A subscapularis-preserving arthroscopic release of capsule in the treatment of internal rotation contracture of shoulder in Erb's palsy (SPARC procedure).

The purpose of this study was to evaluate a minimally invasive subscapularis-preserving arthroscopic release of capsule in the treatment of internal rotation contracture of the shoulder due to Erb's palsy. We performed our procedure (subscapularis-preserving arthroscopic release of capsule) in 10 paediatric shoulders with an average age of 20.2 months and followed them for an average period of 41.5 months. All the patients were assessed clinically and radiologically preoperatively and postoperatively at regular intervals. The Mallet scoring system was used for analysing the results. The averag

Can CT angiography replace conventional bi-planar angiography in the management of severe scapulothoracic dissociation injuries?

Severe scapulothoracic dissociation (SSTD) (Type III or IV; Zelle classification) is often life-threatening and is commonly associated with other devastating injuries. Rapid evaluation, including of the vascular system, is critical to limit the time to definitive therapy. CT angiography (CTA) has evolved as a diagnostic tool, replacing angiography (angio) as it can simultaneously evaluate bony, soft tissue, and vascular injuries. We hypothesized that CTA would be useful in evaluating patients with SSTD. We retrospectively reviewed the trauma registry between June 2002 and June 2010 to identify

Biomechanical evaluation of shear force vectors leading to injury of the biceps reflection pulley: a biplane fluoroscopy study on cadaveric shoulders.

BACKGROUND: The clinical importance of the biceps reflection pulley (BRP), which stabilizes the long head of the biceps tendon (LHB) as it exits the joint, has been shown. However, there is controversy on the pathomechanism of injury to the BRP. The angular orientation of the LHB relative to its origin and distal course changes with joint positions and may place the BRP at risk for injury. PURPOSE: To measure the course of the LHB in common arm positions and to determine the shear and normal (stabilizing) force vectors as well as the excursion of the LHB. STUDY DESIGN: Descriptive laboratory

Arthroscopic Bankart repair: results and risk factors of recurrence of instability.

The purpose of this study was to discover the recurrence rate of instability after arthroscopic Bankart repair and to assess risk factors for recurrence, with special interest in bone defects seen on preoperative plain radiographs. Consecutive unselected patients (182; 186 shoulders) with recurrent instability (redislocation or subluxation) after initial traumatic anteroinferior shoulder dislocation were operated on using arthroscopic suture-anchor repair at our institution during 2000-2005. Demographic data and details of the operation were retrospectively reviewed from case records, and glen

Effect of humeral component version on impingement in reverse total shoulder arthroplasty.

HYPOTHESIS: Reverse shoulder arthroplasty is growing in popularity for patients with deficient rotator cuffs; however, the phenomenon of scapular notching continues to be a concern. This study examined the effects of humeral component version in the Aequalis Reversed Shoulder Prosthesis (Tornier, Edina, MN) on impingement of the humeral prosthesis against the scapula to test the hypothesis that the mechanical contact of the humeral component with the scapular neck is influenced by the version of the humeral component. MATERIALS AND METHODS: Seven shoulders from deceased donors were tested aft

Variations of the intra-articular portion of the long head of the biceps tendon: a classification of embryologically explained variations.

BACKGROUND: Although the intra-articular portion of the long head of the biceps (LHB) usually runs free, different types of fusions with the inferior surface of the capsule are known to be possible. Anatomic variations of this part of the LHB have been previously described and were nearly always considered to be innocent. MATERIALS AND METHODS: Out of 2 populations of 1500 arthroscopies each, we collected prospectively and retrospectively all possible variations of the proximal portion of the LHB. RESULTS: We included 57 cases (1.91%) of this total population in an attempt to describe the co

Physical therapist management following rotator cuff repair for a patient with postpolio syndrome.

BACKGROUND AND PURPOSE: Postpoliomyelitis sequelae, such as gait instability and progressive weakness, predispose people with postpolio syndrome to secondary disabilities. With aging, people who depend on their upper extremities to accommodate lower-extremity deficits may anticipate overuse injuries. The purpose of this case report is to describe the use of mobilization and exercise in postoperative rehabilitation of rotator cuff surgery on a patient with postpolio syndrome. CASE DESCRIPTION: A 48-year-old woman with postpolio syndrome had rotator cuff surgical repair followed by physical the

Treatment of anterior fracture-dislocations of the proximal humerus by open reduction and internal fixation.

