Software-based finger joint range of motion analysis: Current concepts, considerations, and challenges.

BACKGROUND: Finger and wrist joint range of motion (ROM) measurement are key parameters of the hand examination and contribute to the diagnosis and monitoring of hand pathology. Technology-based ROM measurement tools, particularly software-based systems such as image analysis systems or augmented reality--based measurement, have potential to facilitate efficient ROM measurement in both virtual and in-person settings. PURPOSE: The purpose of this paper is to provide some knowledge translation of available and prospective technologies from developer to clinician literature. Specifically, this a

Work-related physical strain and development of joint inflammation in the trajectory of emerging inflammatory and rheumatoid arthritis: a prospective cohort study.

OBJECTIVES: Rheumatoid arthritis (RA) mainly affects small joints. Despite the mechanical function of joints, the role of mechanical stress in the development of arthritis is insufficiently understood. We hypothesised that mechanical stress/physical strain is a risk factor for joint inflammation in RA. Therefore, we studied work-related physical strain in subjects with clinically suspected arthralgia (CSA) as a risk factor for the presence of imaging-detected subclinical joint inflammation and the development of clinical arthritis/RA. METHODS: In 501 CSA patients and 155 symptom-free persons'

Antibiotic exposure and IBD development among children: a population-based cohort study.

OBJECTIVE: To determine whether childhood antianaerobic antibiotic exposure is associated with the development of inflammatory bowel disease (IBD). METHODS: This retrospective cohort study employed data from 464 UK ambulatory practices participating in The Health Improvement Network. All children with ≥ 2 years of follow-up from 1994 to 2009 were followed between practice enrollment and IBD development, practice deregistration, 19 years of age, or death; those with previous IBD were excluded. All antibiotic prescriptions were captured. Antianaerobic antibiotic agents were defined as penicilli

Laboratory animal allergy: an update.

Allergic reactions are among the most common conditions affecting the health of workers involved in the care and use of research animals. Between 11 and 44% of the individuals working with laboratory animals report work-related allergic symptoms. Of those who become symptomatic, 4 to 22% may eventually develop occupational asthma that can persist even after exposure ceases. Allergic symptoms consist of rashes where animals are in contact with the skin, nasal congestion and sneezing, itchy eyes, and asthma (cough, wheezing, and chest tightness). The generation of immunoglobulin E (IgE) antibodi

Systemic sclerosis developing in association with the use of interferon alpha therapy for chronic viral hepatitis.

In 1992 interferon alpha (IFNalpha) was approved by the FDA for the treatment of chronic viral hepatitis B and C. Since then IFNalpha has been implicated in the development of several autoantibodies as well as in the development or exacerbation of various autoimmune disorders. Herein, we describe a 47-year-old female who developed a limited form of systemic sclerosis (SSc) with lung involvement 6 months after the institution of IFNalpha therapy for chronic active hepatitis C. There was no family or personal history of autoimmune diseases. We speculate that the immunomodulatory effects of IFNal

Gaps and opportunities: an agenda for further research, services, and program development in spina bifida.

In this article an agenda is discussed for further research, services, and program development identified in the development of the Life Course Model Web site for individuals with spina bifida, their families and the health care providers who work with them. The gaps identified during development of the Life Course Model Web site revealed that there has been minimal progress made on the research agenda since the 2003 consensus document, "Evidence-Based Practice in Spina Bifida: Developing a Research Agenda" or the summary document from the First World Congress on Spina Bifida Research and Care

Fertile Ground in the OR: Rewriting the Narrative on Family in Plastic Surgery Training.

This editorial discusses the possibility of family life and plastic surgery residency successfully coexisting when training programs intentionally foster supportive cultures, flexible policies, and equitable parental leave. Drawing on personal experiences from a highly family-friendly program, together with emerging literature on pregnancy, parenthood, childcare, and career advancement in surgery, we challenge the notion that residents must choose between surgical excellence and family life. Some practical strategies, including shared workload, flexible scheduling, lactation support, peer ment

Telehealth Administration of the Affect in Play Scale in Children with Developmental Differences.

The Affect in Play Scale (APS) is a standardized pretend play assessment that measures cognitive and affective skills in school-aged children. We examined the comparative utility of the APS via telehealth across three diagnostic groups. Ninety-four children ages 6 to 9 who were either typically developing or had high-functioning autism spectrum disorder or Prader-Willi syndrome completed the APS either in-person or via videoconferencing in a single mode administration design. Equivalence analyses compared in-person versus remote APS scores within diagnostic groups. Paired samples t tests revea

Digital health for cancer prevention: implications for current and future traditional in-person programs.

Digital health is expanding the range of delivery options available for cancer prevention and may support the development of integrated in-person, digital, and hybrid models. This commentary proposes a framework for cancer prevention delivery across modalities. It argues that the expansion of virtual platforms for cancer-related psychosocial and behavioral interventions may be redefining prevention pathways rather than merely adding digital options. It highlights the possible redistribution of some interventions, such as human papillomavirus vaccination and hepatitis B vaccination, toward in-p

Precision nutrition management in hyperlipidemia-associated acute pancreatitis: mechanistic insights and personalized therapeutic approaches.

Hyperlipidemia-associated acute pancreatitis (HLAP), an acute inflammatory disorder triggered by dyslipidemia, has witnessed a rising global incidence with significant health implications. The pathogenesis of HLAP involves complex interactions among lipid metabolism dysregulation, inflammatory cascades, and oxidative stress. Conventional therapeutic approaches, while providing partial symptomatic relief, exhibit limitations in addressing individual variability. Precision nutrition management emerges as a novel paradigm integrating multi-omics profiling (genomic, metabolomic) and clinical param

Case Report: A case of influenza A infection-associated stiff person spectrum disorder with favorable outcome.

