Uterine leiomyoma causing urinary obstruction of the transplanted kidney.

Obstruction of the ureter as a cause of acute or chronic kidney injury in the transplanted kidney is unusual beyond the perioperative period. We present a case of ureteric obstruction, infection and septicemia caused by a large uterine leiomyoma in a patient 8 years post transplantation. Initial treatment comprised of intravenous fluid and antibiotics followed by urgent drainage of the collecting system. Subsequent hysterectomy resolved the obstruction with resolution of renal failure. In young female kidney transplant recipients, gynecologic causes, although rare, need to be considered as pos

Sodium bicarbonate in preventing contrast nephropathy in patients at risk for volume overload: a randomized controlled trial.

BACKGROUND: Sodium bicarbonate has been recently proposed as a prophylactic measure for the prevention of contrast-induced nephropathy (CIN). We aimed to compare the efficacy of the combination of sodium bicarbonate with half saline, and half saline alone in preventing CIN in patients having uncontrolled hypertension, compensated severe heart failure or a history of pulmonary edema. METHODS: Seventy-two patients undergoing elective coronary angiography with a serum creatinine level > or =1.5 mg/dL who had uncontrolled hypertension, compensated severe heart failure or a history of pulmonary ed

Fabry disease: perspectives of urinary proteomics.

Fabry disease (FD) is an X-linked lysosomal storage disorder caused by mutations in the gene encoding the lysosomal enzyme a-galactosidase A (a-GalA). The resulting deficiency in a-GalA activity leads to intra-lysosomal accumulation of neutral glycosphingolipids, mainly globotriaosylceramide (Gb3), in various organ systems. As a consequence, a multisystem disorder develops, culminating in strokes and progressive renal and cardiac dysfunction. Enzyme replacement therapy (ERT) offers a specific treatment for patients affected by FD, though monitoring treatment is hampered by a lack of surrogate

Perspectives of 1H-NMR-based urinary metabonomics in Fabry disease.

High resolution proton magnetic resonance spectroscopy (1H-NMR) of body fluids coupled with multivariate data analysis has led to a new science known as metabonomics. Metabonomics is a powerful tool for investigating any disturbance in the normal homeostasis of biochemical processes. In particular, urine metabonomics provides information on the metabolite phenotype of the human being and is therefore appropriate to study the status of the global system. Here we applied 1H-NMR-based urinary metabonomics in a perspective study of the inherited lysosomal storage disorder known as Fabry disease, s

Risk factors of one year increment of coronary calcifications and survival in hemodialysis patients.

BACKGROUND: Heart and coronary calcifications in hemodialysis patients are of very common occurrence and linked to cardiovascular events and mortality. Several studies have been published with similar results. Most of them were mainly cross-sectional and some of the prospective protocols were aimed to evaluate the results of the control of altered biochemical parameters of mineral disturbances with special regard to serum calcium, phosphate and CaxP with the use of calcium containing and calcium free phosphate chelating agents. The aim of the present study was to evaluate in hemodialysis patie

Functional changes in the aging kidney.

The aging process results in remarkable changes in the kidney. These changes are both anatomical and functional and have been considered the cause of the increased propensity of the elderly to acute or chronic renal failure. However, the majority of the early studies on aging enrolled institutionalized elderly patients with several comorbidities such as hypertension and heart disease which could by themselves induce renal alterations. Recently the selection of subjects lacking renal disease or processes known to affect renal function has demonstrated that aging changes are less pronounced in h

Calcium-sensing receptor as a potential modulator of vascular calcification in chronic kidney disease.

Secondary hyperparathyroidism plays an important role in the mineral and bone disorders that are associated with cardiovascular events in chronic kidney disease patients. Secondary hyperparathyroidism is partially due to decreased calcium-sensing receptor expression in parathyroid glands in these patients. Calcimimetics have been demonstrated to be particularly useful to control parathyroid hormone (PTH) oversecretion and concomitantly reduce serum Ca2+ and phosphorus levels in dialysis patients. However, recent findings highlight the role of calcium-sensing receptor allosteric coactivators as

Serum parathyroid hormone and phosphate influence the levels of circulating CD34+ cells in uremia.

BACKGROUND: Parathyroid hormone (PTH) is an active stimulator of bone marrow osteoblasts; it is involved in the niche organization, ie the bone marrow microenvironment which controls the turnover and the fate of endothelial progenitor cells (EPCs). PTH stimulates EPC production; this action can be measured by counting the number of circulating CD34+ cells. METHODS: This observational cross-sectional study aimed to verify this effect in 3 groups of hemodialysis patients with different serum PTH levels. The first group consisted of 11 patients affected by secondary hyperparathyroidism (SHPTH gr

A case of Paget's disease in hemodialysis.

Paget's disease is the second most common bone disease after osteoporosis and causes an excessive bone turnover. Moreover, chronic kidney failure causes an impairment of bone mineral metabolism and electrolytes and PTH homeostasis. As far as we know, this is the first reported case of Paget's disease in a hemodialysis patient: the patient was also affected by secondary hyperparathyroidism and was successfully treated with clodronate, cinacalcet and paracalcitol. The safety and efficacy of this combined therapy was periodically revised in a 12-month follow-up considering the common markers of b

Anticoagulation for renal replacement therapy: different methods to improve safety.

