Periprosthetic humeral fractures after shoulder and elbow arthroplasty.

Due to rapidly increasing numbers of arthroplasty surgeries of the upper extremity, periprosthetic humeral fractures after shoulder and elbow arthroplasty, formerly described as rare, may hence increase in the near future. Therefore the aim of the present work was to give an overview of the existing literature including possible classifications as well as an update on treatment concepts and experiences with own cases. After a literature research have been done, existing prevalence, classifications and treatment options, mostly described in case series, were processed to create an overview of t

Acute traumatic subclavian artery thrombosis and its successful repair via resection and end-to-end anastomosis.

Subclavian artery thrombosis is a rare complication of clavicle fractures. We reported a 20-year-old man who was admitted to the emergency room after a road traffic accident. He was a pedestrian who was initially hit by a bus and after he fell down on the road, he was run over by a car. On evaluation, he was found to have multiple facial and rib fractures, distal right humerus and right clavicle fracture. Significantly, right radial pulse was absent. After further evaluation including Doppler studies and an angiography which revealed complete obstruction of right subclavian artery just distal

On-table reconstruction of comminuted fractures of the radial head.

The most widely accepted treatment for comminuted fractures of the radial head is either the excision or open reduction and internal fixation. The purpose of the present study is to evaluate the value of an 'on-table' reconstruction technique in severely comminuted fractures of the radial head. In this study, two patients with a Mason type-III and four patients with a Mason type-IV radial-head fracture were treated with 'on-table' reconstruction and fixation using low-profile mini-plates. After a mean follow-up of 112 months (47-154 months), the mean elbow motion was 0-6-141 degrees extension

Resorbable poly-L-lactide plates and screws for the treatment of mandibular condylar process fractures: a clinical and radiologic follow-up study.

PURPOSE: The purpose of this study was to determine whether a resorbable poly-l-lactide (PLLA) miniplate system could be used to treat mandibular condylar process fracture. PATIENTS AND METHODS: Fourteen patients (12 males, 2 females, aged 23.1 +/- 5.7 years) who had mandibular condylar process fractures treated with PLLA implants were recalled for follow-up clinical and radiologic examinations at 3 years. RESULTS: Mouth opening recovered to more than 35 mm and occlusion was stable in all patients. There was no facial asymmetry 3 months postoperatively. Two patients had mild chronic postoper

Treatment of malunited fractures of the anterior tibial spine.

Avulsion fractures of the anterior tibial spine are uncommon injuries, and most often seen in childhood. They can be associated with other intraarticular lesions and have, if adequately treated, a good long-term prognosis. In adults these fractures can be complicated by loss of knee extension because of the displacement of the bony fragment. This is most often as a result of non- or malunion of the displaced fragment. We review two cases of malunited fractures of the anterior tibial spine. Both patients reported persistent knee pain and loss of knee extension, and had had an anterior knee laxi

Morbidity associated with ORIF of intra-articular calcaneus fractures using a lateral approach.

This is a review of 183 patients with 218 displaced intraarticular fractures of the calcaneus treated by open reduction and internal fixation. One hundred and ninety four (89%) wounds underwent primary uneventful healing. Twenty-four wounds (11%) required local wound care. One deep infection occurred in a neuropathic foot that required below-knee amputation. No free-tissue transfers, local tissue flaps or skin grafts were needed in patients who presented initially with a closed fracture. Ninety-five fractures (43.5%) required subsequent surgical procedures (hardware removal-88/95 [93% of secon

Changing patterns in the epidemiology and treatment of zygoma fractures: 10-year review.

A ten year retrospective review of 259 zygoma fractures is presented to highlight changes in epidemiology and treatment. Motor vehicle-related trauma resulted in a majority of the injuries (80.6%), with a high incidence of multiple facial fractures (43.2%). The number of zygomatic and other facial fractures decreased over the duration of the study (by 50.0% and 20.1%, respectively, p < 0.05), perhaps reflecting lowered speed limits and the increased use of seat belts. The proportion of fractures receiving open reduction and internal fixation (ORIF) remained relatively constant (46.3%). At pres

Acetabular "Fleck" Sign: Outcomes of Surgical Repair.

BACKGROUND: Traumatic, posterior hip dislocations in the pediatric population are typically managed by closed reduction to achieve a concentric hip joint. The presence of an acetabular "fleck" sign, despite concentric reduction, has been shown to signify significant hip pathology. The purpose of this study was to evaluate the outcomes of open labral repair through a surgical hip dislocation (SHD) in a consecutive series of patients with an acetabular "fleck" sign associated with a traumatic hip dislocation/subluxation. METHODS: A retrospective review of patients between 2008 and 2022 who pres

Principles or Resources? Best Management of Pediatric Femoral Neck Fractures.

BACKGROUND: Pediatric femoral neck fractures (PFNF) are rare but have a reported complication rate of >30%. To date, there remains a lack of consensus on the definitive risk factors for AVN following PFNF. By comparing the outcomes of PFNF treated at a major US metropolitan area tertiary-level 1 pediatric trauma center (group 1) to an equivalent trauma hospital in a major Latin American capital (group 2), we sought to weigh whether treatment principles or resources are more important for a favorable result. METHODS: This was a retrospective review of all children (<18 years old) treated for P

Surgical Management of Paediatric Naso-Orbito-Ethmoid Fracture using a Stainless Steel Plate: A Case Report.

