Changes in calcitonin release during sympathetic nerve degeneration after superior cervical ganglionectomy of rats.

To assess the role of peripheral sympathetic nerves in the regulation of calcitonin release, rats subjected to superior cervical ganglionect … e significantly increased basal calcitonin levels and restored the depressed calcitonin response to hypercalcemia seen in SCGx rats. Treatment with the beta-adrenoceptor-blocker propranolol counteracted phenoxybenzamine activity but was unable to modify per se calcitonin release in SCGx or sham-operated rats. Basal Ca levels and their increase after intraperito …

A case of advanced breast cancer associated with hypocalcemia.

Although hypercalcemia is a well-known complication of malignant diseases, hypocalcemia seems to be a rather rare one. A 34-yr-old woman with advanced breast cancer who presented hypocalcemia is described. She had generalized multiple osteolytic bone metastases which were progressive in spite of chemo-endocrine and radiation therapy. She was admitted because of severe bone pain and dyspnea caused by bilateral pleural effusion. Laboratory examination on admission showed that the serum calcium was 9.6 mg/dl, serum total protein 5.9 g/dl, serum inorganic phosphorus 4.6 mg/dl, and serum alkaline p…

Cestrum diurnum intoxication in normal and hyperparathyroid pigs.

The effect of ingestion of dried leaves of Cestrum diurnum, a plant shown to contain a 1,25-dihydroxycholecalciferol-like principle, was tested in normal pigs fed 1.2% calcium and 1.0% phosphorus for 10 weeks from weaning and in hyperparathyroid pigs fed 0.8% calcium and 1.6% phosphorus for the same periods of time. Addition of 3% Cestrum diurnum leaf meal rapidly resulted in decreased feed consumption and weight gain, hypercalcemia and hypophosphatasemia. In normal pigs, plasma calcium rose to 16 mg/100 ml within one week and remained high for the 4 week experimental period. In hyperparathyro…

Hyperparathyroidism during pregnancy.

Hyperparathyroidism during pregnancy is clearly associated with an increased incidence of neonatal morbidity and mortality. Although it is i … the hypercalcemic hyperparathyroid pregnancy, transplacental passage of calcium leads to a profound hypercalcemia in the fetus. Since the fetal parathyroid glands are functionally responsive, parathyroid suppression is thought to occur in utero due to high calcium levels. This can lead to neonatal tetany or perhaps permanent neonatal hypoparathyroidism. When a …

Effects of oral vitamin D supplementation on linear growth and other health outcomes among children under five years of age.

BACKGROUND: Vitamin D is a secosteroid hormone that is important for its role in calcium homeostasis to maintain skeletal health. Linear growth faltering and stunting remain pervasive indicators of poor nutrition status among infants and children under five years of age around the world, and low vitamin D status has been linked to poor growth. However, existing evidence on the effects of vitamin D supplementation on linear growth and other health outcomes among infants and children under five years of age has not been systematically reviewed. OBJECTIVES: To assess effects of oral vitamin D su

The calcium-sensing receptor in physiology and in calcitropic and noncalcitropic diseases.

The Ca2+-sensing receptor (CaSR) is a dimeric family C G protein-coupled receptor that is expressed in calcitropic tissues such as the parathyroid glands and the kidneys and signals via G proteins and β-arrestin. The CaSR has a pivotal role in bone and mineral metabolism, as it regulates parathyroid hormone secretion, urinary Ca2+ excretion, skeletal development and lactation. The importance of the CaSR for these calcitropic processes is highlighted by loss-of-function and gain-of-function CaSR mutations that cause familial hypocalciuric hypercalcaemia and autosomal dominant hypocalcaemia, res

Eating disorders should be considered in the differential diagnosis of patients presenting with acute kidney injury and electrolyte derangement.

We present a case of a 40-year-old woman with a history of ongoing anorexia nervosa and bulimia nervosa who has required multiple admissions to hospital for management of acute kidney injury (AKI) and electrolyte derangement. This case is of interest as recent studies have highlighted the significant prevalence of disordered eating and the major public health implications this may have. We discuss the unusual finding of hypercalcaemia in this case and address the investigation and management of AKI and electrolyte disturbance in a patient with anorexia and bulimia.

Cytochrome P450-mediated metabolism of vitamin D.

