[Trauma of the Membranous Trachea Managed by the ENT-Surgeon].

BACKGROUND: Isolated perforations of the membranous trachea are extremely rare but potentially life-threatening complications of endotracheal intubations and minimally invasive tracheostomy techniques. Most of the case-reports have been written by thoracic surgeons or anesthesiologists but both the diagnostic procedures and the therapy are not standardized. The aim of this study was to evaluate the position of the ENT-surgery in the management of these lesions. PATIENTS AND METHODS: Over a period of 6 years we treated 5 females, 3 males and 3 children with iatrogenic lacerations of the poster

Impact of perceived stress, major life events and pregnancy attitudes on low birth weight.

CONTEXT: While low birth weight is the leading cause of infant mortality and morbidity, the factors influencing low birth weight are not well understood. In particular, the relationship between stressful life events and birth outcomes is unclear. It is important for health care providers to better understand the impact of stress on health outcomes. METHODS: Data from a statewide case-control study of 2,378 Missouri mothers are used to examine the relationship of perceived stress, pregnancy attitudes and major life events as psychosocial risk factors on very low birth weight (i.e., birth weigh

State care in childhood and adult mortality: a systematic review and meta-analysis of prospective cohort studies.

BACKGROUND: Removal from family of origin to state care can be a highly challenging childhood experience and is itself linked to an array of unfavourable outcomes in adult life. We aim to synthetise evidence on the risk of adult mortality in people with a history of state care in early life, and assess the association according to different contexts. METHODS: In this systematic review and meta-analysis, we focused on four health outcomes hypothesised to be associated with exposure to early state care: total mortality, cardiovascular disease, cancer, and suicide. We searched the electronic dat

Functional Outcomes and Anti-Reflux Performance of Reconstruction Methods Following Proximal Gastrectomy: A Systematic Review.

Proximal gastrectomy preserves gastric function; however, reconstruction choice influences reflux control and quality of life (QOL), and the comparative efficacy of available techniques remains unclear. A PRISMA-guided search of PubMed and Web of Science Core Collection (January 2000-April 2025) identified studies reporting reflux or QOL outcomes after proximal gastrectomy. Eligible studies included adult gastric cancer cohorts with original clinical data. Two reviewers independently screened articles, extracted predefined variables, and performed a narrative synthesis. The protocol was archiv

How do domain diversity and healthcare system assumptions shape implementation research? A systematic review of theories, models, and frameworks for health digitalization.

INTRODUCTION: Health digitalization entails a complex array of actors and roles, including healthcare stakeholders and commercial digital-health innovators. The implementation of digital health innovations (DHIs) is notoriously prone to failure. Consequently, many theories, models, and frameworks (TMFs) have been developed to explain or guide the process of health digitalization. Depending on their scholarly origins, these TMFs differ in their perspective, origin, and terminology, leading to knowledge fragmentation and confusion. The aim of this review was to create an overview of the availabl

Brief intervention, lasting impact: One-year outcomes of the Bergen 4-day treatment for OCD in Germany.

BACKGROUND: Obsessive-compulsive disorder (OCD) is a prevalent and highly disabling mental disorder. Worldwide, it is associated with substantial individual suffering and long-term societal costs. Although exposure and response prevention (ERP) is the recommended first-line treatment, access to effective care remains limited for reasons such as long treatment duration. These barriers contribute to persistent global treatment gaps. The Bergen 4-Day Treatment (B4DT) is a brief, concentrated form of ERP (cERP) designed to improve treatment efficiency and access. Although initial outcomes are prom

Efficacy of Adalimumab Biosimilars in Managing Perianal Fistula in Moderate to Severe Crohn's Disease: A Systematic Review and Meta-Analysis.

