Effective Use of F-18-Fluorodeoxyglucose Positron Emission Tomography/Computed Tomography to Rule Out Prosthetic Aortic Valve as the Source of Infection.

Several nuclear imaging techniques can be used to diagnose infectious and inflammatory processes. F-18-fluorodeoxyglucose (FDG) positron emission tomography/computed tomography (PET/CT) is a useful diagnostic technique to detect inflammation and infection quickly and accurately. We report the case of a patient with end-stage renal disease (ESRD) and recurrent bacterial infections where FDG PET/CT was used to identify the source of infection as sternal osteomyelitis and rule out suspected infection of the aortic valve prosthesis.

Single photon emission computed tomography/computed tomography of the skull in malignant otitis externa.

Malignant otitis externa is a severe, rare infective condition of the external auditory canal and skull base. The diagnosis is generally made from a range of clinical, laboratory, and imaging findings. Technetium 99m methylene diphosphonate bone scintigraphy is known to detect osteomyelitis earlier than computed tomography. The authors present a patient with bilateral malignant otitis externa where the extent of skull base involvement was determined on 3-phase bone scintigraphy with single photon emission computed tomography/computed tomography.

Use of pristinamycin for infections by gram-positive bacteria: clinical experience at an Australian hospital.

Thirty-six patients were treated with pristinamycin for 46 different microbiological isolates between April 2007 and July 2009. Pathogens included 9 methicillin-resistant Staphylococcus aureus isolates, 13 methicillin-resistant coagulase negative staphylococci, and 9 vancomycin-resistant enterococci. Sites of infections included 12 osteomyelitis cases, 10 prosthetic joints, 4 other prostheses, and 1 epidural abscess. Five patients ceased treatment due to side effects. Ten patients were cured of their infections, and 21 patients had infections successfully suppressed.

Pseudoparalysis of the lower extremity in an infant.

We describe a case of septic arthritis of the knee in an afebrile, well-appearing four-month-old female. She had been evaluated previously for lower extremity disuse attributed to antecedent trauma. Her physical examination was remarkable only for limitation of extension of the affected leg at the knee. Emergency physicians should understand the physical examination findings suggesting joint effusion and the need to consider osteomyelitis/septic arthritis in children less than one year of age with pseudoparalysis.

Pressure ulcerations.

Pressure sore are caused by multiple mechanisms. Preventive management requires relief of pressure, skin care, control of spasms, release of contractures and treatment of infection. Conservative treatment succeeds only when pressure on the ulcer site is relieved. Surgical treatment involves primary, secondary and tertiary procedures. Benefits of surgical closure include avoidance of protein loss, prevention of osteomyelitis and other infections, improved appearance and prevention of carcinoma and amyloidosis. A coordinated team approach, incorporating patient education, is effective.

Disease and mortality in the Early Bronze Age people of Bab edh-Dhra, Jordan.

During the 1977 field season at the Early Bronze Age site of Bab edh-Dhra 92 individuals were recovered from underground shaft tomb chambers. Morbid conditions found in these skeletons include trauma, possibly two cases of tuberculosis, osteomyelitis, post-menopausal osteoporosis and congenital anomalies. Of the 92 skeletons recovered 56 (61%) were 18 years of age or older, 28 (30%) were between 1 and 18 years of age and 8 (9%) were less than one year of age.

Resuscitation and Evaluation With Intraosseous Access: A Review of the Literature in Trauma and Non-Trauma Patients.

According to trauma resuscitation guidelines, intraosseous (IO) access is appropriate when failure to gain intravenous (IV) access is present in trauma, burn, shock, or resuscitation settings for adults or when two failed attempts have been made in the resuscitation of a pediatric patient. However, their effectiveness and use have been debated due to concerns on flow rates, extravasation, compartment syndrome, and osteomyelitis. The objective of this review is to examine the current literature regarding intraosseous access in trauma resuscitation, focusing on interventions and complication rat…

Pott's puffy tumor: A rare complication of sinusitis. A case report.

A rare complication of frontal sinusitis includes Pott's puffy tumor. It manifests as a swelling of the forehead due to the presence of a subperiosteal abscess secondary to osteomyelitis of the frontal bone. A timely diagnosis allows for an early, intensive medical and surgical treatment, which is critical to prevent serious intracranial complications. Here we describe the case of a 12-year-old boy with Pott's puffy tumor as a complication of pansinusitis. This case was a diagnostic challenge; however, a timely treatment allowed for a favorable clinical course.

Lower extremity infections: Essential anatomy and multimodality imaging findings.

