Substance use, health status and service utilisation of older clients attending specialist drug and alcohol services.

INTRODUCTION AND AIMS: The number of older clients attending drug and alcohol (D&A) services is increasing, although there is insufficient knowledge regarding service needs for this group. The aim of this study was to document the patterns of substance use, health status, cognition, social conditions, and health service utilisation of older clients in D&A treatment. DESIGN AND METHODS: A cross-sectional observational study of 99 clients aged ≥50 years (M = 55, SD = 4.5; 77% male) attending specialist D&A services (N = 30 alcohol treatment, N = 69 opioid treatment) in Sydney, Australia. Role of the laboratory in the evaluation of suspected drug abuse.

Despite the high incidence of substance abuse, it remains a common cause of misdiagnosis. In patients who have abused or who are currently abusing drugs, symptoms of a psychiatric illness may be mimicked by either the drug's presence or absence. The laboratory can aid in making a differential diagnosis and eliminating drugs from active consideration as a cause of psychosis, depression, mania, and personality changes. Treatment planning and prevention of serious medical consequences often rest on the accuracy of the admission drug screen. Testing is widely used to assess improvement in substanc

"They've walked the path that they're wanting me to walk": Patient and stakeholder feedback on a peer-delivered behavioral activation intervention for polysubstance use and rural mobile telemedicine retention.

INTRODUCTION: Polysubstance use, particularly co-occurring opioid and stimulant use, disproportionately impacts rural communities. Our team has worked to address the inaccessibility of treatment in rural settings by expanding access to medication for opioid use disorder (MOUD) via telemedicine aboard a mobile treatment unit. Long-term retention in MOUD care remains a challenge, particularly for individuals with polysubstance use. Peer recovery specialists (PRSs) may be uniquely suited to support individuals with polysubstance use in MOUD care through their shared experience and ability to deli

Improvement in Substance Use and Quality of Life in Patients Engaged in a Peer Navigator Program for Opioid Use Disorder.

BACKGROUND: Opioid use disorder (OUD) is associated with high emergency department (ED) utilization, overdose, and unmet social needs. ED-based peer navigator (EDPN) programs may improve engagement in care and address social determinants of health, but data on their clinical and behavioral impact are limited. OBJECTIVE: To assess changes in substance use, quality of life, patient satisfaction, and healthcare utilization among patients enrolled in an EDPN program. METHODS: We conducted a retrospective review of self-reported data using the Substance Abuse and Mental Health Services Administra

National trends in co-use of opioids and methamphetamine among people who inject drugs, 2012-2018.

Reports from substance use disorder treatment admissions have described a trend of rapidly increasing overlapping methamphetamine and opioid use throughout the United States in the past twenty years. We used data from the 2012, 2015, and 2018 cycles of the National HIV Behavioral Surveillance (NHBS) project among people who inject drugs (PWID) to describe trends in methamphetamine-opioid co-use over time and in different US Census regions. We compared the demographic, socio-economic, sexual health, and drug use behavioral characteristics of people who co-used compared to people who primarily u

Naloxone access in perinatal substance use disorder.

BACKGROUND: Risk of fatal drug overdose is higher in pregnant and postpartum people with substance use disorder (SUD) than for nonpregnant women of reproductive age. It is recommended that naloxone is prescribed for pregnancies complicated by opioid or stimulant use disorder. OBJECTIVE: The purpose of this study was to assess the rates of naloxone coprescribing with buprenorphine in a perinatal SUD (PSUD) specialty clinic and identify opportunities for pharmacist-led interventions to improve communication and documentation surrounding naloxone access to achieve a rate of 100% coprescribing of

Associations between traumatic life events and substance use among Black Men who are incarcerated.

The current study examined associations between traumatic life events, current and lifetime importance of mental health and substance use treatment, and crack cocaine use among 201 Black men who were incarcerated and nearing community reentry. Results indicated age, sexual trauma and lifetime importance of drug treatment were significantly associated with an increased likelihood of crack cocaine use. Substance abuse treatment in correctional settings should consider culturally tailored assessment and treatment for history of unaddressed sexual trauma among Black men who are incarcerated and us

Rural houselessness among people who use drugs in the United States: Results from the National Rural Opioid Initiative.

BACKGROUND: Over the last two decades, houselessness and drug-related epidemics both have expanded from urban to rural regions across the United States (US). However, our understanding of the relationship between rural houselessness, drug use, and drug-related harms has not kept pace. The current study addresses this gap by describing houselessness among a large cohort of people who use drugs (PWUD) from rural communities across 10 states. DESIGN: PWUD were recruited using modified chain-referral sampling for a cross-sectional survey capturing houselessness in the prior six months, drug use,

Trends in opioid initiation among people who use opioids in three US cities.

