Factors associated with elevated serum gentamicin levels in hip and knee joints treated with continuous local antibiotic perfusion: A retrospective cohort study.

BACKGROUND: Continuous local antibiotic perfusion (CLAP) delivers high-concentration antibiotics locally to achieve the minimal biofilm eradication concentration. Although CLAP has shown efficacy in orthopaedic infections, data on serum gentamicin levels and associated complications remain limited. This study aimed to evaluate serum gentamicin elevation and related adverse events during CLAP, and to identify contributing factors. METHODS: A retrospective review was conducted of 59 patients who underwent CLAP for hip or knee joints between 2019 and 2024, with perioperative serum gentamicin lev

Reducing unwarranted variation in medication reviews following a fall: a closed-loop audit in primary care.

BACKGROUND: Falls in adults ≥65 are common, costly and preventable. Around one-third fall each year and 5-10% sustain serious injury; hip fractures carry 20-30% one-year mortality and falls cost the NHS ~£2.3bn annually. Polypharmacy and high-risk drugs (e.g. benzodiazepines, anticholinergics, antihypertensives) increase risk. NICE CG161/NG5 and NHS England guidance recommend timely, structured post-fall medication reviews, yet local data suggested unwarranted variation in documentation and follow-up. AIM: To increase guideline-concordant documentation of medication reviews within 4 weeks aft

A fully implantable intraspinal microstimulation device for early preclinical evaluation of feasibility, stability, and functionality.

Electrical stimulation of the spinal cord has shown promise in restoring standing and walking after paralysis, yet key challenges remain in achieving selective and stable movements over long durations of stimulation. Intraspinal microstimulation (ISMS) provides precise and sustained activation of motor circuits in animal models. Nevertheless, translation of ISMS to humans requires assessment of surgical feasibility, safety and long-term stability in an animal model with spine and spinal cord size and morphology similar to those in humans. Here, we demonstrate the development and implementation

Associations of Lipid and Adiposity Indices with Vascular Aging Stages in a Multicenter Chinese Population.

OBJECTIVE: Vascular aging is a significant factor in cardiovascular and cerebrovascular diseases and serves as a predictor of all-cause mortality. Although disturbances in lipid metabolism are known risk factors, their links with standardized stages of vascular aging have not been thoroughly assessed in the Chinese population. This multicenter study analyzed the distribution of vascular aging stages in the China Standardized Vascular Aging Management Centers (VMCs). It investigated the relationships between lipid-adiposity-related indices and the progression of vascular aging. METHODS: A tota

Osteoporosis: a missed link in fall prevention - a retrospective cohort study.

BACKGROUND: Falls and osteoporotic fractures are common in the geriatric population. Although falls increase the risk of osteoporotic fractures, many patients with recurrent falls are untreated for osteoporosis. In our study, we aimed to; (1) assess the incidence of fractures among patients who experience falls during inpatient care in a geriatric ward; (2) investigate the risk factors associated with fractures in this cohort; and (3) investigate whether patients on anti-osteoporotic treatment differed from and had better fall-related clinical outcomes compared to those without anti-osteoporot

Risk factors for avascular necrosis in Delbet type II paediatric femoral neck fractures: a case-control study.

BACKGROUND AND OBJECTIVES: Paediatric femoral neck fractures (PFNFs) are rare but carry a significant risk of avascular necrosis (AVN) of the femoral head. Delbet type II fractures are the most common variant, yet specific predictors of AVN within this subgroup remain unclear. This study aimed to identify risk factors for AVN in Delbet type II PFNFs and evaluate the functional outcomes following management with weight-bearing restriction and bisphosphonate therapy. METHODS: A retrospective case-control study conducted from June 2011 to May 2022 included a total of 15 Delbet type II fractures.

Medial-Pivot Total Knee Arthroplasty with Kinematic Alignment Produces Similar Clinical Outcomes in Valgus and Varus Deformities.

BACKGROUND: Mechanical alignment total knee arthroplasty (TKA) in valgus knees requires soft tissue releases and complex techniques that are unnecessary with kinematic alignment (KA). Few studies have evaluated the outcomes of KA in valgus knees, and fewer have studied KA with medial-pivot (MP) implants. This study compared the clinical outcomes of KA-TKA with MP implants (KA/MP-TKA) in patients with preoperative varus versus valgus alignment. We hypothesized that outcomes following KA/MP-TKA would be equivalent. METHODS: A prospective database was queried to identify patients who underwent p

Correlation of Kidney Injury Molecule-1 and Renal Lipotoxicity among Adult Obese Individuals with Asymptomatic Kidney Function.