BACKGROUND: Perianal fistulas represent a severe complication of Crohn's disease (CD), leading to significant morbidity and a decline in patients' quality of life. Adalimumab, a tumor necrosis factor (TNF) inhibitor, has been widely used for managing CD-related fistulas. However, with the introduction of adalimumab biosimilars, their comparative efficacy in fistula closure remains unclear. This systematic review and meta-analysis assessed the effectiveness and safety of adalimumab biosimilars in achieving complete fistula closure in moderate to severe CD. OBJECTIVE: To evaluate the efficacy o

Maternal High-Fat Diet Alters Fetal Autonomic Activity Independently of Leptin Receptor Genotype in High-Risk Mouse Models of Type 2 Diabetes Mellitus.

BACKGROUND: The Developmental Origin of Health and Disease (DOHaD) concept suggests fetal environmental factors influence disease vulnerability later in life. Type 2 diabetes mellitus (T2DM), a metabolic disease, is associated with autonomic dysfunction. Some T2DM pathophysiological manifestations may exhibit DOHaD features and these autonomic changes could originate during fetal development. OBJECTIVES: This study investigated genetic and environmental T2DM risks on fetal autonomic activity, focusing on leptin receptor gene mutation, maternal high-fat diet, and their interactions. METHODS:

Pelvic floor injury during vaginal birth is life-altering and preventable: what can we do about it?

Pelvic floor disorders after childbirth have distressing lifelong consequences for women, requiring more than 300,000 women to have surgery annually. This represents approximately 10% of the 3 million women who give birth vaginally each year. Vaginal birth is the largest modifiable risk factor for prolapse, the pelvic floor disorder most strongly associated with birth, and is an important contributor to stress incontinence. These disorders require 10 times as many operations as anal sphincter injuries. Imaging shows that injuries of the levator ani muscle, perineal body, and membrane occur in

Reported adverse events during out-of-hospital mechanical ventilation and ventilatory support in emergency medical services and critical care transport crews: a systematic review.

BACKGROUND: Emergency medical services (EMS) and critical care transport crews constantly face critically-ill patients who need ventilatory support in scenarios where correct interventions can be the difference between life and death; furthermore, challenges like limited staff working on the patient and restricted spaces are often present. Due to these, mechanical ventilation (MV) can be a support by liberating staff from managing the airway and allowing them to focus on other areas; however, these patients face many complications that personnel must be aware of. AIMS: To establish the main c

COMMunity of Practice And Safety Support for Navigating Pain (COMPASS-NP): study protocol for a randomized controlled trial with home care workers.

BACKGROUND: Chronic pain is a prevalent and costly problem that often has occupational origins. Home care workers (HCWs) are at high risk for work-related injuries, pain, and disability. Current treatments for chronic pain emphasize medications, which are an inadequate stand-alone treatment and can produce significant adverse effects. METHODS: In this translational study, we will adapt an established work-based injury prevention and health promotion program (COMmunity of Practice And Safety Support: COMPASS) to address the needs of HCWs experiencing chronic pain. COMPASS employs peer-led, scr

Salpingectomy for the Primary Prevention of Ovarian Cancer: A Systematic Review.

IMPORTANCE: Most ovarian cancers originate in the fimbriated end of the fallopian tube. This has led to the hypothesis that surgical resection of the fallopian tubes at the time of gynecologic and nongynecologic surgical procedures-referred to as an opportunistic salpingectomy-may prevent the development of epithelial ovarian cancer for women at an average risk of developing the disease. OBJECTIVE: To compile a comprehensive, state-of-the-science review examining the current landscape of performing bilateral salpingectomy for ovarian cancer prevention. EVIDENCE REVIEW: A systematic review of

Adaptation of anticancer strategies to progress in tumor immunology.

The numerical development of cancer cells during tumor growth and under current forms of therapy is quantitatively described as a fundamental of a scientifically founded anticancer strategy. As to general cancer prevention, suppression of metastasis and reduction of recurrencies, figures are derived, which allow one to assess the minimal number of malignant cells to be killed. The killing potency of current tumor-immunological measures has not so far been sufficient enough to have a good chance of achieving these ambitious goals. The main reason for this is the neglect of the oxygen status of
