime importance in free flap transfers to the lower limbs. To determine the incidence and pattern of vascular trauma, a study was carried out in 126 patients who had Gustillo Type III open fractures of the distal legs and feet that required free flaps for wound coverage. In comparison with the posterior tibial artery, the anterior tibial artery has a much higher incidence of injury with more extensive damage. It can be injured at a more proximal level than estimated from gross inspection during surgery. This should be borne in mind when the anterior tibial artery

Alkoxyresorufin metabolism in white-footed mice at relevant environmental concentrations of Aroclor 1254.

Recent investigations have detected polychlorinated biphenyl (PCB) body burdens in wild white-footed mice (Peromyscus leucopus) captured at hazardous waste sites. Insufficient information is currently available to interpret the toxicological significance of these body burdens. In an effort to provide this information, we investigated hepatic changes and PCB body burdens in white-footed mice following a 21-day dietary exposure to a PCB mixture, Aroclor 1254. Dietary concentrations tested were 0, 2.5, 25, 50, and 100 mg Aroclor 1254/kg diet (reported as ppm). Liver weights were significantly inc…

Foot-ground pressure pattern of flexible flatfoot in children, with and without correction of calcaneovalgus.

The footprint-measuring instrument was used to establish foot-ground pressure pattern of flexible flatfeet in male and female children, aged four to six years. Standing at ease, the children exerted most of their ground pressure by the posterior weight-bearing area (WBA). The rest was distributed between the middle and anterior WBAs, with the middle area usually exerting about 17%, and in extreme cases, as much as 30% of the total foot-ground pressure. Correcting valgus inclination of the calcaneus into neutral by inserting a leather wedge under the medial portion of the heel restored the long…

Metatarsal head resection in the deformed, symptomatic rheumatic foot. A comparison of two methods.

Metatarsal head resection for the symptomatic, deformed, rheumatic foot was performed in 83 feet of 49 patients via a modified plantar approach with functional after treatment, and in 64 feet of 41 patients via a dorsal approach, followed by placement in a plaster cast for 6 weeks. Both groups were comparable with regard to age, sex, and follow-up. From 1975 to 1985, 147 feet (90 patients) were operated in two different hospitals. Of these, 25 patients (39 feet) were lost to follow-up. The average follow-up was 83.3 mo. (range 33-136 mo.). Data were obtained by studying the medical records and…

[Surgical reconstruction of post-traumatic tissue loss of the weight-bearing sole of the foot].

The repair of the weight bearing zones of the foot is a difficult problem for which an ideal solution does not exist. The contribution of pedicled flaps and free flaps allows it nevertheless to obtain reconstructions of good quality, which are however prone to mechanical complications. The pedicled flaps most commonly used are the instep island flap, the lateral supra-malleolar flap and the cross-leg flap. Among the free flaps figure the latissimus dorsi flap, the forearm flap and the free scapular flap. The sensitivity of the flap does not seem to be a decisive advantage if the receptor regio…

[The significance of occupational factors in the pathogenesis of foot mycoses in miners in deep coal mines].

Skin and peripheral vessel functions, allergic status of the body, lysosomal enzymes activities, lipid peroxidation and antioxidant system status were examined in 280 coal miners suffering from mycoses of the soles, as were their working conditions, in order to clear out some aspects in the pathogenesis of this condition. The data evidence grave disorders of the function of the skin and peripheral vessels of the limbs, of the allergic status of the body, and of lipid peroxidation and antioxidant system, this pointing to a possible contribution of occupational factors to some aspects in the pat

Shock absorption of below-knee prostheses: a comparison between the SACH and the Multiflex foot.

Shock waves were measured during walking on a treadmill on the metal tube of a below-knee KBM prosthesis, provided either with a SACH foot or with a Multiflex foot. Accelerations were measured in the axial direction and the dorso-ventral direction, about 160 mm proximal to the sole of the shoe. The accelerations had comparable amplitudes to those measured on normal legs. Dorso-ventral amplitudes (order of magnitude 4 g) were generally higher than the axial amplitudes. For some patients, the SACH foot gave much higher axial accelerations than the Multiflex foot did. In the dorso-ventral directi…

The structure of foot-and-mouth disease virus: implications for its physical and biological properties.

The structure of foot-and-mouth disease virus has been solved at a resolution of 2.9 A by X-ray diffraction techniques. The overall structural organisation of the particle is similar to that seen in other picornaviruses but there are several unique features. Many of these help to explain its characteristic physical and biological properties. In particular the canyon or pit found at the surface of other picornaviruses is lacking, which has important implications for cell attachment and the process of infection. Also there are 60 large disordered protrusions at the surface corresponding to the m…

Bilateral absence of the ulna in twins as a manifestation of the split hand--split foot deformity.

Bilateral absence of the ulna and ulnar rays in twin female infants are described as a manifestation of the split hand--split foot deformity (ectrodactyly). Family history revealed the father had a unilateral split hand. Given the wide variability of expression of this disorder and the availability of prenatal diagnosis, there is a need for obstetricians and pediatricians to recognize parents with this malformation and appreciate the potential for severely affected offspring.