Over a seven-year period we treated a consecutive series of 58 patients, 20 men and 38 women with a mean age of 66 years (21 to 87) who had an acute complex anterior fracture-dislocation of the proximal humerus. Two patterns of injury are proposed for study based upon a prospective assessment of the pattern of soft-tissue and bony injury and the degree of devascularisation of the humeral head. In 23 patients, the head had retained capsular attachments and arterial back-bleeding (type-I injury), whereas in 35 patients the head was devoid of significant soft-tissue attachments with no active art

Shoulder impingement in front-crawl swimming: II. Analysis of stroking technique.

PURPOSE: The purpose of this study was to determine the technical causes of shoulder impingement experienced by front-crawl swimmers. METHODS: The shoulder movements exhibited during performance of the front-crawl stroke were measured using three-dimensional videography, and the instances at which each shoulder was experiencing impingement were identified. RESULTS: On average, impingement occurred 24.8% of the stroke time (%ST). In one or more phases of the stroke cycle, each subject experienced impingement in some trials and not in other trials. This suggests that stroke technique, and not

Arthroscopic transglenoid stabilization versus open anchor suturing in traumatic anterior instability of the shoulder.

Sixty-two consecutive patients with recurrent traumatic anterior instability of the shoulder were prospectively observed. Thirty patients were observed after arthroscopic stabilization, and 32 were observed after open Bankart repair during a mean follow-up of 36 and 40 months, respectively (range, 24 to 60 months for both groups). To reattach the labrum, the arthroscopic technique used transglenoid sutures and the open technique used bone anchors. Redislocation occurred in two patients (6%) in the open repair group and in five patients (17%) in the arthroscopic repair group. Three of the five

Arthroscopic therapy of recurrent anterior luxation of the shoulder by capsular repair.

Sixty-five cases of recurrent anterior unidirectional dislocation of the shoulder were treated by arthroscopic capsular repair. In this approach, the arthroscope is inserted through a posterior portal. Using mostly an anterior portal, a 5-mm hole is drilled across the neck of the glenoid, between the 3 and 4 o'clock positions. A 5-mm working cannula is passed from the posterior portal into the joint. Mostly through the anterior portal, four to six retention U-sutures are placed in the labrum, the middle and inferior glenohumeral ligament, and the subscapularis tendon. A 25-cm-long, 2.0-mm K-wi

Postoperative bisphosphonate therapy is associated with lower prosthesis-related complications after total or reverse shoulder arthroplasty in osteoporotic patients: a TriNetX-based cohort study.

OBJECTIVES: Osteoporosis is a prevalent comorbidity in patients undergoing total or reverse shoulder arthroplasty and is associated with higher rates of periprosthetic fracture, implant loosening, and revision surgery. While bisphosphonates are commonly used to treat osteoporosis, prior research has primarily examined preoperative use. This study aimed to evaluate the association between postoperative bisphosphonate initiation and implant-related complications, particularly revision surgery, in osteoporotic patients undergoing shoulder arthroplasty. DESIGN: We used the TriNetX U.S. Collaborat

Multiple sclerosis is associated with increased shoulder dislocations and aseptic loosening following reverse shoulder arthroplasty: a propensity matched analysis.

INTRODUCTION: Multiple sclerosis (MS) is a chronic neurological condition characterized by muscle spasticity, which may influence the outcomes of reverse shoulder arthroplasty (RSA) for glenohumeral osteoarthritis or proximal humerus fractures. It is unclear whether implant complications and revision rates are increased in patients with MS. The aim was to compare 2-year implant complications and 90- day costs in MS patients undergoing RSA. METHODS: A retrospective analysis was performed using the PearlDiver database (2010-2022). MS patients (N = 764) who underwent RSA either for glenohumeral

Similar short-term outcomes of ultrasound guided posterior intra-articular and anterior extra-articular corticosteroid injection in the treatment of primary frozen shoulder. A randomized controlled, single-blind study.

Frozen shoulder, a condition characterized by the limitation of active and passive shoulder movements, is predominantly managed conservatively. Corticosteroid injection stands as a widely utilized and effective method for treatment. Nevertheless, the optimal approach for administering the injection remains a subject of controversy. We hypothesized that the injection of corticosteroid through anterior into the extra-articular area would yield superior results than the posterior intra-articular injection. This prospective, randomized, single-blind, and single-center clinical study aimed to compa