Influenza A (H1N1) virus infection has been associated with various immune-mediated neurological disorders. Previous studies have reported cases of stiff person spectrum disorder (SPSD) subsequent to viral infections. We report a case of a 4-year-old boy who developed SPSD following H1N1 infection. The patient primarily presented with axial and limb muscle rigidity, painful spasms, and exaggerated startle responses. Electromyography (EMG) revealed continuous synchronous discharges in agonist and antagonist muscles, consistent with the characteristic manifestations of stiff person syndrome (SPS

[Guide of recommendations for the diagnosis and neurorrehabilitative treatment of post-poliomielytic sequelar conditions].

Postpolio syndrome (PPS), which affects 20% to 50% of patients decades after initial poliovirus infection, poses a significant clinical challenge due to its unknown etiology and lack of pathogenic treatment. The main current theories point to degenerative processes secondary to the initial infection as the basis of its pathogenesis. Our guide addresses this complex landscape, providing a structured, evidence-based approach to the management of these sequelae. We emphasize the importance of: A personalized comprehensive evaluation; The development of multidisciplinary programs specifically desi

RISE-Vac-Co-production of Vaccine Education Materials with Persons Living in Prison.

Increasing vaccination knowledge is effective in addressing hesitancy and is particularly important in populations deprived of liberty who may not routinely have access to health information, ensuring health equity. RISE-Vac is a European Union-funded project aiming to promote vaccine literacy, offer, and uptake in prisons in Europe. We consulted persons living in prisons in the United Kingdom (through the Prisoner Policy Network), France, and Moldova to determine their vaccination knowledge gaps, the information they would like to receive, and how they would like to receive it. We received 34

Guidelines on the diagnosis and treatment of active Charcot neuro-osteoarthropathy in persons with diabetes mellitus (IWGDF 2023).

The International Working Group on the Diabetic Foot (IWGDF) has published evidence-based guidelines on the prevention and management of diabetic foot disease since 1999. This is the first guideline on the diagnosis and treatment of active Charcot neuro-osteoarthropathy in persons with diabetes published by the IWGDF. We followed the GRADE Methodology to devise clinical questions in the PACO (Population, Assessment, Comparison, Outcome) and PICO (Population, Intervention, Comparison, Outcome) format, conducted a systematic review of the medical literature, and developed recommendations with th

How we manage cardiovascular disease in patients with hemophilia.

With the striking advances in hemophilia care that have materialized particularly in the last two decades, an increasing number of persons with hemophilia (PWH) have achieved a quality of life and life expectancy very close to that of unaffected individuals. With aging, a growing number of PWH develop age-related co-morbidities, including cancer and cardiovascular disease. The latter (particularly coronary artery disease and atrial fibrillation) represent a new challenge for the hemophilia treatment centers because their management implies a delicate balance between the thrombotic risk and ble

Invited Commentary: The Winding Road to Identifying the Annual Rate of Tuberculosis Infection.

Many persons with immunological tests indicating Mycobacterium tuberculosis infection, such as tuberculin skin tests or interferon-γ release assays, are at risk of progression to tuberculosis disease. Persons whose tests revert to negative may no longer be at such risk. Therefore, identifying the rate of test reversion, potentially indicating cure of M. tuberculosis infection, is an important area of investigation. In their accompanying article (Am J Epidemiol. 2023;192(12):1937-1943), Schwalb et al. extract data on test reversion from prechemotherapy literature and construct a model to predic

Feasibility and Efficacy of Online Neuropsychological Assessment.

Neuropsychological testing has intrinsic challenges, including the recruitment of patients and their participation in research projects. To create a method capable of collecting multiple datapoints (across domains and participants) while imposing low demands on the patients, we have developed PONT (Protocol for Online Neuropsychological Testing). Using this platform, we recruited neurotypical controls, individuals with Parkinson's disease, and individuals with cerebellar ataxia and tested their cognitive status, motor symptoms, emotional well-being, social support, and personality traits. Voice-specific proper name anomia ('phonoanomia') after bilateral temporal hemorrhagic brain lesions.

We describe a patient who developed bilateral temporal lesions due to a haemorrhagic stroke. One year after onset, he showed marked apperceptive prosopagnosia and could not name people from their voices, although he had spared semantic knowledge about people he could not name and could access this knowledge from people's voice. Importantly, the patient was able to retrieve proper name of persons after their description and was able to name sounds and songs; he could also retrieve proper names of monuments, towns and cartoon characters upon visual presentation. Dissociation and psychopathology in Kenyan survivors of a terrorist bombing.

BACKGROUND: Dissociation is a serious psychological condition that is characterized as a pathological outcome of trauma-related experience. Thus, dissociation could be expected to develop in survivors of disaster trauma and to be associated with trauma exposure and psychopathology. METHODS: A sample of 278 disaster-affected Kenyans was assessed 8 to 10 months after the 1998 terrorist bombing of the US Embassy in Nairobi for a study of trauma-related psychopathology and dissociation in the context of personality and culture. Instruments of assessment were the Diagnostic Interview Schedule, the

Monkeypox Virus Infections in Humans.

Human monkeypox is a viral zoonosis endemic to West and Central Africa that has recently generated increased interest and concern on a global scale as an emerging infectious disease threat in the midst of the slowly relenting COVID-2019 disease pandemic. The hallmark of infection is the development of a flu-like prodrome followed by the appearance of a smallpox-like exanthem. Precipitous person-to-person transmission of the virus among residents of 100 countries where it is nonendemic has motivated the immediate and widespread implementation of public health countermeasures. In this review, we
