Duodenal mucosa-associated lymphoid tissue (MALT) lymphoma is very rare, and little is known about its clinical course or association with Helicobacter pylori infection. This report describes the case of a 76-year-old man with a polypoid mass in the duodenal bulb, diagnosed as low-grade MALT lymphoma. H. pylori infection in the duodenal mucosa was confirmed by histology with silver stain. This report describes the case of a patient in whom, after an unsuccessful attempt through the subclavian vein, a permanent pacing lead was inserted through the femoral vein and a left inferior vena cava with azygos continuation. The procedure was followed 4 months later by a pulmonary embolism complicating a right femoroiliac thrombosis. The patient was successfully treated by a percutaneous lead extraction procedure combined with an inferior vena caval surgical interruption.

Recent reports indicate that many older Americans have inadequate vitamin D sources and little to no seasonal increase in their body stores. The attendant secondary hyperparathyroidism of incipient and subtle osteomalacia could weaken cortical bone and aggravate the osteoporotic process linked to aging. Increased awareness, by physicians and our elderly, of the importance of avoiding mild, seasonal or perennial vitamin D deficiency could lead to better implementation of vitamin D supplementation schedules.

In an attempt to ascertain a relationship between genotype and phenotype, we studied the pulmonary and nutritional status of 123 cystic fibrosis patients with known genotype at an age of 8.5-10 years. We submitted the 12-lead electrocardiogram of an easily recognized ventricular tachycardia to more than 2500 critical care nurses for diagnosis. Seventy-eight percent of these failed to diagnose ventricular tachycardia, opting for supraventricular tachycardia with ventricular aberration. It was clear that only 0.5% of 2500 experienced nurses knew the three clues available for diagnosing ventricular tachycardia.

In November 1985 a research program started, with the aim to evaluate seasonal variations in the concentrations of organic and inorganic pollutants in the atmosphere of Genoa. The program provided for the collection of samples of airborne particulate in five different areas with distinct urban characteristics. First results for chromium, lead, and thallium concentrations in an industrial and in an urban area are reported.
