Pinch-off syndrome-related port catheter migration requiring surgical extraction combined with coronary artery bypass grafting.

Pinch-off syndrome is a rare but potentially serious complication of subclavian venous port systems, resulting from mechanical compression between the clavicle and first rib and leading to catheter fracture and embolization. A 63-year-old man with lung cancer presented with a fractured port catheter that had migrated into the left main and lower lobe pulmonary arteries. Endovascular retrieval was attempted but was unsuccessful due to firm endothelial adhesion. Preoperative coronary angiography revealed concomitant significant coronary artery disease. The patient subsequently underwent successf

Robot-assisted surgery for combined mini-invasive hemivertebrectomy with thoracotomy for en bloc resection of a paravertebral primary thoraco-lumbar tumor: technical note.

Primary bone tumors are quite rare spine neoplasms. When they are isolated, to prevent neurological impairment and recurrence, the best treatment is en-bloc negative margins resection. These procedures are often challenging, and technical improvements such as robotics can help surgeons to achieve their goals. We report here the case of a young adult who presented with an inflammatory myofibroblastic tumor (IMT), arising from the rib cage, and involving T12 vertebral body. An en-bloc resection, involving the mass and a right hemi-vertebrectomy was decided in a tumor board. A two- step surgery w

A Case of Obstructive Shock and Respiratory Failure Caused by Chest Wall Hematoma.

A woman in her 70s undergoing dialysis developed respiratory failure and acute circulatory failure accompanied by bleeding after cardiac surgery. A large chest wall hematoma was found in the right anterior chest, and the CT scan showed that the hematoma was compressing the right thoracic cage. After emergency surgery to stop the bleeding and remove the hematoma, the patient's respiratory and hemodynamic status improved immediately. In this case, the elderly dialysis patient had undergone a sternotomy, which made the thoracic cage more susceptible to external compression and reduced its ability

Uterine rupture following motor vehicle collision: possible seatbelt induced lesions.

Road traffic accidents rank among the leading non-obstetric causes of maternal and fetal death. We present a case of a motor vehicle collision involving a pregnant woman at term. In the initial evaluation, the patient was haemodynamically unstable and had an extensive abdominal haematoma. Abdominal ultrasound revealed fetal death and a significant quantity of intra-abdominal free fluid. An emergent exploratory laparotomy was performed, confirming a transverse uterine rupture, significant haemoperitoneum, and a dead fetus in the peritoneal cavity. Additional injuries included right infundibulop

Contemporary Treatment of Microtia-Atresia.

Microtia-atresia is a congenital deformity affecting the external ear and ear canal that can present with varying degrees of severity and morbidity. Treatment occurs along a spectrum and primarily centers on improving aesthetic appearance. Many cases of microtia will not be effectively treated with conservative measures and will require some form of reconstruction. There are several options available, including porous polyethylene implants, autologous rib grafting, and autologous chondrocyte frameworks. Equally significant is the treatment of hearing loss, as many patients with microtia-atresi

Thoracic splenosis: Precision medicine can prevent thoracic surgery.

Thoracic splenosis is a rare condition referring to the auto-transplantation of splenic tissue into the thoracic cavity following splenic trauma. We present a case of thoracic splenosis in a 62-year-old man who at the age of 17 suffered three gunshot wounds to the posterior thorax and abdomen, requiring a splenectomy and intercostal catheter insertion. In 2007, he underwent a thoracotomy and biopsy of a left sided pulmonary mass which was complicated by a haemothorax requiring an emergent return to theatre and rib resection to achieve haemostasis. The biopsy revealed granulation tissue. In 202

How I do it. Thoracic outlet syndrome and the transaxillary approach.

Thoracic outlet syndrome (TOS) is a disease pattern that involves compression of neurologic venous or arterial structures as they pass through the thoracic outlet. TOS was first described as a vascular complication arising from the presence of a cervical rib. Over time, a better understanding of TOS has led to its wide range of presenting symptoms being divided into three distinct groups: arterial, venous, and neurogenic. Of the known cases, the current estimates of the incidence of neurogenic TOS, venous TOS, and arterial TOS are 95%, 3%, and 1%, respectively. The different types of TOS have

Massive hemothorax secondary to internal jugular vein central venous catheter placement in a patient undergoing spinal surgery complicated by chest trauma: a case report.

BACKGROUND: Placement of a central venous catheter (CVC) is a common procedure for spinal surgery and is relatively safe under ultrasound guidance. CASE PRESENTATION: We report the case of a 56-year-old female who underwent ultrasound-guided placement of an internal jugular vein CVC for fluid replacement during spinal surgery for thoracic vertebral burst compression fracture and multiple rib fractures as a result of a high-altitude fall injury. Hemothorax developed intraoperatively. During a thoracotomy, the tip of the CVC was found within the chest cavity. The presence of chest trauma may im

[Functional disorders of the nose - definition, diagnostics and treatment options].

