Pain Point Elimination Bringing Better Longitudinal Engagement and Satisfaction project: qualitative mixed-methods study investigating a pragmatic intervention on physician well-being.

BACKGROUND: Clinician well-being is essential for healthcare sustainability. Burnout and intention to leave remain prevalent in anaesthesiology, yet scalable, evidence-based systems-level interventions are limited. This study aimed to evaluate the impact of a pragmatic, clinician-driven departmental intervention designed to improve daily work experience. We hypothesised that this intervention would reduce burnout and improve related measurements of clinician well-being. METHODS: This mixed-methods study assessed the Pain Point Elimination Bringing Better Longitudinal Engagement and Satisfacti

Whose voice counts? Family caregivers' roles in shared decision making for chronic illness in urban South Korea: A qualitative study.

BACKGROUND: Shared decision making (SDM) is fundamental to patient-centered care, yet dominant frameworks have been developed within dyadic patient-provider models grounded in Western individualistic assumptions. Prior research from East Asian, Sub-Saharan African, and Latin American contexts has documented that medical decision making is frequently family-centered and that caregivers function as recognized actors in clinical encounters. Building on this evidence base, this study examines how caregiver participation is enacted and constrained within the Korean chronic care context, contributin

Supporting a Student With School Absenteeism Through to Employment: A Case Report of Long-Term Home-Based Occupational Therapy for Emotion-Related School Attendance Challenges.

In Japan, rising school absenteeism underscores the need for effective support systems. Students with neurodevelopmental disorders, such as autism spectrum disorder and attention-deficit/hyperactivity disorder, are highly vulnerable to prolonged absenteeism due to difficulties in interpersonal relationships and adaptation to their environment. However, existing literature primarily focuses on adaptation up to school reintegration or progression to higher education, with few studies examining the long-term transition from high school graduation to employment. This report describes the case of a…

Facilitators and barriers to physical activity among older adults in Karachi - a qualitative exploratory study.

BACKGROUND: The global older adult population is expanding, projected to reach two billion by 2050. Pakistan, a low- and middle-income country (LMIC), is home to 7% of its older adult population (above 60). Physical inactivity is prevalent in this group, linked to chronic diseases and mental health disorders. However, little is known about its determinants. This study explores physical activity factors (facilitators and barriers) at individual, interpersonal, community, and policy levels among older adults in Karachi. METHODS: A qualitative exploratory study was conducted in two Karachi distri

Understanding factors that impact patient access and engagement with biomedical and traditional care for hip fractures in The Gambia: An ethnographic study using a social ecological model.

Across Africa, the average age of the population is increasing, leading to a rise in health challenges such as hip fractures. Surgical fixation is the standard of care for hip fracture in older adults. In The Gambia, traditional bone setters (TBS) remain an important source of fracture care, though concerns exist regarding complications. In resource-constrained settings, system constraints may also result in post-operative complications. Currently treatment-seeking decisions are poorly understood. We aimed to map hip fracture care pathways to understand factors affecting engagement with TBS an

Healthcare conflict management in resource-constrained settings: evidence from two hospitals in Ghana.

BACKGROUND: Conflict is a recurring feature of healthcare delivery, especially in hospitals that operate with limited staff, inadequate supplies, and heavy workloads. Although conflict among healthcare workers in Ghana has been documented, little research has explored how it emerges within the everyday routines of resource-constrained hospitals or how staff interpret and respond to it. This study examined the causes, effects, and management of institutional conflict in two public hospitals in the Effutu Municipality, using Organizational Information Theory and the Institutional Logics Perspect

"I always felt like I wasn't supposed to be there". An international qualitative study of fathers' engagement in family healthcare during transition to fatherhood.

OBJECTIVE: Engagement of fathers in family health services confers benefits for the health and wellbeing of the whole family. The childbirth continuum is traditionally considered a feminine event, however, commensurate with the changing paradigm of gender equity in family healthcare worldwide, the role of fathers is in transformation. The aim of the study is to explore father's perceptions and experiences of healthcare engagement during pregnancy and early infant care. DESIGN: Qualitative free-text questions were embedded in a large multi-country, cross-sectional survey, to explored fathers'

Assessing Structural Racism and Discrimination Along the Pre-exposure Prophylaxis Continuum: A Systematic Review.

Structural racism and discrimination (SRD) is deeply embedded across U.S. healthcare institutions, but its impact on health outcomes is challenging to assess. The purpose of this systematic literature review is to understand the impact of SRD on pre-exposure prophylaxis (PrEP) care continuum outcomes across U.S. populations who could benefit from HIV prevention. Guided by PRISMA guidelines, we conducted a systematic review of the published literature up to September 2023 using PubMed and PsycInfo and included peer-reviewed articles meeting inclusion criteria. At least two authors independently

Investigating the Efficacy and Experiences With Narrative Exposure Therapy in Severe Mentally Ill Patients With Comorbid Post-traumatic Stress Disorder Receiving Flexible Assertive Community Treatment: A Mixed Methods Study.