The different methods for anticoagulation of the extracorporeal circuit for renal replacement therapy (RRT) vary in safety profile, bleeding being the most important side effect. Avoiding severe bleeding is a key aim of RRT prescription. The present paper is a clinical review comparing regional anticoagulation with citrate to heparin for continuous RRT, and different anticoagulant strategies for prolonged intermittent therapies. Regional anticoagulation with citrate or the use of prostacyclins provide high safety because they do not increase the patient's risk of bleeding. The use of citrate m

Calcium and phosphate changes after renal transplantation.

Hypercalcemia and hypophosphatemia are frequently observed in recipients of a kidney transplant (KTx). Hypercalcemia has been reported in up to 66% of KTx patients. Many factors have been suggested as the putative causal factors; however, the persistence of moderate-severe secondary hyperparathyroidism, associated with a change in the set-point of the Ca-controlled parathyroid hormone (PTH) secretion, is considered to play a prominent role. Hypercalcemia can negatively impact on both the graft and patient outcome, increasing the incidence of nephrocalcinosis, which can induce a worse graft out

ACE2 and angiotensin 1-7 are increased in a human model of cardiovascular hyporeactivity: pathophysiological implications.

BACKGROUND: ACE and ACE2 produce angiotensin II (Ang II), a vasopressor that induces cardiovascular remodeling, and Ang 1-7, a vasodilator with an antiremodeling effect. While Ang 1-7 has antiarrhythmic properties, at higher concentration it may induce ventricular tachycardia and sudden death. ACE2, therefore, may play an essential role in blood pressure homeostasis, in the long-term complications of hypertension (cardiovascular remodeling), and in the induction of cardiac electric abnormalities. This study evaluated the levels of ACE2 and Ang 1-7 in Bartter's/Gitelman's patients (BS/GS) who h

Kidney early evaluation program: a community-based screening approach to address disparities in chronic kidney disease.

The Kidney Early Evaluation Program (KEEP) is a free, community-based, kidney disease screening program designed to detect chronic kidney disease (CKD) early and promote follow-up evaluation with clinicians to ultimately improve outcomes. This program screens individuals with diabetes, hypertension, or those with a first-degree relative with diabetes, hypertension, and/or kidney disease. Data based on 89,552 KEEP participants screened in 49 states from early August 2000 until end of December 2007 disclosed a prevalence of 34% African Americans, 12.4% Hispanics, 5.6% Asian/Pacific Islanders, an

A case of infectious endocarditis-associated crescentic glomerulonephritis with intracranial hemorrhage.

A 55-year-old woman was admitted to our hospital because of fever and renal impairment. The patient had undergone a tooth extraction 11 months prior to admission. Echocardiography demonstrated vegetation on the mitral valve, and Streptococcus mitis was detected on blood culture. Accordingly, infectious endocarditis (IE) was diagnosed. Renal biopsy showed crescentic glomerulonephritis. Based on the negative staining for immunoglobulins and complement components in immunofluorescence study and lack of dense deposits on electron microscopy, the renal involvement was considered to be of the pauci-

Fluid balance issues in the critically ill patient.

In critically ill patients, fluid balance management is an integral part of the process of care. In patients in shock or severe sepsis, aggressive initial fluid resuscitation has been shown to improve overall prognosis. However, in critically ill patients, cumulative fluid accumulation is recognized as a potential contributing factor to increased morbidity and mortality. Randomized clinical trials are urgently required to assess the role of fluid overload in mortality and morbidity in this population. In the meantime, we should not only focus on acute fluid resuscitation but also on cumulative

Oliguria, creatinine and other biomarkers of acute kidney injury.

Acute kidney injury (AKI) and fluid overload are conditions that require an early diagnosis and a prompt intervention. The recognition of these pathologic conditions is possible in the early stages if specific signs and symptoms are taken into account. Among them, oliguria represents an important sign. Reduced urine output for a certain number of hours may be an important sign of kidney dysfunction. This must be evaluated in conjunction with other factors such as hydration status and use of drugs. At the same time, traditional markers of kidney function such as urea nitrogen and creatinine mus

Technical aspects of extracorporeal ultrafiltration: mechanisms, monitoring and dedicated technology.

Fluid overload may occur in patients with heart failure. Further complications may arise when cardiorenal syndromes develop and the kidneys are unable to eliminate the accumulated fluid. Diuretics represent the fist line of treatment, although in some case they may be ineffective or even dangerous for the patient. In these conditions, extracorporeal ultrafiltration may be required. Extracorporeal ultrafiltration can be performed continuously or intermittently, using dedicated machines. The goal is to remove the right amount of fluid without causing hemodynamic instability or further ischemia t

Fluid assessment and management in the emergency department.

Evaluation of hydration state or water homeostasis is an important component in the assessment and treatment of critically ill patients in the emergency department (ED). The main purpose of ED physicians is to immediately distinguish between normal hydrated, dehydrated and hyperhydrated states. Fluid depletion may result from renal losses and extrarenal losses (from the GI tract, respiratory system, skin, fever, sepsis, third space accumulations). Total body fluid increase can result from heart failure, kidney disease, liver disease, malignant lymphoedema or thyroid disease. In patients with f

Management of acute kidney injury in liver disease.

Acute deterioration in kidney function often occurs in patients with acute and chronic liver disease admitted to hospital. The true incidence of acute kidney injury (AKI) is unknown due to the difficulty in accurately measuring kidney function in patients with liver failure, and the lack of a consensus definition of AKI. There is a current consensus definition of hepatorenal syndrome (HRS), but in current clinical practice, volume-unresponsive AKI or acute tubular necrosis are much more common causes of AKI in patients with liver disease. Due to arterial vasodilatation and compensatory neuroen
