Eliminating hepatitis C in a rural Appalachian county: protocol for the Kentucky Viral Hepatitis Treatment Study (KeY Treat), a phase IV, single-arm, open-label trial of sofosbuvir/velpatasvir for the treatment of hepatitis C.

OBJECTIVES: The overall goal of the Kentucky Viral Hepatitis Treatment Study (KeY Treat) is to eliminate hepatitis C transmission from a county in Appalachian Kentucky by removing the barriers to accessing hepatitis C virus (HCV) treatment. METHODS/ANALYSIS: KeY Treat is a phase IV, open-label, single-arm clinical trial of sofosbuvir/velpatasvir (SOF/VEL) for the treatment of viraemic HCV infections. Those eligible for KeY Treat are at least 18 years of age, viraemic and are residents of the target county. Pregnant women are not eligible. Rapid HCV RNA screening is used to determine eligibili…

Pathophysiology and Treatment of Hepatic Encephalopathy.

Hepatic encephalopathy is a neurologic syndrome caused by hepatic insufficiency or portosystemic shunting, leading to the accumulation of ammonia and other neurotoxins. Clinical signs range from subtle behavioral changes to seizures and recumbency. Diagnosis relies on neurologic signs together with laboratory evidence of liver dysfunction, imaging, and sometimes biopsy. Importantly, hyperammonemia may occur without underlying hepatic dysfunction. Treatment focuses on addressing the underlying hepatic disease, reducing ammonia production and absorption, managing inflammation and cerebral edema,

Hepatic Fibrosis and Liver Cancer.

Primary liver cancer, or hepatocellular carcinoma (HCC), typically emerges in fibrotic livers where persistent inflammation and extracellular matrix (ECM) remodeling create a permissive niche for malignant transformation. Although cirrhosis remains a major risk factor, mounting data show that fibrosis itself, often in the context of metabolic dysfunction-associated steatotic liver disease (MASLD), can promote hepatocarcinogenesis even before cirrhosis develops. This review synthesizes mechanistic insights from hepatic stellate cell (HSC) biology and tumor immunology that position fibrosis as a

Hepatic Actinomycosis: A Diagnostic Challenge.

BACKGROUND: Primary hepatic actinomycosis (HA) is a rare chronic abscess-forming infection caused by an anaerobic gram-positive filamentous bacillus of the genus Actinomyces. It is a great imitator of liver tumors. CASE REPORT: We describe two patients who presented space-occupying lesions in the liver after hepaticojejunostomy. In the first case, hepaticojejunostomy was performed for recurrent choledocholithiasis. He later developed a left lobe abscess with hepaticolithiasis for which left hepatectomy with re-do hepaticojejunostomy was performed. His postoperative histology demonstrated acti

Future Therapies of Hepatic Encephalopathy.

Hepatic encephalopathy, either covert or overt, affects more than half of patients with cirrhosis and has lasting effects even after portal hypertension is corrected. Unfortunately, the current therapeutic options still result in high rates of relapse and progression, in part owing to cost barriers and side effects, leading to poor adherence. This review summarizes emerging treatment options, which could take advantage of alternative disease pathways to improve future care of those with hepatic encephalopathy.

Biomarkers of Hepatic Toxicity: An Overview.

BACKGROUND: Hepatotoxicity is the foremost issue for clinicians and the primary reason for pharmaceutical product recalls. A biomarker is a measurable and quantifiable attribute used to evaluate the efficacy of a treatment or to diagnose a disease. There are various biomarkers which are used for the detection of liver disease and the intent of liver damage. OBJECTIVE: This review aims to investigate the current state of hepatotoxicity biomarkers and their utility in clinical settings. Using hepatic biomarkers, the presence of liver injury, its severity, prognosis, causative agent, and type of

Hepatic sarcoid resembling lymphoma.

Sarcoidosis is an inflammatory disease characterised by non-caseating granulomas that can affect any organ, although lung involvement is the most common. It is rare to find sarcoidosis isolated to extrapulmonary organs. We describe a case of extrapulmonary sarcoidosis with involvement of the liver in a man in his late 40s. His initial clinical history and investigations were more consistent with a diagnosis of lymphoma until a liver biopsy was performed revealing non-caseating granulomas more suggestive of a diagnosis of sarcoidosis. This patient had a history of young-onset ischaemic heart di

Pharmacologic Management of Hepatic Encephalopathy.

Hepatic encephalopathy is a common cause of morbidity and mortality among patients with decompensated liver cirrhosis. In this article, we review the history, mechanism, and evidence for first-line pharmacologic therapies for hepatic encephalopathy including nonabsorbable disaccharides, antibiotics, and electrolyte management. We also review newer, second-line therapies including polyethylene glycol, albumin, branched-chain amino acids, probiotics and fecal microbiota transplant, zinc, and l-ornithine-l-aspartate.

Diagnosis and Management of Hepatic Hydrothorax.

Hepatic hydrothorax is a pleural effusion (typically ≥500 mL) that develops in patients with cirrhosis and/or portal hypertension in the absence of other causes. In most cases, hepatic hydrothorax is seen in patients with ascites. However, ascites is not always found at diagnosis and is not clinically detected in 20% of patients with hepatic hydrothorax. Some patients have no symptoms and incidental findings on radiologic examination lead to the diagnosis of the condition. In the majority of cases, the patients present with symptoms such as dyspnea at rest, cough, nausea, and pleuritic chest p

Atraumatic Hepatic Laceration with Hemoperitoneum.

Introduction: A rare case of atraumatic liver laceration associated with hemoperitoneum is presented in a patient with amyloidosis who came to the hospital for abdominal pain. Case Presentation: The imaging findings reveal significant hepatomegaly with finely heterogeneous hepatic density and subcapsular hypo-dense streaks in segments VI and VII, likely representing lesions. Post-contrast enhancement shows a punctiform contrast medium extravasation within the subhepatic fluid collection, visible from the arterial phase and intensifying in subsequent study phases. Discussion: These imaging find

A Rare Case of Hepatic Gastrinoma.

Zollinger-Ellison syndrome (ZES) is a rare condition caused by gastrin-secreting neuroendocrine tumors known as gastrinomas. We present a case of hepatic ZES presenting as upper gastrointestinal (GI) bleeding in a 70-year-old female. Initial evaluation revealed the patient to be in severe sepsis with septic shock secondary to a urinary tract infection, and her hospitalization was complicated by hematemesis and melena in the setting of multiple duodenal ulcers. Subsequent investigations, including elevated gastrin levels and a somatostatin receptor scan, confirmed the diagnosis of gastrinoma.