In modern practice, imaging plays an integral role in the diagnosis, evaluation of extent, and treatment planning for lower extremity infections. This review will illustrate the relevant compartment anatomy of the lower extremities and highlight the role of plain radiographs, CT, US, MRI, and nuclear medicine in the diagnostic workup. The imaging features of cellulitis, abscess and phlegmon, necrotizing soft tissue infection, pyomyositis, infectious tenosynovitis, septic arthritis, and osteomyelitis are reviewed. Differentiating features from noninfectious causes of swelling and edema are disc…

Staphylococcus schleiferi subspecies coagulans septic shock in an immunocompetent male following canine otitis externa.

Staphylococcus schleiferi bacteremia is an underappreciated cause of septic shock in the critical care department. Although nominally a coagulase variable Staphylococcus and associated with otitis externa infections in canine species, it has been associated with the metastatic infection including osteomyelitis, endocarditis, nephritis, and meningitis in humans. This report records a possible zoonotic case of S. schleiferi subspecies coagulans bacteremia following canine otitis externa associated with septic shock and endovascular infection precipitating intensive care admission for vasopressor…

[Society for Pediatric and Adolescent Rheumatology (GKJR) diagnosis and treatment optimization board-New ways to the diagnosis and treatment of complex diseases : An analysis after 2 years testing in practice].

With the diagnosis and treatment optimization board, the Society for Pediatric and Adolescent Rheumatology (GKJR) has developed a new format for expert-based discussion of rare and complex diseases. So far, 32 cases, predominantly from the areas of hyperinflammation, systemic lupus erythematosus, myositis and nonbacterial osteomyelitis, could be discussed in 8 conferences. The digital format enabled a high number of participants and the involvement of national and international experts. Rare diseases increasingly present modern medicine with challenges, which the GKJR meets with the new format…

Maxillary Mucormycotic Osteonecrosis as a Manifestation of Post-COVID-19 Infection in Non-diabetic Patients: Report of Two Cases.

Mucormycosis is a rare, opportunistic fungal infection prevalent in the elderly, especially in patients with uncontrolled diabetes or a compromised immune system. However, the outbreak of the pandemic declared by the World Health Organization in late 2019 increased the incidence of mucormycosis cases in COVID-19 patients due to increased steroid usage that resulted in an immunocompromised state. The current report presents two cases of oral mucormycotic osteomyelitis in patients who developed diabetes post-COVID infection, along with a review and relevant literature.

Biocompatibility Studies on a Collagen-Hydroxyapatite Biomaterial.

The current treatment of osteomyelitis includes systemic antibiotic therapy and a debridement procedure of the formed biofilm and necrotic tissue. Moreover, cements and three-dimensional scaffolds are used both for the delivery of therapeutic agents and as fillers for bone defects. The aim of our research was to test, on cellular cultures, the biocompatibility of a previously synthesized microporous biocomposite containing hydroxyapatite and a collagen matrix including a therapeutic agent (ciprofloxacin and gentamicin). The scaffold was obtained by direct mineralization namely co-precipitation…

Orocutaneous fistulas of odontogenic origin presenting as a recurrent pyogenic granuloma.

Orocutaneous fistulas, or cutaneous sinuses of odontogenic origin, are uncommon but often misdiagnosed as skin lesions unrelated to dental origin by physicians. Accurate diagnosis and use of correct investigative modalities are important because orocutaneous fistulas are easily confused for skin or bone tumors, osteomyelitis, infected cysts, salivary gland fistulas, and other pathologies. The aim of this study is to present our experience with a patient with orocutaneous fistulas of odontogenic origin presenting as recurrent pyogenic granuloma of the cheek, and to discuss their successful trea…

[An rare complication of scarlet fever : invasive group A streptococcal infection with streptococcal toxic shock syndrome].

Invasive Group A Streptococcus infections and streptococcal toxic shock syndrome are rare complications of common diseases in children such as scarlet fever or impetigo. These invasive diseases are particulary challenging because of their rapid progression and the lack of predisposing factors in most cases. Prompt diagnosis and treatment are mandatory to reduce the mortality associated with these severe diseases. We report the case of an 8- year-old girl who developped an invasive group A streptococcal disease with osteomyelitis and streptococcal toxic shock syndrome in the course of a classic…

A 50-year-old man presenting with rash after exposure to sunlight.

Photodermatoses is a condition characterized by development of a skin rash following exposure to sunlight. Here we present the case of a patient who presented with pigmented, pruritic, blistering rash that developed shortly after exposure to sunlight. Careful history revealed that the patient was on suppressive therapy with doxycycline for osteomyelitis. Following stopping of doxycycline, and symptomatic treatment, the patient had significant improvement in symptoms. The case illustrates that clinicians should be cognizant of drug induced photodermatoses particularly in individuals exposed to …