BACKGROUND: Patients with severe mental illness with repeated interpersonal trauma and post-traumatic stress disorder (PTSD) have a negative illness progression. Traumas are often not treated because of their vulnerability. Narrative exposure therapy (NET) is an effective trauma therapy. It is unknown whether NET is effective and tolerable in these patients receiving community mental healthcare. OBJECTIVES: The objectives of this study are (1) to gain insights into patients' experiences before, during, and after NET concerning changes in PTSD, dissociative and severe mental ill symptoms, care

Neural activation during anticipated peer evaluation and laboratory meal intake in overweight girls with and without loss of control eating.

The interpersonal model of loss of control (LOC) eating proposes that socially distressing situations lead to anxious states that trigger excessive food consumption. Self-reports support these links, but the neurobiological underpinnings of these relationships remain unclear. We therefore examined brain regions associated with anxiety in relation to LOC eating and energy intake in the laboratory. Twenty-two overweight and obese (BMIz: 1.9±0.4) adolescent (15.8±1.6y) girls with LOC eating (LOC+, n=10) and without LOC eating (LOC-, n=12) underwent functional magnetic resonance imaging (fMRI) dur

Spontaneous social behaviors discriminate behavioral dementias from psychiatric disorders and other dementias.

OBJECTIVE: Changes in social behavior are often the first symptoms of neurodegenerative disease. Patients with frontotemporal lobar degeneration (FTLD) often go undiagnosed, or are misclassified as psychiatric patients, because in the absence of cognitive deficits, nonexperts fail to recognize these social changes as dementia symptoms. The object of this study was to improve screening for behavioral dementias in primary care and mental health settings by quantifying spontaneous social behaviors specific to FTLD. METHOD: In a university hospital dementia clinic, examiners blind to subject diag

A Single, Reproducible Dimension Organizes Associations Between the Early Environment and the Developing Brain.

Childhood adversity has enduring consequences for health and development across the lifespan, yet the organizing structure linking diverse environmental experiences to the developing brain remains poorly understood. We jointly modeled 104 measures of children's environments, spanning interpersonal, household, and neighborhood contexts, with 7 neuroimaging modalities of brain structure and function in 5,976 youth. Despite this breadth, only a single adversity-brain dimension emerged and replicated in held-out data. This dimension was defined primarily by socioeconomic and neighborhood deprivati

Why Is It Still So Difficult to Acknowledge the Reality of Borderline Personality Disorder?

Borderline personality disorder (BPD) has emerged as a major clinical and societal challenge since its mid-twentieth-century conceptualization. This paper addresses four persistent obstacles to effective care: diagnostic complexity, interpersonal dysfunction and risk behaviors, high societal burden, and pervasive stigma. We argue that despite recent critiques, many of which propose replacing the diagnosis with trauma-related formulations, BPD remains a valid, coherent, and clinically indispensable construct. Misdiagnosis contributes to iatrogenic harm, while accurate diagnosis enables targeted

Communication and Connection Fostered Care Engagement During the COVID-19 Pandemic: A Qualitative Study of People Living With HIV in New York City.

The COVID-19 pandemic disrupted health care systems and resulted in some delays of care, challenging health care engagement among people living with HIV (PLWH). Using in-depth interviews, we explored the experiences of PLWH during the pandemic and identified barriers to and facilitators of care engagement. We recruited 40 participants from a managed-care Medicaid plan in New York City and conducted phone interviews during fall 2023. Thematic analysis identified 5 themes related to care engagement during the pandemic: (a) health care system strains led to some delays in care; (b) patient-provid

"You're an inmate just deal with it": experiences of formerly incarcerated individuals in New York City during the COVID-19 pandemic.

The onset of the COVID-19 pandemic posed uniquely dire challenges in New York City's jails. Rates of infection and fatality well exceeded those of the general population as the virus spread through congregate residences, heightening the city's status as an early epicenter. Using an inductive, grounded theory approach, we conduct 12 in-depth interviews with persons incarcerated in these jails early in the pandemic to elicit their perspectives on the carceral system's ability to deliver healthcare services and a safe residence. Participants describe concerns of overcrowding, insufficient protect

Bridging attachment theory and interpersonal acceptance-rejection theory in the strange situation procedure in a low-risk sample in Egypt.

This study is the first to explore the relation between children's attachment classifications, assessed by Mary Ainsworth's Strange Situation Procedure (SSP), and mothers' acceptance-rejection behaviors from the lens of Interpersonal Acceptance-Rejection Theory (IPARTheory). As a pilot study, the sample consisted of 23 Egyptian mother-child dyads. The mean age of children in the SSP was 18.6 months (SD = 3.10). Mothers' acceptance-rejection behaviors were explored through the Parental Acceptance-Rejection/Control Questionnaire (PARQ/Control). The pilot study's findings revealed that maternal a
