Umbilical Cord pH Levels and Neonatal Morbidity and Mortality.

IMPORTANCE: Umbilical cord pH (UC-pH) level is an important objective indicator of intrapartum fetal hypoxia and is used to predict neonatal morbidity and mortality. A UC-pH value of less than 7.00 is often defined as a threshold for severe acidosis, but existing evidence is divergent and largely based on UC-pH measurements from selected populations; consequently, the results are challenging to interpret. OBJECTIVE: To investigate the association between UC-pH levels and the risk of adverse neonatal outcomes in a national setting with universal UC-pH measurement. DESIGN, SETTING, AND PARTICI

Prevalence and associated factors of birth defects among newborns in sub-Saharan African countries: a systematic review and meta-analysis.

INTRODUCTION: Birth defects are the most serious causes of infant mortality and disability in sub-Saharan African countries with variable magnitude. Hence, this study was aimed to determine the pooled prevalence of birth defects and its associated risk factors among newborn infants in sub-Saharan African countries. METHODS: A total of 43 eligible studies were identified through literature search from Medline (PubMed), EMBASE, HINARI, Google scholar, Science Direct, Cochrane Library and other sources. Extracted data were analyzed using STATA 15.0 statistical software. A random effect meta-anal…

Access to and use of health services as factors associated with neonatal mortality in the North, Northeast, and Vale do Jequitinhonha regions, Brazil.

OBJECTIVE: To analyze the factors associated with neonatal mortality related to health services accessibility and use. METHODS: Case-control study of live births in 2008 in small- and medium-sized municipalities in the North, Northeast, and Vale do Jequitinhonha regions, Brazil. A probabilistic sample stratified by region, population size, and information adequacy was generated for the choice of municipalities. Of these, all municipalities with 20,000 inhabitants or less were included in the study (36 municipalities), whereas the remainder were selected according to the probability method pro

Does Resuscitation Training Reduce Neonatal Deaths in Low-Resource Communities? A Systematic Review of the Literature.

Every year, nearly 1 million babies succumb to birth asphyxia (BA) within the Asia-Pacific region. The present study sought to determine whether educational interventions containing some element of resuscitation training would decrease the relative risk (RR) of neonatal mortality attributable to BA in low-resource communities. We systematically reviewed 3 electronic databases and identified 14 relevant reports. For community deliveries, providing traditional birth attendants (TBAs) with neonatal resuscitation training modestly reduced the RR in 3 of 4 studies. For institutional deliveries, tra

Can a single lactate value predict adverse outcome in critically ill newborn?

OBJECTIVES: The aim of this study was to investigate the role of the lactic acidosis, as an early predictor of significant consequences and/or a fatal outcome in term neonates after a perinatal asphyxia. BACKGROUND: Severe perinatal asphyxia can generate multiple organ dysfunction and neonatal mortality. METHODS: In routine clinical practice, after an admission to the Intensive Care Unit, lactate concentration was determined in capillary blood samples during the first one to six hours after birth in 55 term newborns with the post-asphyxial hypoxic-ischemic encephalopathy. The control group c

Clinical research informing neonatal care at the University Hospital of the West Indies: 50 years experience.

Neonatology has rapidly grown over the past few years to its present status of a highly technologically driven specialty. Centres in resource restricted countries tend to adopt management guidelines from the developed world and integrate them into local practice. Although international neonatal practices have influenced neonatal care at the University Hospital of the West Indies (UHWI), over the past fifty years there has been local clinical research done at the hospital that has informed local practice. Causes of neonatal mortality have been documented and this has informed neonatal resuscita

Sudden, unexpected and unexplained early neonatal deaths in the North of England.

Early neonatal sudden unexpected unexplained deaths (for which we use the term ENSUD) have not been subject to detailed study. The authors investigated the incidence from 1983 to 2007 in the population of the North East of England and North Cumbria. The authors found 30 cases of unexplained ENSUD, giving an overall rate of 0.35/10 000 live births, with no significant change in incidence over this time, and they identified a further 19 deaths of abandoned babies. The authors conclude that unexplained ENSUD is even more rare than has been appreciated and its incidence has not been altered by the

Prevalence and underlying etiologies of neonatal hypoglycemia.

This study aims at determining the prevalence of neonatal hypoglycemia and its underlying causes. In this prospective study 14168 newborns delivered in Tabriz Alzahra Hospital during 2 years were evaluated in regard to blood glucose level at first 24 h of life. Glucose oxidase method with 4-aminophenazone with a Greiner G-300 was the used method for determining the blood glucose level. Cases with blood glucose < 50 mg dL(-1) were considered as hypoglycemic newborns. Underlying causes of this condition, as well as the short-term mortality rate were determined. Prevalence of neonatal hypoglycemi

[Neonate--newborn condition and prematurity with breech delivery].

INTRODUCTION: Before birth, most of foetuses located in mother's uterus have head presentation down towards the breech exit. Preterm delivery presents the delivery before the end of the 37th gestational week. Etiology of preterm breech deliveries is the same as of the ones at term, such as uterine malformations and myomas, polyhydramnion, foetal abnormalities, high parity, short umbical cord, placenta previa. The complications include passing of the body through insufficiently dilated cervical channel and "capturing " of the head, umbilical cord prolapse, intrapartal hipoxy, discoordinated del

Birthweight discordance in twins and the risk of being heavier for respiratory distress syndrome.

