Oxygen overshoot burden of central sleep apnea and its association with cardiovascular outcomes.

PURPOSE: The role of oxidative stress in central sleep apnea (CSA) remains unknown. We examined whether "oxygen overshoot burden"-a measure of cumulative oxygen saturation levels above baseline-is associated with major adverse cardiovascular events (MACE) in CSA. METHODS: We analyzed 7530 participants from the Sleep Heart Health Study (SHHS; age 64 years; 47.0% male) and the Osteoporotic Fractures in Men Study (MrOS; age 76 years; 100% male). Oxygen overshoot burden was defined as the area under the SpO2 curve above a stable sleep baseline (wakefulness baseline was also considered). In primar

A new method for measuring daytime sleepiness: the Epworth sleepiness scale.

The development and use of a new scale, the Epworth sleepiness scale (ESS), is described. This is a simple, self-administered questionnaire which is shown to provide a measurement of the subject's general level of daytime sleepiness. One hundred and eighty adults answered the ESS, including 30 normal men and women as controls and 150 patients with a range of sleep disorders. They rated the chances that they would doze off or fall asleep when in eight different situations commonly encountered in daily life. Total ESS scores significantly distinguished normal subjects from patients in various di

Cardiorespiratory sleep studies for children can often be performed in the home.

We developed a portable recording system, suitable for unattended use in a patient's home, that quantitates the essential diagnostic elements of pediatric obstructive sleep apnea syndrome (OSAS): obstructive, mixed and central apneas and hyponeas; hemoglobin saturation, sleep vs. wakefulness; body and head positions; snoring: and sleep disturbance. The present paper reviews validation studies and summarizes two recent studies that demonstrate the unique advantages of performing clinical and research cardiorespiratory sleep studies in the child's home. Development of inexpensive, portable recor

Quantifying the sources of discrepancy between total recording time and total sleep time in home sleep apnea testing: insights from home-based polysomnography.

STUDY OBJECTIVES: To quantify the contribution of sleep onset latency (SOL), wake after sleep onset (WASO), and wake after sleep offset (WASF) to the discrepancy between total recording time (TRT) and total sleep time (TST) in home-based polysomnography (PSG) using patient-activated and deactivated monitoring devices. METHODS: This observational study enrolled patients with a high pretest probability of obstructive sleep apnea who underwent unattended home-based PSG. We measured the duration of SOL, WASO, and WASF to quantify the discrepancy between TRT and TST. TRT was defined as the interva

Recognizing Poor Sleep Quality Factors During Oral Health Evaluations.

Oral health practitioners routinely perform oral health assessments for the dental patient to determine if oral disease is present. Systemic health is often a contributor to oral health concerns. One area in particular that has a direct effect on oral structures and oral health is poor sleep quality and open mouth breathing. Sleep is a fundamental process of the human body, which regulates core biological functions. Sleep quality reflects a person's ability to fall asleep, stay asleep, and enter into the various rejuvenating sleep cycles for the full duration. A person who does not obtain qual

Sleep Disorders in the Allergy-Immunology Clinic.

The immunologic consequences of disordered sleep are evident in many disease states, including multiple allergic diseases and disorders cared for in the allergy-immunology clinic. Allergic rhinitis is significantly associated with disordered sleep, most commonly obstructive sleep apnea, which worsens the allergic process itself through chronic intermittent hypoxia and increased inflammatory activation. Patients with asthma also commonly suffer from obstructive sleep apnea, with both conditions worsening nighttime airway resistance. Disrupted sleep decreases the reaction threshold for patients

Impact of Upper Airway Collapse and Sleep Parameters on Daytime Sleepiness and Sleep Quality.

OBJECTIVE: Standard overnight polysomnography (PSG) scores only the apneahypopnea index (AHI) as a predictor of sleep apnea. It is proven that the arousals also contribute especially to the daytime symptoms of obstructive sleep apnea syndrome (OSA). The authors aimed to search the relevance of arousals and obstruction site in the upper airway with the sleep parameters, daytime sleepiness and the sleep quality. MATERIALS AND METHODS: A total number of 118 subjects were recruited. All of them received a full-night attended PSG and a thorough otolaryngologic examination to identify the site of o

The impact of autonomic nervous system dysfunction on breathing during sleep.

Ten patients with autonomic nervous system dysfunction (familial dysautonomia, juvenile diabetes, or Shy-Drager syndrome) were studied to assess the impact of their impairment on breathing during sleep. Several types of breathing dysfunction during sleep were identified independent of the patients' primary complaints. Obstructive sleep apnea syndrome was the most common; central sleep apnea and disturbances of te respiratory oscillator also were seen. Esophageal reflux was found to be the cause of some sleep-related problems. The observed respiratory irregularities were not associated with the

Central Sleep Apnea Detection by Means of Finger Photoplethysmography.

UNLABELLED: Obstructive Sleep Apnea (OSA) and Central Sleep Apnea (CSA) are two types of Sleep Apnea (SA) with different etiologies and treatment options. Home sleep apnea testing based on photoplethysmography-derived peripheral arterial tonometry (PAT HSAT) has become the most widely deployed outpatient SA diagnostic method. Being able to differentiate between CSA and OSA based solely on photoplethysmography-data would further increase PAT HSAT's clinical utility. The present work proposes a method to detect CSA using finger photoplethysmography (PPG) data and evaluates the proposed method ag

Temporomandibular Disorders Within the Context of Sleep Disorders.