The surgical spectrum of functional rhinosurgery includes nasal septum surgery, septorhinoplasty and nasal concha surgery. Based on the German guideline "Disorders of the inner and/or external nose (with functional and/or aesthetic impairment)" published in April 2022, which was prepared on behalf of the German Society of Otorhinolaryngology, Head and Neck Surgery, we discuss the indications, diagnostic approaches, the planning of the surgery and postoperative care. The most common findings of the external nose with a functional impairment include crooked nose, saddle nose, and tension nose. C

Technical tips in non-robotic endoscopic mitral valve surgery: How to approach and expose the mitral valve.

The non-robotic endoscopic mitral valve surgery is performed via a right mini-thoracotomy without the use of rib spreader or robotic assistance. Although it has several advantages over the direct vision minimally invasive and robotic approaches, operating in very limited working space without robotic assistance is technically demanding. In the first place, it is impossible to perform non-robotic endoscopic mitral valve surgery safely and efficiently without knowing how to approach and expose the mitral valve in very limited working space without robotic assistance. This manuscript introduces d

[Pulmonary Hematoma Likely due to a Contralateral Chest Injury, Requiring Differentiation from Lung Tumor:Report of a Case].

A 23-year-old man was referred to our department for chest abnormal shadow. Computed tomography (CT) of the chest revealed a well-defined 1.2 cm nodule in the S4 segment of the right middle lobe and a well-defined 1.8 cm nodule in the S10 segment of the right lower lobe. The patient was found to have a fracture in the left fifth rib due to the falling accident at playing snowboard, two months before. He was diagnosed with a benign tumor in the right middle lobe and the right lower lobe and was performed surgery. Thoracoscopy revealed a yellowish-brown tumor in the right middle lobe( S4) and a

The Role of Ultrasound in Venous Thoracic Outlet Syndrome: Lesson Based on a Case Report.

Thoracic outlet syndrome is a rare neurovascular disease, usually caused by compression of subclavian vessels and/or brachial plexus by a cervical rib or hypertrophic scalene muscles. When the subclavian artery is compressed, it can cause perfusion deficiency with cyanosis and greater fatigue of the upper limb; if the subclavian vein is compressed, it can cause venous congestion with swelling and pain in the upper limb. When compression affects the brachial plexus, it can cause tingling or paresthesia of the upper limb. It can be mono or bilateral, congenital, or acquired. The diagnosis of vas

Use of the extended pectoralis major myocutaneous flap as a wrap-around for mediastinal tracheal repair.

Persistent tracheoesophageal fistulas requiring flap repair of the trachea in the mediastinum are uncommon. A 44-year-old man developed a 5 cm x 2 cm persistent posterior tracheal defect after failed tracheo-esophageal fistula repair. The defect was patch repaired using the de-epithelialized skin paddle of an extended pectoralis major flap tunneled into the mediastinum. The use of cross-field ventilation, second rib removal and an anterior tracheostomy slit were crucial for posterior tracheal repair. No major complication occurred. The patch repair with the de-epithelialized skin paddle of pec

Clinical report of a brain magnetic resonance imaging finding in Noonan syndrome.

Noonan syndrome (NS) is an autosomal dominant disease caused by aberrant up-regulated signaling through RAS GTPase. It is characterized by facial dysmorphisms, short stature, congenital heart defects, malformations of rib cage bones, bleeding problems, learning difficulties, or mild intellectual disability. Additional intracranial findings in NS patients include tumors, midline anomalies, and malformations of cortical development. In this report, we present the case of a young female patient, with a known diagnosis of Noonan syndrome that in complete well being developed two brain lesions, in

Spot the Pneumothorax.

Subcutaneous emphysema (SE) and pneumomediastinum are commonly associated with critically ill patients with blunt or penetrating trauma, in particular lower rib fractures. It however rarely needs urgent intervention, and routine use of chest tube tracheostomy or mediastinal drains is not recommended as the patients do not go on to develop a respiratory compromise. Our case is novel as it describes a case of subcutaneous emphysema with acute upper airway compromise and respiratory distress requiring urgent bilateral wide bore subcutaneous drains and thoracic drain insertion. The patient require

BCR/ABL1-positive B-lymphoblastic Lymphoma Successfully Treated with Dasatinib-combined Chemotherapy.

We herein report a rare case of BCR-ABL1-positive B-lymphoblastic lymphoma (B-LBL). An 18-year-old woman had a history of persistent left-sided chest pain. Positron emission tomography showed increased metabolic activity in the fifth rib, duodenum, and pancreas. The pathological findings of the pancreas, duodenum, and bone marrow confirmed the diagnosis of B-LBL. Polymerase chain reaction of duodenum and bone marrow also revealed a minor BCR-ABL1 fusion gene. She was diagnosed with BCR-ABL1-positive B-LBL and administered dasatinib and prednisolone. She achieved complete remission two weeks af
