Infection point of care tests bridge the experience gap of antibiotic STOP decisions: clinicians versus students.

BACKGROUND: Point of care tests (POCTs) offer accurate rapid diagnostics for infection, but not reduced antibiotic overuse in antibiotic stewardship (ABS) studies. Prescribing behaviour shaped by clinical experience may influence antibiotic decisions more than test performance. Understanding prescribing behavioural differences may inform ABS education. OBJECTIVES: To find out whether antibiotic decision making differ among medical students and intensive care clinicians when offered POCT use. METHODS: An observational study depicted four simulated clinical vignettes of hospital acquired pneum

Trends in Prolapse Repair Approaches Before and After Ban of Transvaginal Mesh.

INTRODUCTION: The introduction of transvaginal mesh (TVM) in 2002 for pelvic organ prolapse (POP) repair gained initial popularity until official warnings were released about its associated complications. This study compares the changes in practice patterns between Urogynecology and Reconstructive Pelvic Surgery (URPS)-trained and non-URPS-trained urologists following the warnings and eventual ban of TVM for prolapse repair. METHODS: Case logs from 2001 to 2022 were queried from the American Board of Urology. POP repair cases were identified by their Current Procedural Terminology codes and c

Implementation of two non-drug pain prescriptions for musculoskeletal pain in primary care clinics.

BACKGROUND: Chronic pain affects over 50 million Americans and is the main reason for primary care consultations. Clinical practice guidelines recommend use of non-drug therapies as first-line treatments. However, adoption and implementation of these guidelines are hindered by logistics. Exercise and transcutaneous electrical nerve stimulation (TENS) are effective non-drug strategies for managing chronic musculoskeletal pain, both of which have shown to reduce opioid usage. This study presents the results of implementing the Bundle for Exercises and TENS (BEsT) in primary care to facilitate pr

Designed for simplicity, used for complexity: The systemic pressures shaping walk-in clinic practices and outcomes.

Walk-in clinics (WICs), appreciated for their accessibility and convenience, have become an increasingly popular healthcare option in Ontario for patients with and without primary care enrolment. Despite their utility, WICs face criticism for delivering lower-quality care compared to comprehensive, enrolment-based primary care models. Critics argue that WICs contribute to system inefficiencies and encourage practice patterns misaligned with population health goals. This study explored physician perspectives on two key outcomes often associated with low-quality care in WICs: repeat primary care