Naso-orbito-ethmoid (NOE) fractures in paediatric patients are uncommon but can result in significant functional and aesthetic impairments if not managed promptly. This case report describes a paediatric patient, who sustained an NOE fracture following self-fall. The patient exhibited pain, periorbital swelling, ecchymosis, and depressed nasal bridge. The patient was evaluated clinically and radiologically, revealing displacement of the NOE complex. Given the potential for long-term complications, open reduction and internal fixation with a stainless steel plate and screws under general anaest

Adjunctive IlluminOss Fixation for a Vancouver B1 Periprosthetic Femoral Fracture: A Case Report.

Open reduction and internal fixation with plates and intramedullary nails is often the standard approach for periprosthetic femoral shaft fractures. However, in patients with recent hip arthroplasty, these traditional methods may be invasive, difficult to position, or carry a higher risk of periprosthetic complications. The IlluminOss system (IlluminOss Medical, East Providence, RI, USA) offers a minimally invasive alternative that delivers a light-curable polymer through a balloon catheter that takes the shape of the intramedullary canal without the need for rigid instrumentation that may ind

Bilateral Simultaneous Patellae Fractures Following Knee Trauma: A Case Report.

Bilateral simultaneous fractures of the patellae were first documented in the literature in the early 19th century, and they remain uncommon today. The patella is the largest sesamoid bone in the body and plays an important role in the knee extensor mechanism. Isolated bilateral patellar fractures following trauma are exceedingly rare (2-3% of all patellar fractures) and most commonly occur with dashboard injuries or various pathological disorders. We report a case of a 43-year-old healthy African male who presented with bilateral simultaneous extensor mechanism disruption following knee traum

Accelerated Return to Play in Professional Basketball Players With Surgically-Treated Metacarpal Shaft Fractures.

BACKGROUND: Traditional rehabilitation protocols for surgically treated metacarpal shaft fracture allow for return to play at 6-8 weeks post-operative. This may be devastating for the elite athlete. We outline a protocol that may allow for professional basketball players to successfully return to sport within four weeks following surgery. METHODS: Professional basketball players who sustained non-thumb metacarpal shaft fractures were included. All athletes underwent open reduction and internal fixation of the injured metacarpal. Patients were subsequently enrolled into an accelerated rehabili

Primary Total knee Arthroplasty: A Viable Surgical Option for Complex Tibial Plateau Fractures in Elderly.

The optimal treatment of complex tibial plateau fractures in elderly is still controversial. The aim of the study was to retrospectively analyze the clinical and radiological outcomes of primary total knee arthroplasty (TKA) versus open reduction and internal fixation (ORIF) in elderly patients presenting with acute complex tibial plateau fractures. Between June 2011 and December 2015, we have analyzed the clinical outcomes of 22 primary total knee replacements and 21 ORIFs. The mean follow-up was 27 months. The intra- and postoperative complications, as well as the knee society score (KSS), w

The burden of hardware removal in ankle fractures: How common is it, why do we do it and what is the cost? A ten-year review.

BACKGROUND: Ankle fractures account for 9% of all fractures and 40% require surgical management. The ankle is the most common site of hardware removal. The purpose of our study was to investigate the incidence, indication and economic cost associated with removal of hardware from the ankle. METHODS: We conducted a ten-year retrospective review of 1482 patients treated by open reduction internal fixation for an unstable ankle fracture. Skeletally immature patients were excluded. Data collected was cross referenced from patient medical records, the radiological and electronic patient database.

Acetabular fractures following rugby tackles: a case series.

INTRODUCTION: Rugby is the third most popular team contact sport in the world and is increasing in popularity. In 1995, rugby in Europe turned professional, and with this has come an increased rate of injury. CASE PRESENTATION: In a six-month period from July to December, two open reduction and internal fixations of acetabular fractures were performed in young Caucasian men (16 and 24 years old) who sustained their injuries after rugby tackles. Both of these cases are described as well as the biomechanical factors contributing to the fracture and the recovery. Acetabular fractures of the hip

Early motion protocol for select Galeazzi fractures after radial shaft fixation.

Galeazzi fractures traditionally are treated in long arm casts with the wrist fully supinated for 6 weeks after open reduction and internal fixation. Recent literature suggests that early motion can be permitted for a subset of Galeazzi fractures. Defining a safe postoperative protocol that allows immediate elbow motion, immediate platform weight bearing, and early wrist motion might decrease elbow morbidity, increase range of motion, and improve outcomes. A retrospective review of a prospectively collected database of 26 patients at a level I trauma center was conducted. Early motion protocol

Outcome of shoulder hemiarthroplasty in acute proximal humeral fractures: a frustrating meta-analysis experience.

Limited evidence is available on the optimal treatment of complex fractures of the proximal humerus. No randomized prospective studies have compared hemiarthroplasty, open reduction internal fixation and/or conservative treatment. Two systematic reviews are available but they do not include angle-stable plate osteosynthesis or third-generation prosthetic designs. We conducted a systematic review of studies published in the last 10 years. The MOOSE guidelines were followed and we focused on functional outcome and its relation to age, gender and tuberosity healing after hemiarthroplasty. Sixteen