INTRODUCTION AND AIMS: The increased availability of prescription opioids (PO) and non-medical prescription opioids (NMPO) has fundamentally altered drug markets and typical trajectories from initiation to high-risk use among people who use opioids (PWUO). This multi-site study explores trends in opioid initiation in three US cities and associations with sociodemographic factors, current drug use and overdose risk. DESIGN AND METHODS: We analysed survey data from a cross-sectional study of PWUO in Baltimore, Maryland (n = 173), Boston, Massachusetts (n = 80) and Providence, Rhode Island (n =

The management of opiate use disorders in France: results of an observational survey of general practitioners.

BACKGROUND: When opioid-agonist treatments were approved in France in 1995, opiate use disorders began to be managed and treated by general practitioners (GPs), who have since then been encouraged to treat substance use disorders (SUDs) for heroin and other illegal substances. The objective of this study was to describe rates of: 1) SUDs in general practices in France; 2) characteristics of GPs treating SUDs; and 3) clinical practices surrounding SUDs. To place these data in the context of SUD treatment, we also gathered information from practicing SUD specialists. METHODS: Between December 2

The temporal association between substance use and intimate partner violence among women arrested for domestic violence.

OBJECTIVE: There is a paucity of research on the temporal association between substance use and intimate partner violence (IPV) perpetration and victimization, especially among women arrested for domestic violence. The current study examined whether the probability of IPV perpetration and victimization increases following alcohol or drug use relative to days of no use among women arrested for domestic violence. METHOD: Women arrested for domestic violence and court referred to batterer intervention programs who met criteria for hazardous drinking participated in the current study (N = 105). W

Reliability of the Timeline Followback for cocaine, cannabis, and cigarette use.

The Timeline Followback (TLFB), a retrospective calendar-based measure of daily substance use, was initially developed to obtain self-reports of alcohol use. Since its inception it has undergone extensive evaluation across diverse populations and is considered the most psychometrically sound self-report measure of drinking. Although the TLFB has been extended to other behaviors, its psychometric evaluation with other addictive behaviors has not been as extensive as for alcohol use. The present study evaluated the test-retest reliability of the TLFB for cocaine, cannabis, and cigarette use for

A multi-level analysis of counselor attitudes toward the use of buprenorphine in substance abuse treatment.

Despite evidence that buprenorphine is effective and safe and offers greater access as compared with methadone, implementation for treatment of opiate dependence continues to be weak. Research indicates that legal and regulatory factors, state policies, and organizational and provider variables affect adoption of buprenorphine. This study uses hierarchical linear modeling to examine National Treatment Center Study data to identify counselor characteristics (attitudes, training, and beliefs) and organizational factors (accreditation, caseload, access to buprenorphine, and other evidence-based p

Examining the impact of parental risk on family functioning among homeless and housed families.

The present study used data from 132 families that were homeless and a comparison group of 434 housed families in order to compare family functioning across the two groups. Family functioning was assessed by family support workers when the families sought help from one of seven family service agencies in Washington, DC. Multiple regression models showed that there were few differences between homeless and housed families; a difference that did emerge showed that homeless families fared better than housed families in terms of children's developmental stimulation. However, when considering the c

Developmental antecedents of late adolescence substance use patterns.

This longitudinal study examined antecedents of substance use behavior among 176 (53% male) adolescents. Adolescents were classified as (a) abstainers (n = 19), (b) experimenters (n = 65), (c) at-risk youth (n = 63), and (d) abusers (n = 29) based on their reported substance use behavior at age 17.5. Parental behavior, peer competence, and problem behavior, measured from early childhood through age 16, were examined as predictors of substance use patterns. Multinomial logistic regression models revealed that early maternal hostility, externalizing behavior problems in first grade and at age 16

Effects of borderline personality disorder features and a family history of alcohol or drug dependence on P300 in adolescents.

Decrements in P300 amplitude have been associated with familial risk for alcoholism as well as several other psychiatric disorders characterized by disinhibited behavior. The present study examined the P300 in relation to Borderline Personality Disorder (BPD) features in adolescents with a paternal history of alcohol or drug dependence. One hundred and seventy-five males and females, aged 14-20, were assigned to groups based on BPD features (BPD+ vs. BPD-), family history of substance dependence (negative FH-, alcohol FHA, drug FHD) and gender. BPD features were assessed using the Structured C