Twin pregnancies are becoming common as a result of increased assisted reproduction. Studies have shown that the smaller twin of a pair is at greater risk of morbidity and mortality. Our aim was to determine if there is a relation between birth-weight discordancy and neonatal morbidity and mortality and to test the occurrence of respiratory distress syndrome (RDS) in discordant twins. For 5 years all twins admitted to our intensive care unit were included in the study. Discordance was calculated as the difference between twins' weights divided by the heavier weight. Diagnosis of RDS was made b

[Perinatal mortality due to congenital syphilis: a quality-of-care indicator for women's and children's healthcare].

Syphilis is a persistent cause of perinatal mortality in Rio de Janeiro, Brazil, where this study was performed using data from the mortality data system and investigational reports for fetal and neonatal deaths, mandatory in municipal maternity hospitals. From 1996 to 1998, 13.1% of fetal deaths and 6.5% of neonatal deaths in municipal maternity hospitals were due to congenital syphilis. From 1999 to 2002, the proportions were 16.2% and 7.9%, respectively. For the city of Rio de Janeiro as a whole from 1999 and 2002, the proportions were 5.4% of fetal deaths and 2.2% of neonatal deaths. The p

Sudden and unexpected deaths after the administration of hexavalent vaccines (diphtheria, tetanus, pertussis, poliomyelitis, hepatitis B, Haemophilius influenzae type b): is there a signal?

UNLABELLED: Deaths in temporal association with vaccination of hexavalent vaccines have been recently reported. The objective of this paper is to assess whether these temporal associations can be attributed to chance. Standardised mortality ratios (SMR) for deaths within 1 to 28 days after administration of either of the two hexavalent vaccines in the 1st and 2nd year of life were determined using the respective annual rates for sudden unexpected deaths (SUDs) from the national vital statistics. The distribution of SUD cases and the vaccination uptake by month were estimated from surveys and s

The risk of neonatal death and respiratory distress syndrome in relation to birth weight of preterm infants.

The aim of this study was to determine the risk of respiratory distress syndrome (RDS) and neonatal death (NND) in relation to birth weight in preterm neonates. A group of 255 singleton preterm neonates born at 22-36 weeks were examined. The mean birth weight for each gestational week was estimated from a fitted curve. Each birth weight was recalculated as a multiple of the mean. This approach allowed description of a range of birth weight for the whole population of preterm infants. As expected, the incidence of neonatal death and respiratory distress syndrome was higher among the less mature

The impact of prenatal care on fetal and neonatal death rates for uninsured patients: a "natural experiment" in West Virginia.

A three-county program in southern West Virginia was developed by an obstetric practice to deliver prenatal care to a population of uninsured patients. Between January 1984 and December 1986, 1331 (29.4%) of 4534 patients were delivered at a level 2 hospital after prenatal care within the clinic program. The hospital-wide fetal death ratio declined from 11.8 to 7.2 per 1000 live births during the years of clinic operation, a statistically significant reduction (P = .02). Uninsured patients experienced a statistically significant reduction in fetal death ratio during the program, from 35.4 to 7

HIV-1 infection and perinatal mortality in Zimbabwe.

As part of a survey of the causes of perinatal mortality at Mpilo Maternity Hospital, 220 neonatal deaths and the mothers of 221 stillbirths were tested for HIV-1 antibodies. The HIV positive rate in neonatal deaths was 23.6% (95% confidence interval (CI) 18.0 to 29.2%), significantly higher than 15.4% (95% CI 10.6 to 20.1%) in stillbirths. Perinatal deaths from congenital malformations, birth asphyxia, pregnancy induced hypertension, placental abruption, and oFther non-infectious causes had similar low HIV positive rates averaging 8.1% (95% CI 3.9 to 12.3%). Deaths from septicaemia had a sign

When do perinatal deaths in multiple pregnancies occur?

Perinatal mortality for multiple pregnancy remains at least 5 times the rate for singleton births. The major causes are neonatal deaths due to gross immaturity before 30 weeks' gestation, and stillbirths due to intrauterine growth retardation at all gestations, but especially after 32 weeks. Sixty four per cent of perinatal losses before 30 weeks' gestation occur before 26 weeks, highlighting the need to commence prophylactic measures earlier than usually recommended. The perinatal mortality in infants in multiple births weighing more than 2,500g is the same as that of singletons, but is 10 ti

[Late primiparity: what has changed in the last 25 years?].

The case reports of late primiparae greater than 35 years of age were analysed in respect of the change of anamnestic risks, of the course of pregnancy, birth and puerperium, as well as of infant data. The incidence of late primiparae rose in our clinic from A 1.8% (n = 292, infants n = 294), via B 2.6% (n = 210, infants n = 213) to C 3.7% (n = 222, infants n = 225.) Previous history of internistic diseases was rare during all periods. In period C, the anamnestic risk "Status after sterility treatment" attains increasing importance (17%). The incidence of pre-eclamptic toxaemia (toxaemia assoc

Confidential inquiry into perinatal deaths in the Mersey region. Mersey Region Working Party on Perinatal Mortality.

In a confidential inquiry into perinatal deaths in the Mersey region in 1979 obstetric and paediatric assessors reviewed the circumstances surrounding each case in order to identify any avoidable factors contributing to the death. The area covered by the inquiry was determined by the mother's place of residence and related to 58% of total births (liver births and stillbirths) and 61% of perinatal deaths in the region. Avoidable factors were present in 182 (59%) of the 309 perinatal deaths. Low-birthweight infants (less than or equal to 2500 g) with no maternal complications comprised the large
