Auditory hallucinations in those populations that do not suffer from schizophrenia.

The following article discusses the phenomenon of auditory hallucinations in those who do not suffer from schizophrenia. Research has shown the occurrence of auditory hallucinations in the general population to such an extent that they cannot be said to be pathognomonic of psychiatric illness. In addition, it has long been known that certain hallucinatory experiences occur in health, such as hypnagogic and hypnopompic hallucinations. However, there are fundamental differences in the characteristics of these experiences. In the psychiatric population, these tend to be frequent, intrusive, and d

Pharmacotherapeutic treatment of nightmares and insomnia in posttraumatic stress disorder: an overview of the literature.

Sleep disturbances are hallmark symptoms of posttraumatic stress disorder (PTSD). Where the subjective experience of nightmares and insomnia in PTSD patients is very real indeed and demands treatment, objective research findings on disordered sleep architecture in PTSD are inconclusive and inconsistent. After reviewing the literature an insufficient number of controlled studies are published to formulate evidence-based guidelines. Several studies have methodological limitations, such as small group sizes and heterogenic samples. Large randomized controlled trials (RCTs) need to be conducted in

Role of corticotropin releasing factor in anxiety disorders: a translational research perspective.

Anxiety disorders are a group of mental disorders that include generalized anxiety disorder (GAD), panic disorder, phobic disorders (e.g., specific phobias, agoraphobia, social phobia) and posttraumatic stress disorder (PTSD). Anxiety disorders are among the most common of all mental disorders and, when coupled with an awareness of the disability and reduced quality of life they convey, they must be recognized as a serious public health problem. Over 20 years of preclinical studies point to a role for the CRF system in anxiety and stress responses. Clinical studies have supported a model of CR

Factors associated with insomnia among civilians seeking treatment for PTSD: an exploratory study.

This study explored relationships between insomnia (as measured by Item 13 of the Clinician-Administered PTSD [Posttraumatic Stress Disorder] Scale) and other PTSD symptoms, comorbidity, and measures of depression, worry, and panic in a mixed-trauma sample of 143 treatment-seeking civilians with PTSD (median age 39.4 years, 83% female, 96% White). Regression analyses showed that, adjusting for sleep medication use, severity of nightmares and diminished interest in pleasurable activities accounted for unique variance in insomnia severity level. Severity of nightmares and depression accounted fo

Forensic evaluation of trauma syndromes in children.

There are significant differences between a clinical evaluation and a forensic evaluation [289-291]. These differences must be kept solidly in mind in performing the evaluation. The forensic evaluator needs to assess the validity of complaints, including the possibility of malingering and the child's ability to describe symptoms accurately, the connection between the symptoms and a given incident, and the potential long-term sequelae of a trauma. The goal of the interview is not to treat, but to obtain information. Assessing the validity of complaints is perhaps the greatest challenge. This re

Television images and psychological symptoms after the September 11 terrorist attacks.

Exposure to graphic television images may exacerbate psychological symptoms in disaster situations. We tested the hypotheses that (1) more frequent viewing of television images of the September 11 terrorist attacks was associated with posttraumatic stress disorder (PTSD) and depression, and that (2) direct exposure to disaster events had an interactive effect with media viewing. We recruited 1,008 adult residents of the borough of Manhattan in New York City through a random-digit-dial telephone survey conducted between October 16 and November 15, 2001. Respondents who repeatedly saw "people fa

Treatment of chronic nightmares in adjudicated adolescent girls in a residential facility.

PURPOSE: To evaluate imagery rehearsal therapy for the treatment of chronic nightmares in a sample of adolescent girls. METHODS: Adolescent girls ranging in age from 13 to 18 years were recruited from the Wyoming Girls School in Sheridan, Wyoming (treatment group, n = 9; control group, n = 10). These girls had previously suffered a high prevalence of unwanted sexual experiences in childhood and adolescence, and thus many suffered from nightmares, sleep complaints, and posttraumatic stress symptoms. Imagery rehearsal therapy was provided in a 1-day (6-h) workshop. Imagery rehearsal consists of

Combat, dissociation, and posttraumatic stress disorder in Australian Vietnam veterans.

The specificity of various wartime stressors for different posttraumatic stress disorder (PTSD) symptoms is inconsistently reported in the literature. Combat, wounding, and peritraumatic dissociation have not been assessed together in their effects on each of the various PTSD symptom clusters. This cohort study of a random sample of male Australian Army Vietnam veterans yielded psychiatric assessments of 641 subjects. PTSD measures comprised symptom criteria for reexperiencing, numbing and avoidance, hyperarousal, and PTSD diagnosis both lifetime and current within the past month. Logistic reg

[Cerebral dysfunction occurring after years of latency].

A number of basically very different factors may affect the central nervous system and result in progressive loss of mental or physical functions immediately or after a latent period of decades, possibly due to the initial factor being combined with an additive effect of aging. Intoxication with 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP) induces degeneration of the basal ganglia. Various vira e.g. poliovirus and retrovirus may give rise to acute or late effects after several years. The Concentration Camp Syndrome may lead to a dementing illness with a latency of 10-30 years. Posttraumatic stress disorder in incarcerated juvenile delinquents.