This paper provided an overview of the knowledge on the relationship between temporomandibular disorders (TMDs) and the main sleep conditions and disorders of dental interest, namely, sleep bruxism (SB), sleep apnea, and gastroesophageal reflux disease (GERD). It emerged that although the topic of SB as a possible detrimental factor for the stomatognathic structures has been the most studied, evidence is growing that SB, obstructive sleep apnea, and GERD, all belong to a circle of mutually interacting sleep disorders and conditions that, in turn, may be associated with TMDs. The pathophysiolog

Lifetime Care of Duchenne Muscular Dystrophy.

Individuals with Duchenne muscular dystrophy (DMD) have evolving sleep and respiratory pathophysiology over their lifetimes. Across the lifespan of DMD, various sleep-related breathing disorders (SRBD) have been described, including obstructive sleep apnea, central sleep apnea, and nocturnal hypoventilation. In addition to SRBD, individuals with DMD can be affected by insomnia, chronic pain and other factors interfering with sleep quality, and daytime somnolence. The natural progression of DMD pathophysiology has changed with the introduction of therapies for downstream pathologic pathways and

Sleep-Disordered Breathing and Excessive Daytime Sleepiness.

Sleep-related breathing disorders include obstructive sleep apnea (OSA), central sleep apnea, sleep-related hypoventilation, and sleep-related hypoxemia. Excessive daytime sleepiness (EDS) is frequently reported by patients with OSA but is not invariably present. The efficacy of positive airway pressure therapy in improving EDS is well established for OSA, but effectiveness is limited by suboptimal adherence. Non-OSA causes of sleepiness should be identified and treated before initiating pharmacotherapy for persistent sleepiness despite adequately treated OSA. Further research on the identific

Sleep-disordered breathing in neurologic conditions.

Sleep-related breathing disorder or sleep-disordered breathing (SDB) encompasses central sleep apnea (CSA), obstructive sleep apnea (OSA), and sleep-related hypoventilation or hypoxemic syndromes. SDB is common in neurologic conditions that affect the central and/or peripheral nervous systems. Patients with neurologic conditions are at risk for SDB due to a combination of factors such as muscular weakness, damage to areas of the brain that control respiration, use of sedating medications, and weight gain from limited physical activity. This article discusses recognition and treatment of SDB as

Sleep disorders and sleep problems in childhood.

Sleep problems are common in childhood. A distinction is made between problems in which polysomnography is abnormal (i.e., the parasomnias, sleep apnea and narcolepsy) and problems that are behavioral in origin and have normal polysomnography. The parasomnias--sleep terrors, somnambulism and enuresis--appear to be related to central nervous system immaturity and are often outgrown. Obstructive sleep apnea syndrome (OSAS) is frequently missed in children and can often be cured through surgery. Behavioral sleep problems may be overcome after parents make interventions. Physicians can be of great

Long-Term Management of Sleep Apnea-Hypopnea Syndrome: Efficacy and Challenges of Continuous Positive Airway Pressure Therapy-A Narrative Review.

Sleep apnea-hypopnea syndrome (SAHS) is a respiratory disorder characterized by cessation of breathing during sleep, resulting in daytime somnolence and various comorbidities. SAHS encompasses obstructive sleep apnea (OSA), caused by upper airway obstruction, and central sleep apnea (CSA), resulting from lack of brainstem signaling for respiration. Continuous positive airway pressure (CPAP) therapy is the gold standard treatment for SAHS, reducing apnea and hypopnea episodes by providing continuous airflow. CPAP enhances sleep quality and improves overall health by reducing the risk of comorbi

Causal associations between sleep apnea and dementia: a two-sample Mendelian randomization study.

INTRODUCTION: Observational studies suggest an association between sleep apnea and dementia, but causality and directionality are unclear. This study investigated bidirectional causal relationships between sleep apnea and various dementia types using two-sample Mendelian randomization (MR). MATERIAL AND METHODS: This study used summary-level data from genome-wide association studies (GWAS). Sleep apnea (including obstructive sleep apnea (OSA) and generalized sleep apnea) exposure data were from a European ancestry study. Dementia (general, Alzheimer's, vascular, etc.) outcome/exposure data we

Impact of CPAP treatment for a short period in moderate-to-severe OSAS patients: a randomized double-blind clinical trial.

INTRODUCTION: Obesity is the most frequent reversible agravating factor of obstructive sleep apnea syndrome, with physical activity very important for its control. Continuous positive air pressure during sleep is the "gold standard" treatment for obstructive sleep apnea syndrome. OBJECTIVE: we aimed to investigate if the use of continuous positive air pressure for a short period (7 days), would improve sleep quality, daytime sleepiness, and the disposition for physical activity. METHODS: Eighty obstructive sleep apnea syndrome patients were randomly assigned as follows: group I - continuous

Validating respiratory index of auto-titrating positive airway pressure device with polysomnography.

PURPOSE: Positive airway pressure (PAP) devices have been widely used as the first line of treatment in obstructive sleep apnea (OSA). Most advanced PAP devices support the estimation of respiratory index (RI) using the patient's mask airflow. In addition to the compliance factor for PAP device use, which is important for monitoring patient sleep health, RI is also becoming important for monitoring. However, there are few reports that validate RI of a PAP device with polysomnography. METHODS: Between January 2015 and December 2017, 50 participants were enrolled who were diagnosed with OSA and

Nocturnal cluster headache associated with sleep apnea. A case report.

We describe a 49-year-old man with chronic cluster headache unresponsive to all medications. Following investigation in the sleep lab he was found to have obstructive sleep apnea (OSA) with associated oxygen desaturations during rapid eye movement (REM) sleep. He awakened during one of these episodes with a typical headache. Treatment with nasal CPAP abolished his OSA and desaturations, and largely abolished his headaches. He then developed central apneas during REM sleep. Further treatment with BiPAP, with a set backup rate, abolished both the apneas and the headaches. We conclude that there
