The discriminatory cost of ICD-10-CM transition between clinical specialties: metrics, case study, and mitigating tools.

OBJECTIVE: Applying the science of networks to quantify the discriminatory impact of the ICD-9-CM to ICD-10-CM transition between clinical specialties. MATERIALS AND METHODS: Datasets were the Center for Medicaid and Medicare Services ICD-9-CM to ICD-10-CM mapping files, general equivalence mappings, and statewide Medicaid emergency department billing. Diagnoses were represented as nodes and their mappings as directional relationships. The complex network was synthesized as an aggregate of simpler motifs and tabulation per clinical specialty. RESULTS: We identified five mapping motif categor

Automating annotation of information-giving for analysis of clinical conversation.

OBJECTIVE: Coding of clinical communication for fine-grained features such as speech acts has produced a substantial literature. However, annotation by humans is laborious and expensive, limiting application of these methods. We aimed to show that through machine learning, computers could code certain categories of speech acts with sufficient reliability to make useful distinctions among clinical encounters. MATERIALS AND METHODS: The data were transcripts of 415 routine outpatient visits of HIV patients which had previously been coded for speech acts using the Generalized Medical Interaction

Vaccine adverse event text mining system for extracting features from vaccine safety reports.

OBJECTIVE: To develop and evaluate a text mining system for extracting key clinical features from vaccine adverse event reporting system (VAERS) narratives to aid in the automated review of adverse event reports. DESIGN: Based upon clinical significance to VAERS reviewing physicians, we defined the primary (diagnosis and cause of death) and secondary features (eg, symptoms) for extraction. We built a novel vaccine adverse event text mining (VaeTM) system based on a semantic text mining strategy. The performance of VaeTM was evaluated using a total of 300 VAERS reports in three sequential eval

An i2b2-based, generalizable, open source, self-scaling chronic disease registry.

OBJECTIVE: Registries are a well-established mechanism for obtaining high quality, disease-specific data, but are often highly project-specific in their design, implementation, and policies for data use. In contrast to the conventional model of centralized data contribution, warehousing, and control, we design a self-scaling registry technology for collaborative data sharing, based upon the widely adopted Integrating Biology & the Bedside (i2b2) data warehousing framework and the Shared Health Research Information Network (SHRINE) peer-to-peer networking software. MATERIALS AND METHODS: Focus

Structural models used in real-time biosurveillance outbreak detection and outbreak curve isolation from noisy background morbidity levels.

OBJECTIVE: We discuss the use of structural models for the analysis of biosurveillance related data. METHODS AND RESULTS: Using a combination of real and simulated data, we have constructed a data set that represents a plausible time series resulting from surveillance of a large scale bioterrorist anthrax attack in Miami. We discuss the performance of anomaly detection with structural models for these data using receiver operating characteristic (ROC) and activity monitoring operating characteristic (AMOC) analysis. In addition, we show that these techniques provide a method for predicting th

A Dimensional Bus model for integrating clinical and research data.

OBJECTIVES: Many clinical research data integration platforms rely on the Entity-Attribute-Value model because of its flexibility, even though it presents problems in query formulation and execution time. The authors sought more balance in these traits. MATERIALS AND METHODS: Borrowing concepts from Entity-Attribute-Value and from enterprise data warehousing, the authors designed an alternative called the Dimensional Bus model and used it to integrate electronic medical record, sponsored study, and biorepository data. Each type of observational collection has its own table, and the structure

The application of naive Bayes model averaging to predict Alzheimer's disease from genome-wide data.

OBJECTIVE: Predicting patient outcomes from genome-wide measurements holds significant promise for improving clinical care. The large number of measurements (eg, single nucleotide polymorphisms (SNPs)), however, makes this task computationally challenging. This paper evaluates the performance of an algorithm that predicts patient outcomes from genome-wide data by efficiently model averaging over an exponential number of naive Bayes (NB) models. DESIGN: This model-averaged naive Bayes (MANB) method was applied to predict late onset Alzheimer's disease in 1411 individuals who each had 312,318 S

Evaluation of computer-generated reminders to improve CD4 laboratory monitoring in sub-Saharan Africa: a prospective comparative study.

OBJECTIVE: Little evidence exists on effective interventions to integrate HIV-care guidelines into practices within developing countries. This study tested the hypothesis that clinical summaries with computer-generated reminders could improve clinicians' compliance with CD4 testing guidelines in the resource-limited setting of sub-Saharan Africa. DESIGN: A prospective comparative study of two randomly selected outpatient adult HIV clinics in western Kenya. Printed summaries with reminders for overdue CD4 tests were made available to clinicians in the intervention clinic but not in the control

Adjusting outbreak detection algorithms for surveillance during epidemic and non-epidemic periods.

Many aberration detection algorithms are used in infectious disease surveillance systems to assist in the early detection of potential outbreaks. In this study, we explored a novel approach to adjusting aberration detection algorithms to account for the impact of seasonality inherent in some surveillance data. By using surveillance data for hand-foot-and-mouth disease in Shandong province, China, we evaluated the use of seasonally-adjusted alerting thresholds with three aberration detection methods (C1, C2, and C3). We found that the optimal thresholds of C1, C2, and C3 varied between the epid

Using electronic health record alerts to provide public health situational awareness to clinicians.

Alerting providers to public health situations requires timeliness and context-relevance, both lacking in current systems. Incorporating decision support tools into electronic health records may provide a way to deploy public health alerts to clinicians at the point of care. A timely process for responding to Health Alert Network messages sent by the New York City Department of Health and Mental Hygiene was developed by a network of community health centers. Alerts with order sets and recommended actions were created to notify primary care providers of local disease outbreaks. The process, eff

A Bayesian spatio-temporal method for disease outbreak detection.

A system that monitors a region for a disease outbreak is called a disease outbreak surveillance system. A spatial surveillance system searches for patterns of disease outbreak in spatial subregions of the monitored region. A temporal surveillance system looks for emerging patterns of outbreak disease by analyzing how patterns have changed during recent periods of time. If a non-spatial, non-temporal system could be converted to a spatio-temporal one, the performance of the system might be improved in terms of early detection, accuracy, and reliability. A Bayesian network framework is proposed

A heuristic indication and warning staging model for detection and assessment of biological events.

OBJECTIVE: This paper presents a model designed to enable rapid detection and assessment of biological threats that may require swift intervention by the international public health community. DESIGN: We utilized Strauss' grounded theory to develop an expanded model of social disruption due to biological events based on retrospective and prospective case studies. We then applied this model to the temporal domain and propose a heuristic staging model, the Wilson-Collmann Scale for assessing biological event evolution. MEASUREMENTS: We retrospectively and manually examined hard copy archival l

A pragmatic approach to implementing best practices for clinical decision support systems in computerized provider order entry systems.

Incorporation of clinical decision support (CDS) capabilities is required to realize the greatest benefits from computerized provider order entry (CPOE) systems. Discussions at a conference on CDS in CPOE held in San Francisco, California, June 21-22, 2005 produced several papers in this issue of JAMIA. The first paper reviews CDS for electronic prescribing within CPOE systems; (1) the second describes current controversies regarding creation, maintenance, and uses of CPOE order sets for CDS; (2) and the third presents issues related to certification as a potential means of validating CPOE sys