OBJECTIVE: To assess the prevalence of posttraumatic stress disorder (PTSD) in severely delinquent subjects and to measure associated personality characteristics. METHOD: Eighty-five incarcerated boys (mean age 16.6, SD = 1.2) with mostly violent offenses were studied. The sample was representative of the California Youth Authority population. They received a standard psychiatric screen, a semistructured interview for PTSD, and self-report questionnaires measuring personality traits and defenses. A nonclinical sex- and age-matched group was used for comparing psychometrics. RESULTS: Subjects

Premilitary MMPI scores as predictors of combat-related PTSD symptoms.

OBJECTIVE: The authors used data collected before military service to assess predictors of combat-related lifetime symptoms of posttraumatic stress disorder (PTSD). METHOD: The subjects were 131 male Vietnam and Vietnam-era veterans who had taken the MMPI in college and who were interviewed as adults with the Structured Clinical Interview for DSM-III-R. Scores on the basic MMPI scales were used to predict combat exposure, lifetime history of any PTSD symptoms given exposure, and lifetime PTSD classification (symptoms only, subthreshold PTSD, or full PTSD). RESULTS: Group means on the MMPI sc

Catastrophic stress and factors affecting its consequences among Southeast Asian refugees.

Effects on mental health of the stress of being interned in a refugee camp were assessed in a community survey of 1348 Southeast Asians. The impact on depressive mood proved significant but short-lived. Social support derived from the ethnic community and from an intact marriage moderated the risk of developing depressive symptoms, apparently by enhancing a sense of identity and belongingness. A psychological coping mechanism--avoidance of the past--buffered the impact of camp stress on depressive symptoms. While refugees brought into the country under private sponsorship were expected to have

Characteristic expressions of combat-related posttraumatic stress disorder among Israeli soldiers in the 1982 Lebanon War.

This study assessed the clinical picture of two groups of Israeli soldiers: front-line soldiers who had been treated for combat stress reaction during the 1982 Lebanon War (n = 382); and matched control front-line soldiers who did not sustain combat stress reaction (n = 334). Subjects were screened one year after the war for posttraumatic stress disorder (PTSD) and psychiatric symptomatology using the symptom checklist-90 (SCL-90). Results indicated that anxiety, depression, hostility, and obsessive-compulsive problems were the most salient features of PTSD among combat stress reaction casualt

Auditory hallucinations in combat-related chronic posttraumatic stress disorder.

Intrusive images have been reported to occur in a broad range of people with posttraumatic stress disorder, but the frequency of intrusive auditory perceptions has rarely been addressed. This study compared five posttraumatic stress disorder veterans experiencing auditory hallucinations with 31 nonhallucinating veterans on demographic, military, postmilitary, and symptom variables. Veterans who reported auditory hallucinations had higher combat exposure and more intense posttraumatic stress disorder symptoms than the other veterans. These veterans also tended to be more refractory to treatment

Association between traumatic stress load, psychopathology, and cognition in the Philadelphia Neurodevelopmental Cohort.

BACKGROUND: Traumatic stressors during childhood and adolescence are associated with psychopathology, mostly studied in the context of post-traumatic stress disorder (PTSD) and depression. We investigated broader associations of traumatic stress exposure with psychopathology and cognition in a youth community sample. METHODS: The Philadelphia Neurodevelopmental Cohort (N = 9498) is an investigation of clinical and neurobehavioral phenotypes in a diverse (56% Caucasian, 33% African American, 11% other) US youth community population (aged 8-21). Participants were ascertained through children's

Parkinson's Disease in Times of War: Gaps in Knowledge and Emerging Controversies.

Parkinson's disease (PD), the second most common neurodegenerative disorder, is influenced by genetic susceptibility and environmental exposures. Conditions of war, including traumatic brain injury, chronic stress, toxin exposure, malnutrition, and healthcare disruption, may significantly affect both PD risk and management. Studies have linked traumatic brain injury, post-traumatic stress disorder, and prolonged psychological stress to increased PD risk, while wartime deprivation may serve as a long-latency factor, with disease onset occurring decades later. Management of PD during conflict pr

Dynamic time warping network analysis of posttraumatic stress symptoms in Japanese first responders.

Background: A more inclusive approach for addressing the diverse spectrum of mental health experiences is needed. Network theory frames psychopathological complaints as complex dynamic systems, that can be modelled in a network of mutually interacting symptoms. These networks may provide important markers of symptom improvement or exacerbation, and inform strategies for early intervention and treatment. In previous research, we identified five distinct Post-Traumatic Stress Disorder (PTSD) symptom severity trajectories within a cohort of first responders from the Japan Ground Self-Defense Forc

The neural circuits and molecular mechanisms underlying fear dysregulation in posttraumatic stress disorder.

Post-traumatic stress disorder (PTSD) is a stress-associated complex and debilitating psychiatric disorder due to an imbalance of neurotransmitters in response to traumatic events or fear. PTSD is characterized by re-experiencing, avoidance behavior, hyperarousal, negative emotions, insomnia, personality changes, and memory problems following exposure to severe trauma. However, the biological mechanisms and symptomatology underlying this disorder are still largely unknown or poorly understood. Considerable evidence shows that PTSD results from a dysfunction in highly conserved brain systems in
