Nocturnal sleep architecture disturbances in early methadone treatment patients.

The subjective and objective sleep patterns of patients with opioid dependence have been previously reported, but the sleep characteristics of patients in early methadone treatment, especially the objective sleep patterns, remain largely unexamined. This study was designed to explore the nocturnal sleep structure of patients on early methadone treatment. Twenty male methadone treatment (MT) patients and 20 male age- and body mass index-matched controls were assessed with overnight limited polysomnography. Subjective sleep was assessed using the Pittsburgh Sleep Quality Index (PSQI) and the Epw…

Characterization of sleep disturbance in Cornelia de Lange Syndrome.

OBJECTIVE: Prior studies have suggested that sleep disturbance is common in Cornelia de Lange Syndrome (CdLS); however, the nature of this sleep disturbance has not been well characterized. In this study, we evaluate the prevalence of sleep disordered breathing (SDB) and sleepiness in children and young adults with CdLS. METHODS: Caregivers of 22 patients with CdLS completed 3 validated Pediatric Sleep Questionnaires: the Pediatric Sleep Questionnaire (PSQ), Pediatric Daytime Sleepiness Scale (PDSS), and OSA18. RESULTS: Both measures of SDB (OSA18 and PSQ) suggest that 35-36% of these patien…

Is 'MILD' sleep-disordered breathing in women really mild?

Sleep-disordered breathing (SDB) consists of episodes of periodic obstructive or central sleep apnea and partial upper airway obstruction. The first two are well recognized and diagnosed, although still underdiagnosed. Traditionally, research in SDB has focused mainly on male patients with obstructive sleep apnea using apnea/hypopnea index (AHI) as a measure of severity. This has led to overrating of AHI as the only marker of SDB and underestimating of SDB in women. However, recently, partial upper airway obstruction has been acknowledged to be pathological and it may cause symptoms of SDB. Th…

Sleep-related disordered breathing in children with syndromic craniosynostosis.

BACKGROUND: Syndromic craniosynostosis patients are at risk for sleep-related disordered breathing (SRDB) but the role of polysomnography (PSG) in assessing these patients has not been fully explored. Our aim was to evaluate the prevalence or severity of SRDB in children with syndromic craniosynostosis or the impact of treatments on their SRDB. METHODS: We conducted a retrospective review of all patients with syndromic craniosynostosis referred between 1996 or 2008 for an initial PSG to rule out SRDB. For those with SRDB, we reviewed the interventions post PSG. RESULTS: 35 patients (18 femal…

Sleep patterns in Parkinson's disease patients with visual hallucinations.

Visual hallucinations (VHs) in Parkinson's disease (PD) can be a frequent and disturbing complication of the disease with 33% of PD patients undergoing long-term treatment experiencing VHs during the course of their illness. One line of evidence that is emerging as a possible risk factor in the occurrence of VHs is the sleep-wake cycle and sleep behavior in patients with PD. This study compared sleep patterns in a group of visually hallucinating Parkinson's patients with a group of nonhallucinating PD patients and an age-matched control group. Nocturnal sleep was assessed by actigraphy and dia…

Obstructive sleep apnea syndrome (OSAS) in mouth breathing children.

UNLABELLED: It is well known that mouth breathing is associated with adenotonsillar hypertrophy - which is the main cause of obstructive sleep apnea among children. Despite the importance of this matter, there are only a handful of studies showing the relationship between OSAS and mouth breathing. AIM: to determine the prevalence of obstructive sleep disorders in mouth breathing children and study its correlation with otorhinolaryngological findings. STUDY DESIGN: Retrospective cohort study. METHOD: Data analysis from 248 medical charts of mouth breathing children seen at the Pediatric Otol…

Sleep apnoeas and neurobehavioral effects in solvent exposed workers.

BACKGROUND: Exposure to organic solvents may cause an increase of sleep apnoeas, which may explain the excess of fatigue, concentration and memory problems reported in exposed workers. METHODS: Polysomnography was performed in 21 long-term exposed printers and 27 controls. In addition, a questionnaire regarding sleep related complaints, Q16 questionnaire and computerized neurobehavioral tests were administered. The groups matched well regarding age, weight, neck circumference and schooling level. A semi-quantitative cumulative exposure index was calculated. RESULTS: Excessive sleepiness whil…

Positive occipital sharp transients of sleep (POSTS): a reappraisal.

OBJECTIVE: Positive occipital sharp transients of sleep (POSTS) are considered a normal variant seen in non-REM sleep; their asymmetrical presentation and relationship with EEG abnormalities have received scarce attention to date. We analyzed these features in a large prospective EEG recordings' sample. METHODS: In this case-control study, over 6 months we collected consecutive patients showing POSTS on their EEG. They were matched with consecutive control subjects (two for each). Demographical data, asymmetries for POSTS and alpha activity, and lateralized or diffuse occurrence of EEG abnorm…

Atrial overdrive pacing in sleep apnoea: a meta-analysis.