The vitamin D signal transduction system involves a series of cytochrome P450-containing sterol hydroxylases to generate and degrade the active hormone, 1α,25-dihydroxyvitamin D3, which serves as a ligand for the vitamin D receptor-mediated transcriptional gene expression described in companion articles in this review series. This review updates our current knowledge of the specific anabolic cytochrome P450s involved in 25- and 1α-hydroxylation, as well as the catabolic cytochrome P450 involved in 24- and 23-hydroxylation steps, which are believed to initiate inactivation of the vitamin D mole

Characteristics, management and outcome of primary hyperparathyroidism in South Africa: a single-centre experience.

INTRODUCTION: Primary hyperparathyroidism (PHPT) is a common endocrine disorder characterised by hypercalcaemia and elevated parathyroid hormone (PTH) levels. However, it remains a relatively underdiagnosed disease in the developing world primarily due to a lack of routine blood chemistry screening. The aim of this analysis was to evaluate the characteristics, management and outcome of patients with PHPT at a tertiary referral clinic in South Africa. METHODS: A retrospective analysis was undertaken on all patients with a diagnosis of PHPT attending the endocrinology clinic at a tertiary refer

Utility of the urine calcium-to-creatinine ratio to diagnose primary hyperparathyroidism in asymptomatic hypercalcaemic patients with vitamin D deficiency.

BACKGROUND: Primary hyperparathyroidism (PHP) is the most common cause of hypercalcaemia, and often requires surgical treatment. Familial hypocalciuric hypercalcaemia (FHH) has similar biochemical features to PHP, but requires no treatment. The most common biochemical method used to distinguish between PHP and FHH is the urine calcium-to-creatinine ratio (UCCR). Vitamin D deficiency may alter the renal excretion of calcium, but it is unclear how vitamin D deficiency affects the diagnostic performance of UCCR. AIM: To examine the reliability of UCCR to detect PHP in patients presenting with as

Morphological changes in the parathyroid gland of rats with humoral hypercalcaemia of malignancy.

Morphological changes in parathyroid chief cells were investigated in rats with humoral hypercalcaemia of malignancy (HHM), induced by intraperitoneal inoculation with a rat pulmonary adenocarcinoma cell line, IP-B12. Inoculation with IP-B12 cells resulted in the production of parathyroid hormone-related protein (PTHrP); thus, IP-B12-bearing rats developed hypercalcaemia, with elevated plasma PTHrP concentrations and decreased serum PTH, at 5 to 7 weeks after the inoculation. Morphometrical analysis revealed an increase in the cytoplasmic area of chief cells in IP-B12-bearing rats, as compared

[PTH assay in the first postoperative day after thyroidectomy early predictor postoperative hypocalcemia?].

BACKGROUND: The purpose of our study is to verify if PTH assay on the first postoperative day is a reliable early predictor of the onset of hypocalcemia. METHODS: Between October 1999 and May 2000, a prospective trial involved 162 patients who underwent total or near total thyroidectomy at our institute. On the basis of PTH assay on first day we divided the patients in three groups: group A 28 patients with PTH < 10 pg/ml; group B 34 patients with PTH between 10 and 16 pg/ml; group C 100 patients with PTH > 16 pg/ml. RESULTS: In group A: 22 of 28 patients (78.5%) developed postoperative hypo

Failure of high doses of calcitriol and hypercalcaemia to induce apoptosis in hyperplastic parathyroid glands of azotaemic rats.

BACKGROUND: Whether calcitriol administration, which is used to treat secondary hyperparathyroidism in dialysis patients, induces regression of parathyroid-gland hyperplasia remains a subject of interest and debate. If regression of the parathyroid gland were to occur, the presumed mechanism would be apoptosis. However, information on whether high doses of calcitriol can induce apoptosis of parathyroid cells in hyperplastic parathyroid glands is lacking. Consequently, high doses of calcitriol were given to azotaemic rats and the parathyroid glands were evaluated for apoptosis. METHODS: Rats w

Evaluation of parathyroid adenomas by intravenous digital subtraction angiography.

Intravenous digital subtraction angiography (DSA) successfully localized three previously suspected parathyroid adenomas in the lower part of the neck and one in the mediastinum. Our experience suggests that this method may have widespread application for preoperative evaluation of parathyroid adenomas, particularly after unsuccessful surgical exploration or when recurrent or ectopically located adenomas are likely. In three other cases abnormal vascular blushes in the neck on intravenous DSA were related to thyroid disease. Close correlation with the clinical and laboratory findings is necess