BACKGROUND: Obesity is a chronic metabolic condition characterised by excessive fat deposition with deleterious effects on human health. Obesity can adversely affect renal function by causing defective lipid build-up and increased deposition of fat in the renal sinus. AIMS: The aim of this study was to establish the relationship between serum Kidney Injury Molecule-1 (KIM-1) concentration and renal lipotoxicity, using serum free fatty acid (FFA) level as its surrogate marker, among asymptomatic adult obese individuals. METHODS: A cross-sectional study of adult obese outpatients in the Univer

Spontaneous Onset of Postoperative Chilaiditi Syndrome: A Case Report and Its Potential Other Association?

Chilaiditi syndrome is characterized by the interposition of the colon between the liver and diaphragm, leading to various abdominal symptoms. Current literature explores the suspected etiology of this condition including diaphragmatic conditions such as phrenic injury or deficient peritoneal attachments that lead to laxity of suspensory ligaments or enlarged subdiaphragmatic space; however, there is considerable ambiguity in the contributing factors to the development of Chilaiditi syndrome due to its rarity. Furthermore, an association between Chilaiditi syndrome and psychiatric conditions h

Two types of insoles design to influence running biomechanics in opposite directions and individual responses.

INTRODUCTION: Global running patterns vary along a spectrum defined by the degree of body verticality. This continuum ranges from extension (upright extended postures) to flexion (forward-leaning positions characterized by flexion at the hips and knees). Understanding these patterns is crucial for effective injury rehabilitation. Recent research has identified inefficiencies in vertical load management, leading to the development of extension- or flexion-based exercises. Insoles, while not typically designed for comprehensive extension or flexion adjustments, can complement these exercises. Th

Thromboembolic safety and clinical benefits of tranexamic acid beyond hemostasis in post-COVID-19 patients undergoing major arthroplasty: A STROBE-compliant retrospective study.

The safety of tranexamic acid (TXA) in patients with recent coronavirus disease (COVID-19) infection undergoing major arthroplasty remains unclear. We aimed to evaluate whether TXA increases thromboembolic risk in post-COVID-19 patients undergoing major arthroplasty. Using the TriNetX database, we identified patients aged ≥50 years who underwent total knee or hip arthroplasty with documented COVID-19 within 3 months prior to surgery. Patients who received TXA on the day of surgery were compared with those who did not. The primary outcome was 6-month risk of venous thromboembolism (VTE), with s

Association between serum uric acid to creatinine ratio with metabolic profile in subjects with non-alcoholic fatty liver disease (NAFLD).

BACKGROUND: Non-alcoholic fatty liver disease (NAFLD) is a prevalent chronic liver condition strongly linked with metabolic syndrome components. The serum uric acid to creatinine (SUA/Cr) ratio has recently emerged as a potential biomarker of metabolic dysfunction. However, its clinical relevance in individuals with NAFLD remains underexplored. The objectives of the current research was to investigate the association between the SUA/Cr ratio and metabolic risk factors in adults diagnosed with NAFLD. METHODS: This cross-sectional study included 226 adults with ultrasonography-confirmed NAFLD (

Bikini versus Traditional Incision in DAA THA: Patient Satisfaction, Surgeon Observation, and LFCN Damage.

BACKGROUND: The bikini incision (BI) used in direct anterior approach total hip arthroplasty (DAA THA) theoretically improves scar aesthetics by utilizing the inguinal crease; however, current literature suggests its transverse orientation increases the risk of injury to the lateral femoral cutaneous nerve (LFCN). This study examines (1) patient and surgeon satisfaction with scar appearance and (2) differences in post-operative LFCN sensation between the vertical, traditional incision (TI) and the BI. QUESTIONS/PURPOSES: In this pilot study, the following questions were investigated: (1) Do B

Hip internal rotation restriction and heel raise mechanics as risk factors for Jones fractures in youth football players.

BACKGROUND: Jones fractures, stress fractures at the proximal metaphyseal-diaphyseal junction of the fifth metatarsal, are common in sports that involve repetitive jumping and turning, particularly football (i.e., soccer). The incidence among Japanese football players is higher than in Europe, with a high refracture rate, particularly in conservatively managed cases. Although surgical treatment is preferred for athletes, limited data exist on risk factors in youth players. This study aimed to investigate the prevalence and risk factors for Jones fractures among high-level Japanese high school