AIMS: Sleep apnoea (SA) is a common breathing disorder that affects 5% of the North American adult population. It has been suggested that suppressing periods of bradycardia associated with apnoea may reduce the autonomic imbalance associated with SA, thereby improving the respiratory condition. The goals of this study were to conduct a systematic review to identify all randomized clinical trial data evaluating atrial overdrive pacing (AOP) for the treatment of SA and to perform a meta-analysis to estimate the true effect of AOP on SA. METHODS AND RESULTS: A systematic review of the literature…

Alterations in sleep EEG activity during the hypopnoea episodes.

The Obstructive Sleep Apnoea Hypopnoea Syndrome (OSAH) means "cessation of breath" during the sleep hours and the sufferers often experience related changes in the electrical activity of the brain and heart. The aim of this paper is to investigate any possible changes in the human electroencephalographic (EEG) activity due to hypopnoea (mild case of cessation of breath) occurrences by applying the non-linear and linear time series methods. The results from this study indicated significant changes in the human EEG activity due to hypopnoea episodes by applying the non-linear, Lyapunov exponent …

Sleep deprivation attenuates inflammatory responses and ischemic cell death.

Although the biological function of sleep remains uncertain, the consequences of sleep deprivation are well-described and are reported to be detrimental to cognitive function and affective well-being. Sleep deprivation also is strongly associated with elevated risk factors for cardiovascular disease. We used a mouse model of cardiac arrest/cardiopulmonary resuscitation to test the hypothesis that acute sleep deprivation would exacerbate neuroinflammation and neurodegeneration after global ischemia. The resulting data led to a rejection of our hypothesis that sleep deprivation is necessarily de…

Triggering of nocturnal arrhythmias by sleep-disordered breathing events.

OBJECTIVES: This study sought to evaluate respiratory disturbances as potential triggers for arrhythmia in patients with sleep-disordered breathing (SDB). BACKGROUND: SDB is associated with an increased risk of atrial fibrillation and nonsustained ventricular tachycardia (NSVT) as well as a predilection for sudden cardiac death during nocturnal sleeping hours. However, prior research has not established whether respiratory disturbances operate as triggers for nocturnal arrhythmias. METHODS: Overnight polysomnograms from the Sleep Heart Health Study (n = 2,816) were screened for paroxysmal at…

Severe obstructive sleep apnea syndrome with symptomatic daytime bradyarrhythmia.

We report a case of severe obstructive sleep apnea (OSA) in a patient with symptomatic daytime cardiac bradyarrhythmia. Continuous positive airway pressure therapy prevented atrioventricular blocks that emerged after cardiac pacing for sick sinus syndrome. OSA could be associated with daytime bradyarrhythmia.

Sleep enhances false memories depending on general memory performance.

Memory is subject to dynamic changes, sometimes giving rise to the formation of false memories due to biased processes of consolidation or retrieval. Sleep is known to benefit memory consolidation through an active reorganization of representations whereas acute sleep deprivation impairs retrieval functions. Here, we investigated whether sleep after learning and sleep deprivation at retrieval enhance the generation of false memories in a free recall test. According to the Deese, Roediger, McDermott (DRM) false memory paradigm, subjects learned lists of semantically associated words (e.g., "nig…

Sleepiness and sleep-related accidents in commercial bus drivers.

INTRODUCTION: Professional drivers are at high risk of sleepiness due to a combination of factors including shift work and obstructive sleep apnea/hypopnea syndrome (OSAHS), and sleepiness in professional drivers is highly dangerous. This study aimed to determine the prevalence of excessive daytime sleepiness and accident rates in bus drivers. MATERIALS AND METHODS: Drivers employed at bus depots within 30 miles of Edinburgh were given a sleep questionnaire. One thousand eight hundred fifty-four drivers were approached, and 677 (37%; 25 female) completed questionnaires with a 97% response rat…

Clinical foundation for efficient treatment of obstructive sleep apnea.

Only a few decades ago, the entity known as obstructive sleep apnea (OSA) was unknown and untreated. Now, there is a rush to put literally millions of Americans on continuous positive airway pressure devices. Community practice standards are changing yearly under pressure from strong forces based on economic incentives for industry, government, and physicians, independent of the actual medical evidence supporting treatment and efficacy. Medicare has lowered the diagnostic threshold for diagnosis and reimbursement; the International Classification of Sleep Disorders, Revision 2 (2005) has allow…

Optimal treatment of obstructive sleep apnea and excessive sleepiness.

INTRODUCTION: Collapsibility of the upper airway in obstructive sleep apnea (OSA) causes repeated arousals from sleep, decreased oxygen saturation of the blood, and excessive sleepiness (ES). Patients with OSA are at increased risk of cardiovascular and cerebrovascular disease, and experience occupational and vehicular accidents more frequently than the general population. Furthermore, the life expectancy of patients with untreated OSA is significantly reduced. METHODS: A MEDLINE search of articles published between 2003 and 2008 was conducted using the search terms: obstructive sleep apnea […
